Provider Education Coordinator would have a Bachelor's degree in a related field and at least 5 years of Medicare Advantage or Commercial risk adjustment experience including at least 4 years of experience in a HCC Coding Audit or Network Management role or equivalent combination of education and experience.
CPC and/or CRC credentials preferred
Skills and Attributes:
Strong consulting, communication (written and verbal), influencing, and facilitation skills
Proficiency in using Microsoft Office tools and proven ability to display analytical data and research findings for effective presentations and improved decision making.
Knowledge of health care industry trends, provider relations, and risk adjustment HCC coding, ICD10 diagnosis coding.

What You Will Do at Cambia:
Develops, coordinates, and drives risk adjustment education initiatives with network providers and acts as primary point of contact for assigned key provider engagements across Commercial and Medicare lines of business
Coordinate outreach, communication, training, and education for providers that have been identified through various operational reporting channels as a candidate for provider education engagement
Works with key leaders within the provider organization to engage with and drive successful performance of the provider engagement.
Assists senior leadership with special projects related to advancing Cambia's value based and innovative provider relationships.
Participates in definition of project deliverables, providing consultation, recommendations, and solutions, reviewing project mandates and directives to determine overall provider engagement approach
Leads, under the direction of management, in design and implementation of initiative related administrative policies and procedures and provides guidance and direction to the work team

The expected hiring range for a Provider Educator is $83,300.00 - $111,600.00 depending on skills, experience, education, and training; relevant licensure / certifications; performance history; and work location. The bonus target for this position is 10%. The current full salary range for this role is $78,000.00 to $128,000.00.

Provide quality care for all stages of client life and in settings outside of the client home as required.

Understand and regularly review Positive Behavior Support Plans (PBSPs) and Functional Assessments (FAs) for applicable clients. Follow strategies identified in the client's PBSP in response to their challenging behaviors.
Follow support plans in regards to medication administration, and document medications in the Medication Administration Record (MAR).
Support clients to complete ADLs as independently as possible. Assist and supervise clients with daily personal care (e.g., bathing, toileting, etc.) to ensure their safety and well-being at all times. Assist clients in carrying out basic household chores (e.g., cooking, cleaning, doing laundry, etc.).
Support clients' access to the community by referring to activity calendars and allowing flexibility for scheduled and unscheduled outings. Assist clients with community interactions. Be professional, and lead by example.
Establish and maintain positive communication, working relationships, and professional boundaries with clients, guardians/family members, healthcare practitioners, community members and co-workers. Keep the Area Manager apprised as issues arise.
Assist clients with financial transactions as directed by supervisor or Assistant Site Manager. Keep receipts for all client purchases, and document appropriately.
Assist clients in accessing appropriate transportation, either by agency vehicle or by public transportation (note: staff are required to use personal vehicles - with active insurance and updated vehicle inspection when appropriate agency vehicles are unavailable).
Help keep agency vehicles clean and in good repair by reporting any damage or problems to the Area Manager.
Assist and instruct clients in emergency evacuation procedures. Conduct monthly apartment safety checks.
Read information boards and Therap SComms/T-Logs/ISP data, check box for notes, and exchange pertinent information with staff during shift change.
Complete all documentation on assigned clients prior to leaving shift. Documentation includes, but is not limited to, MARs, ADLs, Intake/Elimination Records, IISP goals, T-Logs, etc.
Report unusual or emergent situations to the Area Manager. Complete Therap General Event Reports (GERs) within 24 hours of the incident for management review.
Immediately report all alleged or suspected abuse/neglect/exploitation to Residential Care Services (Complaint Resolution Unit) and the Area Manager.
Safeguard the confidentiality of client records and information (see Confidentiality Pledge).
Participate in all designated in-service trainings and attend all staff meetings, as directed by the Area Manager.
Punch in and out of Paycom at proper times to ensure time worked is reported correctly. Submit timecard, verifying its accuracy by due date. Communicate to management any discrepancies between time worked and schedule.
Abide by Cascade Connections' leave request policy if unable to work a regularly scheduled shift. (All leave requests approvals are at the discretion of the Area Manager based on needs of the program).
Perform other duties as assigned by management.

GENERAL FUNCTION:
Direct Support Professionals (DSP) work directly with adults with developmental disabilities in a residential setting and assist clients with activities of daily living. DSPs provide support and training to the program participants by assisting them in the core service areas of health and safety, personal power and choice, positive recognition by self and others, community integration, constructive relationships, and building skill competence.
RESPONSIBILITIES:
CLIENT SUPPORT
Support clients to help achieve their PCSP and IISP goals and objectives, collect and record data in accordance with department guidelines.
Instruct and support clients in daily living skills (daily cleaning, laundry, basic repairs, and yard work as needed), ensure basic health and hygiene needs are met, support with meal preparation, leisure activities,
community living and integration skills, assist clients in caring for their pets (where applicable), and ensure all related tasks are being completed daily.
Observe and monitor clients and document their activities and any incidents or difficulties on appropriate forms per program guidelines and client plans. Report any changes (mental, physical, emotional, or behavioral) to appropriate personnel and document it in client records accordingly.
Support clients by providing transportation to the client to access the community.
May be required to work alone with clients who may exhibit challenging or aggressive behavior. With the training provided, DSPs will learn how to respond to such behavior and deescalate situations.
Accompany clients to recreational activities, doctor appointments, IISP goal outings or work.
Assist clients with money skills and purchases/shopping.
Support clients with medications per their individual plans. Ensure all delegated tasks are followed per RND instructions and follow all doctors' orders. Report any problems or difficulties with medications or treatments to the supervisor immediately.
Peri-care and assistance with personal grooming as needed.
TRAINING
Complete 75 Hour LTC Worker Training and remain in compliance with Continuing Education requirements annually.
Attend regular state and agency mandated trainings as scheduled and stay current on all trainings.
Prior to driving clients, must attend agency driver training.
Attend and participate in monthly staff meetings.
COMMUNICATION
Maintain excellent communication with other staff and Program Manager regarding all aspects of client care, household operations, personnel, and VOA/DS business.
Appropriately maintain professional caring relationships with the clients while preserving healthy professional boundaries.
Perform other duties as necessary or assigned.
PHYSICAL DEMANDS AND WORKING CONDITIONS:
While performing the duties of this job, DSP is frequently required to read, speak, stand, walk, and sit.
Must be able to fulfill all documentation requirements, including documenting on the computer.

Required to physically assist some clients when transferring, bathing, toileting, and ambulating. Staff may need to physically lift or steady clients using approved methods.
