Primary Duties:
Work closely with Community Health Advocates at MedStar Good Samaritan Hospital to address social needs of Food Rx patients. On site Wednesday, Thursday, and Friday.
Work closely with providers in the Collaborative Care Program outpatient clinic as part of the interdisciplinary team to identify eligible patients for the Food Rx program, coordinate enrollment and follow-up, and provide MNT/DSME consults. Establish and serve as Lead RD for the food pantry at The Franciscan Center in Baltimore, MD.
On site Mondays and Tuesdays. Provide Diabetes education, nutrition counseling, weight management strategies, and meal planning to patients within the Food Rx Program.
Work with and report to the Regional Clinical Manager for Food Rx.
Incorporate evidence-based strategies into the population health management of patients with complex medical and social needs, especially patient populations with food insecurity and uncontrolled diabetes.
Other responsibilities, as assigned.
Qualifications:
Bachelor's degree in nutrition/dietetics required.
Master's degree required if credentialed after 12/31/2023.
Registered Dietitian in state of Maryland required, or eligible to obtain MD licensure required.
CDCES certification preferred.
CDR registration required.

Duties and Responsibilities:
* Assembles accurately meal trays, nourishments, floor supplies, tube feedings and snacks for patients or cafeteria customers. Delivers regular and late trays within the standard time frame.
* Complies with departmental polices to include polices on break times, meal times, eating in kitchen, labeling, dating foods items, uniforms, etc. Consistently wears I.D. badge. Demonstrates a professional appearance.
* Maintains safe and sanitary work conditions. Completes cleaning assignments in trayline area, dishroom and delivery of carts.
* Cleans and assembles sanitized silverware into bags and prepares bread in bags for next meal period.
* Promotes and enhances positive customer relations with an emphasis on good customer service, demonstrates courtesy when speaking to patients/visitors and demonstrates professional behavior at all times.
* Sets up trayline stations with an adequate amount of supplies (i.e. food, beverages, supplement, salads, condiment, etc.) to begin trayline on time with no need to restock during meal service. Replenishes trayline stations following meal adequately for next meal service.
* Delivers patient trays to room by following standard procedures outlined:
a. Knock on door prior to entering room
b. Greet patient by name, introduce yourself, smile and establish eye contact
c. Verify room number, diet and patient before giving tray to the patient using established patient Identifiers
d. Place tray on bedside table, wheel table to appropriate place for patient to eat
e. Retrieve trays from all patient rooms and check pantry areas for stray trays on all units within 45 minutes after tray delivery
f. Check all units delivered to a second time for missing trays. Communicate trays left on units to Supervisor.

Develop and maintain a good working rapport with inter-department personnel, as well as with other departments within the facility to assure that food service can be properly maintained to meet the needs of the residents.

Staff Development:
Participate in and assist in departmental studies and projects as assigned or that may become necessary.
Attend and participate in workshops, seminars, in-service training programs, etc., as directed.
Dietary Services:
Serve meals that are palatable and appetizing in appearance.
Assist in serving meals as necessary and on a timely basis.
Serve food in accordance with established portion-control procedures.

Remove food trays from carts, dining rooms, etc., and take to dishwashing area.
Prepare and deliver snacks, etc., as instructed.
Safety and Sanitation:
Prepare food, etc., in accordance with sanitary regulations as well as with our established policies and procedures.
Follow safety regulations and precautions at all times.
Follow established Infection Control and Universal Precautions policies and procedures when performing daily tasks.
Ensure that the department is maintained in a clean and safe manner by assuring that necessary equipment and supplies are maintained.
Report all hazardous conditions/equipment to your supervisor immediately.
Report all accidents/incidents to your supervisor on the shift in which they occur.
Assist in maintaining food storage areas in a clean and properly arranged manner at all times.
Dispose of food and waste in accordance with established policies.

Overview of the Role
The Nutrition Site Aide is responsible for the management of the meals for food safety and compliance. In addition, this position will provide client intakes and screenings and monitor reservations and contributions. This position is contingent upon continued grant funding.
Major Responsibilities/Activities
Oversees food service to ensure proper portion control.
Oversees the contribution collection process to ensure adherence to policies and procedures governing participant contributions.
Provides oversight of the site when the Nutrition Recreation Manager is absent.
Registers all nutrition site participants by completing the DC Office on Aging client intake form and nutrition screening and updates the forms annually.
Ensures data is entered correctly into CSTARS.
Keeps meal and activity attendance records, including meal quantity and quality. Prepares reports as required.
Maintains adherence to the standard meal reservation process for ordering meals.
Receives and inspects food delivered daily.
Follows food handling intake and procedures.
Makes sure all serving and food consumption areas are clean.
Monitors sanitary practices of volunteer food.
Must be a certified food handler.
Clear communication skills, including writing, verbal communication, and customer.
Strong attention to detail and the ability to meet deadlines is required.
Must be courteous and respectful to clients at all times.
Key Qualifications
Possession of high school diploma, GED, or equivalent experience
Food Handler Certification required
CPR Certification
Proficient with Microsoft Suites Application
Must be knowledgeable of issues and challenges that impact older adults.
Must be sensitive, courteous, and respectful to the needs of older adult participants.
