Collaborate with the HCBS Supervisor and HCBS Programmer to develop goals for the individual served.
Provide services to individuals based on their unique goals and behavioral care plan. This includes:
o Teaching individuals how to accomplish their goal (rather than completing a task for them).
o Using behavioral strategies that support individuals and address any behaviors that do not lead to their success.
o Motivate and encourage individuals to participate in their behavioral care plans and reach their goals.
o Participate in recreational, community, and social activities with those you serve, honoring the choices of the individual.
o Display and understanding of when it is appropriate to apply rights-restrictions, only as determined by the interdisciplinary team and when indicated in the individuals behavioral care plan.

Provide safe environments and instruction to individuals served regarding the practices that will help them reach their goals.
Provide care to individuals that help them present their best selves to the community. This may include helping with hygienic needs, choosing appropriate clothing, administering medication, and ensuring they have food options suitable for their dietary needs.
Be an advocate for individuals receiving services to have as much control over their own lives as possible. All individuals should be treated with respect and should feel empowered to live their life as independently as physically possible.

Document passing medication appropriately and accurately as well as schedule II medications.

II. This is a part-time/full-time, Para-professional level position.

1. The Secondary Sleeping SCLS will be available to the awake overnight staff in the event of an emergency that requires evacuation of individuals.
2. Provide a supportive and therapeutic atmosphere.
3. Follow the assigned work schedule.
4. Never leave individuals receiving twenty-four hour care unsupervised.
5. Attend staff meetings and scheduled in-service training sessions.

Education: High School Diploma or Equivalency/HiSet/GED
Experience: Human Service Experience is preferred but not required.
Driving: This position requires possession of valid Driver's License, legal driving privileges and ability in the State of Iowa and qualification with Mainstream Living's underwriter as an insurable driver. Exceptions to this may be made for select shifts, e.g., overnight shifts.
Other Certification: n/a
Computer: Basic data entry skills required.
Post hire: Must demonstrate competencies to complete internal training, including training to pass medication.

ESSENTIAL DUTIES AND RESPONSIBILITIES
This position is considered an essential health care worker position and in the event of an emergency may be required to work one-on-one with members to ensure their safety.
Assure that action steps contained in the individual plan are carried out; Incorporate members' service plan into daily routine by providing support, supervision and encouragement according to their established rehabilitation and recovery goals.
Assist in identifying, developing and facilitating opportunities that reflect the members' skills, needs, and interests.
Work with member's interdisciplinary teams and attend staffings as needed. (In SCL program: schedule and attend interdisciplinary team meetings. Develop, write, implement and update individual service plans, assessments, behavior support plans, individual goals and social histories)
Meet member nutritional needs as outlined in member supervision plan, including facilitating or completion of following menu, food portion and meal preparation.
Complete member service documentation in a timely, accurate, and thorough manner required to meet billing and productivity requirements.
Provide instruction to the individual as needed to improve understanding of medications, therapeutic intent and adverse reactions.
Link individuals to other support services as needed.
