Home Health Nurse Practitioner Your role in this new full-time permanent career will be to support patients at home who are suffering from Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) by forming deep-rooted relationships and preparing them both emotionally and physically for the challenges of managing kidney disease. Experience with Palliative or dialysis patient care is a bonus. Expect a competitive salary starting at $110k and a 10% bonus based on experience.
