Position Summary
The Day Habilitation Specialist (Day Hab Specialist) will assist individuals with developmental disabilities to maximize their personal and vocational potential in their center and community. The Day Hab Specialist provides directcare support and assists in the implementation and documentation of each individual's program plan. Day Habilitation Specialists for designated programs may be expected to support other programs based on staffing and attendance. Floating Day Hab Specialists will be assigned based on need. The following position standards apply to both regular and floating Day Hab Specialists.
Full time schedule: Monday - Friday 7:45am - 3:30pm
Salary Range:
Day Hab Specialists I (drivers)- $17.90 to $19.90 per hour
Day Hab Specialists II (drivers)- 18.41- 20.41 per hour
Note: The rates outlined above are contingent upon being deemed eligible to drive by our insurance carrier. Should you be deemed ineligible to drive and the program is able to make a non-driver accommodation, a lower rate of pay will apply.
Position Standards:
Delivers and documents the services, supports, safeguards, behavioral documentation and activities identified by ISP/CFA and the Day Habilitation plan. Completes the monthly note and ensure it covers the individual's response to service within the seven days of the following month for assigned individuals.
Performs individual specific personal care as required, including but not limited to assistance with toileting, transferring, changing needs, handwashing and oral care. Documents any finding on appropriate forms including but not limited skin integrity, bowel and repositioning charts.
Ensures active engagement and appropriate communication with staff and individuals.
Assumes responsibility for medication administration and appropriate documentation in an accurate and timely manner.
Develops a trusting and friendly rapport with individuals served while maintaining professional boundaries.
Fulfills mandated reporter role by interrupting and reporting suspected abuse to administration immediately.
Ensures that the center is kept safe, clean and decorated in an age appropriate and comfortable manner and that is kept in good repair inside and out.
Demonstrates proficiency in all goals and skillsets outlined in the NYS DSP Core Competencies, on which performance will be evaluated.
Responsible for providing transportation to individuals receiving services driving agency vehicles with a capacity up to 14 passengers.
Ensures all required trainings and certifications are current, and seeks further support and training where needed.
Consistently exhibits the qualities of a committed and professional employee including but not limited to: respect toward others, strive for excellence, teamwork and unity.

This facility is dedicated to recovering and rehabilitating dogs and cats who have suffered physical and emotional trauma and delivers medical and behavioral interventions to prepare them for adoption or return to the owner when appropriate.
AICAs support the behavioral and medical recovery and rehabilitation of animals in the ARC by providing medical treatments and behavioral enrichment and interventions, including during all animal care activities. AICAs interact with animals during their recovery to detect potential problems or changes in their medical or behavioral progress and communicate those effectively to clinical professionals, administer medications, provide daily care, engage in behavioral enrichment activities, and document/maintain records of all activities. One of the most critical functions of this role is serving as a member of a team of medical, behavior and sheltering personnel, all dedicated to working together to provide holistic care for all our animals.
Responsibilities:

Behavior 40%:
Engage in behavioral and enrichment activities, following enrichment program protocols. Recommend animal-specific changes to regimen, including frequency, duration, or type.
Assist in behavioral health evaluations, assessments, trainings, and other animal interventions.
Exercise and socialize animals and manage playgroups, under the direction and guidance of the ARC and CARE Behavior team. Abide by existing dog-walking programs and observe and report reactions with environmental events or human/animal interactions, and/or problems/changes in urination or defecation. Encourage healthy behaviors and interactions throughout, and reward animals appropriately.
Feed animals, provide clean water, and ensure a clean environment while interacting with animals to encourage and promote healthy behaviors, and detecting, logging, and reporting to the medical and behavior teams any irregularities or changes in animal nutrition, hydration, or behaviors (including responses to your interactions). Observing animals during these interactions is vital to the recognizing medical and behavioral changes/abnormalities early, maximizing the efficacy of potential treatments.
Monitor the comfort of shelter animals, looking for any signs of distress, anxiety, or physical ailments, and ensure that their physical environment is not resulting in discomfort (e.g., appropriate bedding (clean and fold laundry), space, adverse neighboring animals, including by maintaining the cleanliness of both animal enclosures and other workspaces that facilitate the arrival of new animals.
Always provide safe and smooth fear free handling of animals.
Monitor animal holding areas for appropriate music and/or auditory stimulation and report observations.
Medical 40%:
Ensure the welfare of animals in our care by attending to their physical and emotional health and comfort, following all ASPCA protocols.
Read and follow all kennel signage and treatment sheets regarding animal treatment and care.
Under the guidance of the ARC medical team, administer medication and medical treatments in a low stress manner, following all medication administration protocols.
Recognize potential medical issues and communicate effectively with medical teams so that they can quickly and effectively attend to animals.
Team Support 20%:
Send timely medical and behavior observation reports and actively engage with the team to implement interventions.
Work in partnership with other teams (Medical, Behavior, and across departments) to continually develop knowledge, and support continuity of care for the animals.
Collaborate with other ICAs, and attend and actively participate in daily ICA rounds, to support the continuity of care for the animals.
Integrate volunteers into daily care, providing guidance and coaching as needed; demonstrate respect and gratitude for volunteer support.
Transport animals, using low-stress animal handling techniques and following applicable organizational policies and protocols, between ASPCA facilities and to partner agencies.
Attend and participate in team meetings and training classes as scheduled.

The Position:

Resident Care Aide (RCA) Essential Job Functions
Ability to assist residents with Daily Living Activities as needed that may include meal assistance, grooming, medication management, showering, dressing/undressing, toileting, skin care.
Ability to move and walk quickly and steadily in an emergency situation.
Ability to assist with wheelchair transport of resident(s).
Ability to apply/utilize Personal Protective Equipment (PPE) as required.
Responsible for responding immediately to residents' requests.
Assists residents with the following Activities of Daily Living (ADL's), according to their plan of care, Individual needs and care preferences:
Bathing
Dressing
Eating
Exercise
Housekeeping
Meal Preparation
Assisting with Medical Supplies/Equipment
Positioning
Grooming
Toileting
Transferring
Personal Emergency Response
Routine Skin Care
Laundering of Clothing
7. Notifies Supervisor of changes in residents conditions or needs:
Receives report about status of residents from nurse prior to providing care.
Follows plan of care in providing daily living and personal care services for resident.
Informs nurse after providing personal care services and weighing residents and other services or tasks as assigned.
Maintains facility standards for resident care by:
Maintains knowledge of protecting rights of residents.
Ensures that all residents are treated with respect and kindness.
Reports to nurse in charge about facility equipment that needs repair, unsafe conditions in room or on the floor, or personal needs of residents.
Completes at least (12) hours of in-service programs annually to reinforce or enrich knowledge of resident needs and care.
Demonstrates respect for personal possessions of residents and helping maintain an orderly and pleasant environment on the floor for staff and residents.
Assists with service in the dining room for all meals as needed. Maintain facility standards for service and customer focus techniques.
Informs nurse responsible for the care of resident and document duties performed as necessary before leaving for break or end of shift.
Attends facility meetings, staff meetings and required or preferred in-services.
Generates and develops ideas which improve the quality of care for residents or increase position productivity and satisfaction.
Performs all position duties according to safety rules and practices required for this position and as required to ensure the general safety of staff, residents and visitors.
Exhibits the highest level of customer service towards residents, staff and visitors.
Consistently demonstrates the ability to respond to changing situations in a flexible manner in order to meet current needs, such as reprioritizing work as necessary.
Utilizes electronic timekeeping system as directed.
Arrives to work on time, regularly, and works as scheduled.
Recognizes and follows the dress code of the facility including wearing name tag at all times.
Follows policy and procedure regarding all electronic devices, computers, tablets, etc.
Supports and abides by Elderwood's Mission, Vision, and Values.
Abides by Elderwood's businesses code of conduct, compliance and HIPAA policies.
Performs other duties as assigned by supervisor, management staff or Administrator.
Qualifications
Resident Care Aide (RCA) Qualifications:
Eighteen (18) years of age required; high school diploma or equivalent preferred.
Completion of Resident Care Assistant (RCA) training on Relias. Twenty-two modules and scoring 80% or better on post tests within 21 days from start date.
Certificate of Completion will be maintained and placed in the staff members Human Resource file.
Completion of 5 days (40hrs) of orientation with the RCA Competency being completed before the staff member works on their own.
Staff members should be completing their orientation (40 hrs) and working simultaneously on the RCA Modules on Relias.
Staff members may work on their own after completing their (40 hrs) of orientation and the RCA Competency is completed upon completion of orientation. They can continue working on the Relias modules and post tests while working and must complete them within 21 days from their start date.
Resident Care Aide (RCA) Skills and Competencies:
Ability to verbally communicate well with residents, families and staff members.
Ability to read and understand task assignments, ability to report verbally and provide written documentation as required.
Display of genuine concern for the elderly; a dependable, patient and compassionate attitude towards residents. Respectful demeanor of residents rights and wishes.
Ability to cooperate and maintain respectful treatment with others.
Sense of responsibility, maturity, diplomacy and ability to work well with residents, families and other staff members.
Ability to perform assigned tasks to a high degree of excellence and cleanliness.
This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
