Responsibilities*
Support the daily operations of the Milk Room to ensure optimal safety for all patients receiving prepared products from the Milk Room. Perform Milk Room operations including: Handling of breast milk, verifying and preparing all breast milk and/or formula feedings per physician orders, delivering prepared products, maintaining a sanitary environment in the preparation area, inventorying Milk Room stock and equipment, and maintaining proper confidentiality and security of all information and data used in the scope of the position. Contact appropriate allied health staff (Lactation Consultants, Nursing etc.) should parents need assistance with lactation and breast milk issues.
Required Qualifications*
* Associate's degree granted by a U.S. regionally accredited college or university.
* Extensive daily heavy lifting, pushing of carts, walking, and standing in the Milk Room and in patient care units is required to perform the duties of this job.
Desired Qualifications*
* Experience and knowledge of proper handling of infant formula and breast milk for preparation and fortification. Knowledge of newborn and pediatric nutrition.
* Experience and knowledge of aseptic techniques.
* Highly functional abilities with technology such as e-mail, computerized ordering systems, word processing, and nutrient analysis software.
* Ability to work independently in an ever-changing and potentially stressful environment.
Work Schedule
This position requires the temporary employee to work at a minimum of one 8-10 shift per week, with the opportunity to work up to 40 hours, dependent on operational needs.

PRINCIPLE RESPONSIBILITIES/ESSENTIAL FUNCTIONS:
Provide comprehensive nutritional services to consumers and case managers navigating home delivered meals menu and program primarily via phone; may include screening, menu checking/modification, ongoing nutrition intervention and diet education
Mange Choice Meal/Medically Tailored Meal plan program - complete program eligibility assessments, work with consumers and case managers to customize meals plans based on dietary needs, work with consumers to update orders, prepare report to be processed by QA Coordinators and Kitchen Operations
Responsible for interviewing new and current home-delivered meal clients, assessing current clients for additional support needs, and determining client eligibility to receive LifeCare Alliance services and/or enrollment into project over the phone and in the home. This includes scheduling and interviewing clients on phone and in the home, completing appropriate applications and paper work; determining funding eligibility and services for which client qualifies; providing alternate sources to those ineligible; providing follow-up with agency staff; and documenting client changes and concerns.
Responsible for working closely across departments, attending and contributing to cross department meetings.
Collaborate with the internal assessment team to address observation reports and to process internal referrals to other LifeCare Alliance departments.
Oversee nutrition education outreach (bi-annual) and temperature monitoring (quarterly) for home delivered meal and congregate program.
Provide friendly, efficient, and professional telephone, email, and in-person customer service support to customers, case managers, drivers, kitchen staff, and other agency personnel.
WORKING CONDITIONS:
Work in agency office except when completing in home assessments throughout Franklin County. Weekly travel should be expected.
Generally works eight scheduled hours between agency opening and closing, Monday through Friday, generally 8am-5pm. Some remote work may be approved, but majority will be in office.

QUALIFICATIONS:
Minimum Qualifications: Must be a graduate of an approved dietary manager's, diet technician, or dietitian's course that meets the state regulations. Process the Missed Text Reminder Report. Performs anthropometrics and hematological data collection including finger sticks.
Obtains health assessment data and provides nutrition consultation to pregnant, postpartum, and breastfeeding women, infants, and preschool children.
Restock nutrition education materials.
Assist in the development of nutrition education materials/classes (bulletin boards, PowerPoints, handouts, lesson plans).

Purpose of Your Job Position
