Essential Duties and Responsibilities: Conduct in-person crisis assessments using NC-SNAP or ASAM Criteria to determine eligibility and medical necessity for MCM services. Provide therapeutic interventions, including de-escalation, counseling, and crisis planning, to stabilize beneficiaries in community settings. Develop and update Person-Centered Plans (PCPs) in collaboration with beneficiaries, families, and other stakeholders, ensuring goals address crisis needs (Subsection 5.7.1, page 7 of NC Medicaid Clinical Coverage Policy 8A). Supervise Qualified Professionals (QPs), Associate Professionals (APs), and Paraprofessionals, providing clinical oversight and ensuring interventions align with the PCP (Subsection 5.6, page 6 of NC Medicaid Clinical Coverage Policy 8A). Coordinate with Local Management Entity/Managed Care Organizations (LME/MCOs), emergency departments, law enforcement, and other providers to link beneficiaries to follow-up services. Document all interventions in service notes, including date, purpose, intervention details, duration, effectiveness, and beneficiary progress, per Subsection 5.8.2 (page 8 of NC Medicaid Clinical Coverage Policy 8A).
