Essential Duties and Responsibilities Conducts a preliminary assessment for new referrals. In coordination with assigned Case Manager, assists with developing a plan of service, participates in continued stay reviews and personal future plans. Determines direct care service delivery staffing needs and arranges for the delivery of services through conducting interviews, hiring, supervising, producing payroll payments, evaluating, and discipline of Direct Care Worker. Participates in team meetings with family members, professionals, and case managers to implement procedures and programs that ensure regulation compliance. Coordinates schedules for both waiver and generic services for client that includes staffing of all alternative living services. Performs Medicaid billing payroll functions for Direct Care Worker on a bi-weekly basis and assures the integrity of that billing by approving time, attendance and time cards, reviews and approves direct care travel, and documents work performed. Monitors all in-home services on a regular basis that includes ensuring Direct Care Worker compliance with completion of progress notes, required paperwork, code of ethics, and meeting agency requirements for initial and continued employment. Ensures direct care staff is compliant in following positive behavior plans and completing reports as required. Completes in a timely and accurate manner monthly, quarterly, and annual reports and various other reports as determined by Supervisor. Provides training to new Direct Care Worker in general procedures and individual specific training. Reviews Direct Care Worker progress notes, inputs data into client workbook monthly, and creates quarterly reports that focus on recipient's progress in meeting goals and objectives. Counsels recipients and families regarding staffing issues, scheduling, and other issues under their scope of responsibilities, and is on call to assist with staffing emergencies. Responsible for monitoring client medication regimens, ensures staff is at all times aware of medications being used; monitors consumption of medications on at least a monthly basis; ensures screenings are performed at a frequency determined by the prescribing physician, and any additional requirements as determined by DDS rules and regulations. The Direct Care Supervisor will serve as a liaison between staff and the Physician regarding medication. Prepares appropriate paperwork for new plans, continued stay reviews and revisions in accordance with DDS standards. Works with Case Management entity to assist and act as liaison for recipients with professional services to include, but not limited to: early intervention coordinators, doctors, public schools, case management agencies, counseling services, Human Services, nursing homes, retirement centers, and other DDS agencies. Serves as the individual's advocate in assuring access to a wide range of services. Documents services that are needed by or recommended for the individual but do not exist within the individual's local community, and works to develop the availability of these services. Assures that all personal rights of the individual are maintained, including freedom of provider choice, civil, and legal rights. Perform any other related duties as required or assigned. Essential Duties and Responsibilities Conducts a preliminary assessment for new referrals. In coordination with assigned Case Manager, assists with developing a plan of service, participates in continued stay reviews and personal future plans. Determines direct care service delivery staffing needs and arranges for the delivery of services through conducting interviews, hiring, supervising, producing payroll payments, evaluating, and discipline of Direct Care Worker. Participates in team meetings with family members, professionals, and case managers to implement procedures and programs that ensure regulation compliance. Coordinates schedules for both waiver and generic services for client that includes staffing of all alternative living services. Performs Medicaid billing payroll functions for Direct Care Worker on a bi-weekly basis and assures the integrity of that billing by approving time, attendance and time cards, reviews and approves direct care travel, and documents work performed. Monitors all in-home services on a regular basis that includes ensuring Direct Care Worker compliance with completion of progress notes, required paperwork, code of ethics, and meeting agency requirements for initial and continued employment. Ensures direct care staff is compliant in following positive behavior plans and completing reports as required. Completes in a timely and accurate manner monthly, quarterly, and annual reports and various other reports as determined by Supervisor. Provides training to new Direct Care Worker in general procedures and individual specific training. Counsels recipients and families regarding staffing issues, scheduling, and other issues under their scope of responsibilities, and is on call to assist with staffing emergencies. Responsible for monitoring client medication regimens, ensures staff is at all times aware of medications being used; monitors consumption of medications on at least a monthly basis; ensures screenings are performed at a frequency determined by the prescribing physician, and any additional requirements as determined by DDS rules and regulations. The Direct Care Supervisor will serve as a liaison between staff and the Physician regarding medication. Prepares appropriate paperwork for new plans, continued stay reviews and revisions in accordance with DDS standards. Works with Case Management entity to assist and act as liaison for recipients with professional services to include, but not limited to: early intervention coordinators, doctors, public schools, case management agencies, counseling services, Human Services, nursing homes, retirement centers, and other DDS agencies. Serves as the individual's advocate in assuring access to a wide range of services. Documents services that are needed by or recommended for the individual but do not exist within the individual's local community, and works to develop the availability of these services.
