PURPOSE AND SCOPE: Supports the mission, core values and customer service of Home Dialysis Services & Dialysis Care Center, LLC (HDS/DCC). The Home Hemodialysis Patient Care Technician is responsible for providing exceptional hemodialysis care to patients in homes, under the supervision of RN assigned to the patients. PRIMARY DUTIES / ACTIVITIES: •Monitor vital signs (temperature, pulse, etc.) and patient condition, and document observations appropriately. •Take fluid samples or other specimens for testing and prepare for shipping. •Assist nursing staff in administering basic treatment, i.e. hemodialysis therapy. •Monitor and document status of patient and instruct patient in vascular accesses etc., under the direction of RN. •Review lab work, physician orders and reinforce treatment care with patient. •Enter appropriate ICD-9 codes for lab draws into the computer. •Perform basis nursing procedures (e.g. changing bandages). •Ensure patients have adequate patient care supplies, monitor and maintain inventories. •Prepare the dialysis equipment before treatment and care for the equipment after treatment. •Complete flow sheets (via EMR) to verify patient billing. •Comply with company policies and procedures in accordance with state and federal regulations for patient care and services (CMS, HIPAA, OSHA, and other governing agencies). •Participate in quality assurance and cost containment programs. •On- call responsibilities. •Other duties, as assigned. 2 CUSTOMER SERVICE: • Responsible for supporting the values and customer service standards of HDS/DCC. • Accountable for outstanding customer service to all staff, clients and vendors • Develops and maintains effective relationships through effective and timely communication. • Initiates action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner. Track ALL equipment, putting together a log with their serial numbers. Coordinate with nurses and technicians when assistance is required with designated scheduled programing. Repairs and maintains dialysis machines, R.O. water systems and provides support documentation. Performs electrical safety assurance and performance testing on all dialysis machines and related electrical equipment Monitors R.O. system and log sheet, on a monthly basis, for completeness and accuracy, noting any deviations from normal standards Performs monthly biomedical audit and documents findings to ensure that dialysis staff are following company policies and procedures. When notified, participates in on-site ESRD surveys and inspections by state and federal surveyors Participates and presents as a member of each facility's Quality Assurance Performance Improvement (QAPI) Complete Biomed section of QAPI log Orders and maintains appropriate parts inventory for all equipment serviced. Help with stocking and arranging treatment supplies in stock room. The Dietitian provides nutritional services to patients in order to maximize the patients' well-being and improve outcomes. Works as part of an Inter-Disciplinary Team to maximize patient care and support the company to meet the goals of the QAPI activities. PRIMARY DUTIES / ACTIVITIES: Monitor subjective and objective data to determine the need for timely intervention and follow-up. Review patient lab work and make individualized recommendations/adjustments to diet and bone medications. Recommend therapeutic diets in consideration of cultural preferences and changes in treatment based on the patient's nutritional needs in consultation with the patient's physician. Counsel a patient, patient's family, and a patient's significant other on prescribed diets and monitoring adherence and response to diet therapy. Referring patients for assistance with nutrition resources such as financial assistance, community resources or in-home assistance. Follow federal and state regulations. Collect objective and subjective patient data to assess nutrition status. Responsible for a comprehensive nutrition assessment with an educational component within 30 days from patient admission to dialysis facility Conduct a nutrition reassessment annually or more often if indicated. Participate in monthly clinics with the patient and family members to review the prescribed diet and dietary compliance and make adjustments to bone medications per protocol and in conjunction with the Physician. Available to answer questions and discuss problems with the dialysis patients when excessive weight-gain occurs, or when other instances of suspected dietary non-compliance occurs. Available for staff education regarding the nutritional care of the patient. Participate in monthly interdisciplinary patient care conferences. Assure that follow-up progress notes are written in each patient's records each month. Perform regularly-scheduled evaluations of standard menus as necessary for renal patients, revising and/or updating menus as necessary, and participates in monthly interdisciplinary patient care conference. Document patient's care plan and long-term program. Will be available for conferences with salesmen concerning products that could be used in providing nutritional care for renal patients and makes recommendations for possible purchases. Monitoring vital signs during treatment and taking patient vitals pro and post dialysis; blood pressure, temperature, weight and documenting it on the Hemodialysis Treatment Record Perform vascular access evaluation pre-treatment and report unusual findings to RN immediately Perform cannulation of patient's vascular access or performing catheter care, as allowed by state law. PRIMARY DUTIES / ACTIVITIES: Supervises, mentors the Patient Care Technicians (PCTs) and directs the day-to-day patient care. Reviews new staff's documentation for accuracy. Supervises and performs direct patient care including but limited to taking and monitoring vital signs, pre-evaluation, initiation, monitoring, termination, access homeostasis, and post evaluation of dialysis Responsible for documentation, physical nursing assessment of patients, administering dialysis Medications, transcribing physician orders assessing dialysis adequacy, drive catheter reduction. Assist with patient scheduling. Monitoring and reviewing lab results, ordering and following up of labs through Ascend, ensures correct lab tubes are utilized for prescribed lab samples, ensure that all labs are packaged for shipping correctly, Reports alert/panic and abnormal lab results to physician. Maintains competency with all emergency operational procedures. Ensures verification and availability of adequate emergency equipment.
