Job Summary: Sanford Federal, Inc. is looking to hire dietitian/nutritionist to provide dietitian services at the Clifton T. Perkins Hospital Center.
Qualifications:
· Must possess a current license as a Dietitian/Nutritionist from the Maryland Board of Dietetic Practice
· Must be registered by the Commission on dietetic Registration (CDR) and must maintain a current registration during their time of employment.
· Minimum experience of one year as a Dietitian/Nutritionist.
Duties:
· Provide Dietitian Food Service operations for the patients at the hospital.
· Provide Professional Clinical Nutrition care services for the patients at the hospital.
· Apply nutrition principles in patient nutritional care and make sure the services are performed in accordance with the individual treatment plans developed by health care professionals and under the standards and policies of the Dietetic program of the health care facility.
· Plan and execute clinical nutritional care services, such as nutritional screening, nutritional history and assessment, and nutritional education and counseling for assigned patients of a state health care facility.
· Consult with medical and other professionals concerning individual food habits and needs of assigned patients/clients.
· Plan menus for modified diets based on indicated physiological and psychological needs of patients/clients
· Monitor meals served for conformance to diet prescriptions and acceptance by patients/clients.
· Participate in medical staff conferences, rounds, and consultations
· Document pertinent nutritional information on patient/client medical records.
· Counsel patients/clients and others on the requirements and importance of prescribed modified diets and how to plan and prepare food.
· Assist with the evaluation of dietetic program operations using quality assurance criteria to assure compliance to the dietetic program standards for food purchasing, preparation, service, preservation, sanitation, and safety.
· Conduct educational and in service training sessions on nutritional standards in food service and dietetic practice for professional, paraprofessional, and other support staff assigned to a dietetic department or involved in patient/client care.
· Maintain records and prepare reports.

Provides medical nutrition therapy for clinic medical patients and/or high-risk Women, Infants, and Children (WIC) clients. Functions as an interdisciplinary healthcare team member to assess and treat medical conditions and address individual nutrition needs.
Essential Functions/Responsibilities/Duties
Conducts nutritional screening and assessment for patients/clients to include anthropometric and laboratory data, collection of nutrition history, calculation of nutrition adequacy, and identification of drug-nutrient interactions. Utilizes motivational interview to identify desired goals and outcomes of nutrition care in collaboration with patient/client and health care professionals. Considers the variables that may influence the patient/client s food related behaviors including culture and social determinants of health. Evaluates and provides evidence based nutrition recommendations to address patient/client medical needs. Understands and evaluates patient/client nutrition status and its relationship to complex disease states including diabetes, cardiovascular disease, chronic kidney disease, gastrointestinal disorders, cancer, food allergies and obesity. Collaborates with physicians and other health care professionals to advocate for and implement optimal nutrition care. Provides preventative services and counseling at individual patient/client visits and group meetings. Serves as preceptor providing nutrition education and guidance to dietetic interns and students. Develops training materials and participates in mentoring of staff. Assists in monitoring compliance to Yakima Valley Farm Workers Clinic (YVFWC) procedures, federal qualified health center quality metrics and/or state and federal WIC requirements. Maintains awareness of community demographics and resources. Connects clients to resources. Designs and implements outreach activities to raise community awareness of program services. Completes documentation in the appropriate electronic medical record to document patient/client interaction. Qualifications
Education: Bachelor's Degree in Dietetics from a U.S. regionally accredited college, university, or foreign equivalent. Completion of ACEND (Accreditation Council for Education in Nutrition and Dietetics) accredited academic coursework in a Didactic Program in Dietetics. Experience: One year s experience in clinic, primary care, outpatient or community setting preferred. Professional Licenses/Certificates/Registration: Registered Dietitian or registration eligible (must obtain RD credential within 180 days of employment). May require the following certifications based on location and job duties performed. Medical Assistant - Registered (WA WIC Dietitians). Certified Dietitian (if going to bill for Maternity Support Services [MSS]). Licensed Dietitian (if going to bill for Medical Countermeasures ([MCM]). Knowledge/Skills/Abilities: Bilingual (English/Spanish) preferred. Must demonstrate the ability to communicate at level 9 on the language proficiency scale to receive bilingual differential pay. Responsibilities
Patient Care
Screens patients for nutrition risk.
Conducts a nutrition assessment for patients at nutrition risk and for dietitian consults. Obtains timely and appropriate data and analyzes/interprets data based on evidence-based standards.
Assesses patients for malnutrition through nutrition focused physical exams and other nutritionally relevant patient data.
Nutrition Order writing privileges per CMS guidelines, state regulation, and hospital policy.
Practices standardized nutrition charting through the nutrition care process.
Develops and implements an individualized plan for nutrition intervention in accordance with the patient's medical program goals and objective, nutrition prescription, and self-management training. May require development of a therapeutic plan for specialized nutrition support, including enteral and parenteral nutrition.
Utilizes assessment and evaluation techniques that consider the varied needs of age-specific populations as well as cultural, religious and ethnic concerns.
Evaluates the effectiveness and measures progress towards outcomes of medical nutrition therapy interventions. Reassesses nutrition care process and implements changes as indicated.
Communicates with the medical team through interdisciplinary meetings, rounds, and direct dialog.
Provides appropriate documentation, in the electronic medical record, that summarizes the nutrition care plan including nutrition assessment, diagnosis, plan, implementation, and progress toward goals.
Works cooperatively with food service staff to assure conformance to diet prescription.
Demonstrates accountability for the proper use of patients' protected health information.
