Job Title: Registered Dietitian

Seeking a Registered Dietician to join The Residence of Kenilworth Park team and support our residents. As a member of our Dietary Team, the Dietician provides caring and compassionate services to our residents through nutrition education and assessment of individual nutritional needs.
Duties and Responsibilities:
Perform nutrition assessments, annual and quarterly reviews, interim assessments for residents with compromised nutritional status.
Participate in care plan development, meal observation, and resident/family, visitation.
Provide nutrition education: Advise patients and their families on nutritional principles, dietary plans, diet modifications, and food selection and preparation. Counsel individuals and groups on basic rules of good nutrition, healthy eating habits, and nutrition monitoring to improve their quality of life.
Care Planning: Assess nutritional needs, diet restrictions and current health plans to develop and implement dietary-care plans and provide nutritional counseling. Consult with physicians and health care personnel to determine nutritional needs and diet restrictions of patient or client. Attend resident care planning meetings as needed; be an active member of the Interdisciplinary Team and participate in pertinent meeting as needed.
Clinical focus: Assess resident weight changes and provide a written report to Nursing Services on a monthly basis: Upon notification from the Nursing staff of weight change, the Registered Dietitian shall assess weight change/significant weight change and implement appropriate interventions; attend weight risk meetings.
Staff Education: Conduct in-service Education for the facility staff, participate in new employee orientation.
Serve as a resource to the Foodservice Department.
· Be familiar with and follow protocols, policies and practices established by HallKeen Assisted Living, the Food Service Director, ServSafe, Board of Health and any others as applicable.
· Perform other duties as requested or assigned.

INTRODUCTION
Unity Health Care provides comprehensive health care services at the DC correctional facilities. As part of Unity's responsibilities, they are required to provide evaluation and assessment of medical diets provided by the correctional food vendor. They also have a small number of patients with specific conditions who need nutrition assessment and counseling. A part-time registered dietician nutritionist is needed to fulfill these functions for approximately 2 hours/week or 8 hours/month.
MAJOR DUTIES Summary of Responsibilities
1. Diet/menu evaluation
§ Evaluate all diets served at DC correctional facilities for nutritional adequacy, at least annually or whenever the menu is changed substantially.
§ Provide a report of the evaluation findings.
§ Monitor the effectiveness and appropriateness of therapeutic diets served at DC correctional facilities
2. Nutritional Instructions & Education:
§ Provide tailored nutrition education to patients receiving dialysis treatment

- Provides nutrition assessments and interventions to living donor candidates as requested by physicians.
- Provides ongoing nutrition strategies for complex, high risk patients.
- Develops and review guidelines used for nutritional assessment and intervention of pre and post-transplant as well as for living donors.
- Reviews medical documentation and oversees nutrition care provided.
- Follows up with high risk patients as needed/requested.
- Demonstrates good organizational skills and be able to multi-task, prioritize, and quickly adjust to changes when needed.
- Accesses information as needed from all data sources (data base, electronic charts, outside providers, etc.).
- Attends all departments-specific training and demonstrate proficiency related to safety and job-related hazards.
- Participates in at least one transplant related performance improvement project.
- Participates in monthly QAPI meetings.
- Participates in weekly patient selection meetings.
- Effectively communicates expectations.
- Reviews or develops educational material for transplant candidates, recipients and living donors.
- Documents nutrition assessment in the patient's medical record.

Skills and Requirements
Must Haves:
- Bachelor's Degree in Nutrition / Dietetics.
- Registered Dietitian / Nutritionist (RDN).
- Washington DC Dietitian License.
- 1-2 years' experience required.
Ideally transplant, inpatient, or dialysis center experience. Skills:
- Effective communication skills, both verbal and written.
- Effective interpersonal skills.
- Ability to maintain confidentiality.
- Ability to adapt to multiple and changing priorities.
Other Information (Team & D2D:
- This team member will report directly into the Director of Transplant and collaborate closely with the other dietitian on the team.
- The dietitians on the team alternate monthly between inpatient and outpatient facilities and responsibilities.
Inpatient D2D: rounds @9, educating patients that are being discharged on timeline and next steps, perform liver evaluation and assessments, documentation, and assisting with TPN / tube feeds.
Outpatient D2D: consulting for frailty, ensure candidates are eligible for liver / kidney transplants, assist with hematology cases by educating patients.
