Main Duties & Responsibilities
Performs history and physical on new/return patients throughout the lifespan.
Assess, plan, and treat patient needs within an urgent care setting and triage patients to differentiate between immediate and less urgent care.

QUALIFICATIONS
Education
M.S. in Physician Assistant Studies, MSN or DNP from accredited nursing school, OR MD/DO
For MD/DO candidates - Completion of residency training in Family Medicine or Pediatrics and Board certified or Board Eligible in Family Medicine or Pediatrics.
For Advanced Practice Providers - Active PA or APRN license in Illinois and Board Certified. Must maintain an active collaboration practice agreement with an Erie Physician if does not have Full Practice Authority.

Responsibilities
Complete required data entry and paperwork in accordance with program and agency policies and procedures, including but not limited to LOCUS, Health Risk Assessment/Physical Health Assessment (HRA/PHA,) IM-CANS including applicable modules & other applicable assessments, progress notes and any other assigned requirements:
100% of IM-CANS to be completed prior to due dates and,
Progress notes and any other required data entry or paperwork submitted within forty-eight hours of the delivery of the service.
Provide services identified in the IM-CANS to meet consumer needs that are consumer driven and meet the program requirements, including:
Maintaining a minimum of assigned consumer service levels,
Completing pre-authorizations and re-authorizations of services, timely and as needed,
Services primarily to include IATP, therapy, community support & medication training and monitoring; and services to support systems; including maintaining a regular group and individual consumer service schedule,
Providing services in the location most conducive to the consumer's level of change, including his/her apartment, or another community location,
Linking to additional clinical resources when needed and
Completing clinical screenings of new tenant housing referrals, when assigned.
Promote the recovery model of treatment including trauma-informed care, hope, respect, empowerment, health/wellness and spirituality/connectedness by assisting consumers with regaining control over their own lives through self-directed recovery; including utilizing recovery language in all applicable documentation.
Promote positive interactions within Chestnut and with the community, especially with colleagues from substance use programs, natural support systems, hospitals, medical offices, court systems and government agencies, to sustain a favorable program reputation.
Attend meetings and consumer staffings, participate in program development activities and provide support and backup to other team members as necessary, including:
Attending and ensuring supporting documentation of 100% of monthly, required Rule 132 supervision if not a LPHA, and
Attending all mandatory Chestnut trainings and CEU requirements if applicable, utilizing Relias systems for the majority of elective trainings.

The Perioperative Aide at Riverside Healthcare plays a crucial role in supporting the surgical team by ensuring the operating room is prepared and maintained to the highest standards. This position is responsible for assisting with patient transport, equipment setup, and room turnover between procedures. The Perioperative Aide ensures a safe, clean, and organized environment to support optimal patient care. The Perioperative Aide, under supervision and/or according to established guidelines assists with the smooth operation of the Anesthesia Department by maintaining and providing equipment and supplies in inventory and to anesthetizing locations.
Essential Duties
Assist operating room (OR) staff with patient lifting, positioning, and transfers to and from the OR.
Prepare and clean OR suites, ensuring proper equipment, furniture, and supplies are available.
Maintain and organize anesthesia equipment, supplies, and drugs in OR rooms, OB, and travel carts.
Clean anesthesia equipment, scrub sinks, and mop areas between cases and at the end of the day.
Manage stock levels by restocking turnover carts, sterile water, saline, and other solutions.
Ensure proper handling and disposal of Sharp containers and biohazard materials.
Retrieve blood from the blood bank, and pharmacy orders, and transport specimens to the lab.
Support turnover between cases by terminally cleaning OR rooms and preparing rooms for high-priority and emergency cases.
Maintain hallways and lounges by keeping equipment organized and ensuring cleanliness.
Work with Environmental Services (EVS) to ensure waste removal and blanket warmer maintenance.
Participate in daily huddles, unit meetings, and process improvement discussions to optimize team efficiency.

At Unleashed, our kennel/daycare attendants supervise play groups, provide potty breaks, food, medication, and snuggle-time with dogs who attend daycare during the day and sleep overnight in our boarding suites or with the attendant in our "group boarding studio." This position requires sustained physical activity and a tolerance for barking and cleaning up urine, feces, and vomit. Our staff members must be active and on their feet for most of the day, have patience with loud and rowdy (but lovable!) dogs, communicate well with customers and staff-mates, and have a strong interest in canine behavior. An understanding of canine communication and body language is necessary to efficiently recognize and interrupt inappropriate social behavior or overly-rough play, and split dogs into appropriate play groups according to personality and play style. An interest in reward-based dog training methods is strongly encouraged.
