A Case Manager-Rehabilitation who excels in this role:
* Completes departmental orientation, initial and annual competencies.
* Assists with departmental specific performance improvement initiatives collecting and reporting data as requested by supervisor.
* As appropriate, consults other departmental staff to collaborate in patient care delivery, identify barriers to care and or discharge and develop solutions/resolution.
* Completes documentation per workflow timeline and content requirements including completion of the Individual Plan of Care (IPoC) per CMS guidelines.
* Schedules family conferences and/or communicates with caregiver following each team conference and more often as needed to keep patient and designated caregiver informed of progress and provides appropriate information related to goal achievement, course of rehabilitation stay, and plans for discharge.
* Coordinates weekly patient care team conferences to facilitate development, monitoring and refinement of treatment plan to achieve identified patient goals and outcomes.
* Reviews the patient's assigned CMG and helps the team identify any potential missed comorbid conditions that are actively being treated during the patient's stay. Communicates any findings to the HIM team.
* Communicates effectively with nursing, therapy and other ancillary departments to ensure proper utilization.
* If no Lead Case Manager, the CM participates as the facility representative for national CM Conference calls and communicates new information to the facility CMs.
* Assists with concurrent and retrospective utilization review activities including denials and appeals.
* Works with physicians to conduct peer review with payer medical director when indicated.
* Ensures clinical updates are provided to all insurance payers when due and all payer communications are documented in Meditech.
* Coordinates discharge planning needs including but not limited to; home health services, physician follow up care, durable medical equipment, medical supplies, healthcare services, outpatient therapy, dialysis, skilled nursing care, assisted living care, hospice care, private duty care, etc.
* Responsible for coordinating all patient care needs prior to discharge ensuring a safe thorough discharge plan.
* Ensures patient choice is offered and documented as per CMS' Conditions of Participation for Discharge Planning.
* Identifies trends that impact the quality, cost effectiveness, patient experience and delivery of care services and brings to departmental leadership meetings for discussion and action.
* Performs intake assessment on patient within 24 to 72 hours of admission, preferably within 48 hours.
* Performs follow-up assessments per Case Management Plan and/or hospital policy.

Your services support the Veterans Benefits Administration Medical Disabilities Examinations program. Veterans are scheduled for appointments at contracted facilities for compensation and pension exams and embeds practitioners in facilities as needs warrant. Dedicated facilities are in areas where there is a large Veteran population with the potential for high volume.
The compensation and pension exam helps the VA answer questions about Veterans' current health.
The practitioners will not decide the percent of disability or rating, develop treatment plans, or prescribe medication.
Completed documentation is sent to the VA for final determination on all disability claims.
Prior to the exam, the practitioner reviews a Veteran's medical or mental health history, using electronic bookmarks that highlight the portions applicable to the Veteran's claimed condition.
The practitioner is guided through a web-based examination form in LHI's Provider Portal to capture the Veteran's responses during the exam.
Electronic exam documentation is known as a Disability Benefits Questionnaire (DBQ) and must be submitted promptly within 48 hours of each appointment.
The number of DBQs completed per exam will vary based on the Veteran's claimed conditions.
Administrative time is provided to complete exam documentation and ensure timely submission.
The documentation is reviewed by a team of medical professionals to ensure contract compliance and guarantee the verbiage aligns with customer requirements.
Requests for changes or corrections to exam documentation must be submitted within 48 hours of the appointment.
Review pertinent medical history documentation and provide one-time, non-treatment compensation and pension exams for Veterans.
The Practitioner will capture responses during the exam through a web-based examination form; all documentation will be completed electronically.
Completed exams are sent to the U.S. Department of Veterans Affairs for disability award determinations.
General Medical take 45 minutes to several hours, depending on number of disabilities being claimed; Practitioner will be provided an approximate appointment length estimate for each scheduled exam, based on the Veteran's claim(s)

12T Technical Engineer

Technical Engineers pen the plans that lead construction crews to success. As a Technical Engineer in the Army National Guard, you will develop the skills necessary to help build, plan, and repair airstrips, docks, barracks, roads, and other projects.
Your training will enable you to assume duties that include construction site development, such as technical investigation, surveying, drafting, and the development of construction plans and specifications. Specific duties may include: construction material testing; drawing maps, charts, and making scale drawings of roads, airfields, and buildings; conducting land surveys and computing survey results; preparing structure wiring and plumbing diagrams; building scale models of land areas that show hills, lakes, roads, and buildings; and piecing together aerial photographs to form large photomaps.
Job Duties
* Draw topographic maps and charts using Computer Aided Drafting systems and software
* Conduct geodetic and construction surveys and results utilizing Automated Integrated Survey Instruments and Global Positioning Survey Technology
* Provide technical support for vertical and horizontal construction projects

Job training for a Technical Engineer consists of 10 weeks of Basic Training, where you'll learn basic Soldiering skills, and 17 weeks of Advanced Individual Training, depending on the specialty. Part of this time is spent in the classroom and part in the field.
