Key Responsibilities
Support residents with daily living needs (grooming, bathing, hygiene, mobility, communication)
Assist with medication administration, following the Seven Rights
Supervise and participate in community/recreational outings

TRANSPORTATION OF PERSONS SERVED

Follow all inspection/cleanliness of vehicle requirements before/after each use.
Ensure safe entry and exit protocols, as well as ensure the safety of individuals while in a moving vehicle (i.e., tie-downs and seatbelts).
Triplog application (with Magic Trip enabled) must be used for all personal vehicle and fleet mileage driven during your scheduled work day. Trips must be reviewed, corrected, and submitted daily or at the start of the next shift.
Applicable inspections performed via Fleetio application prior to usage of any BI vehicle at any time.
OTHER
Assist with the setup of various program spaces for activities (chairs, tables, etc.).
All BI working hours should be performed at a BI location, in the individual's home, or at a community site with the individual. Exceptions need to be pre-approved by your supervisor.
Provide support as a Direct Support Professional working in any BI program, including but not limited to providing in-home care, transportation, and in-community services for persons served.

Position Overview
The Relief Residential Counselor provides support and counseling services to an adult population with psychiatric, substance abuse and/or HIV-AIDS related disabilities. Relief Residential Counselors advocate for and respond to clients' treatment and rehabilitation needs. All agency services are provided from the philosophy of social rehabilitation, which integrates teaching daily living skills in a safe and healthy environment.
Primary Duties and Responsibilities
Perform intake interviews and assist with client treatment plan development, implementation and discharge planning.
Assess the clinical needs of resident clients.
Evaluate clients for potential crisis situations and determine appropriate intervention.
Conduct individual and group counseling sessions as assigned.
Monitor and document clients' progress toward achievement of individual goals.
Record clinical notes and hourly rounds to meet Medi-Cal standards and licensure requirements.
Maintain shift coverage and primary supervision and support of residential clients.
Act as a role model - guiding clients and facilitating appropriate behavior about daily living skills, self-care, personal interaction, social relationships and constructive time management.
Facilitate and supervise clients with residential house chores, meal preparation, and group activities.

Maintain agency 30 hour per year staff development training requirement.

Seeks consultation from physician on any cases that he/she may feel exceeds his/her ability to manage, when the PA is in doubt of the diagnosis, when a patient fails to respond to treatment or when the patient expresses a desire to see the physician.
As needed, participates in team conferences to review status of patient care.
When needed, participates in data collection and outcome measurement to insure quality care is being provided.
Works collaboratively with team members to ensure appropriate and efficient flow of patients through clinic appointments.
Completes and submits coded fee tickets for each patient encounter.
Promotes excellent customer service.
Education:
Graduate of an accredited Physician Assistant program approved by the National Commission of Certification of Physician Assistants (NCCPA).
Or Master's required. Graduate of an accredited nursing program including a Nurse Practitioner Program.
Licensure/Credentials:
Current Physician Assistant licensure by the State of California Physician Assistant Examining Committee and Board Certified.
OR Current California licensure as a Nurse Practitioner and Board Certification by the American Nurse Credentialing Center, American Academy of Nurse Practitioners, American Association of Critical Care Nurses, or Pediatric Nursing Certification Board.
Current AHA or equivalent certification in CPR.
Maintains a current DEA license for schedule 2 drugs and above.
Maintains a current Furnishing Number for medications
Experience:
Experience as a Physician Assistant practicing in a related field, or completion of internship in primary care at NorthBay.
OR Two years' experience as a Nurse Practitioner OR four years of clinical experience as a Nurse, OR completion of internship at NorthBay Health. One year of experience in related field and ambulatory care preferred.
Skills:
Demonstrates ability to evaluate patient's physical and emotional status.
Demonstrates ability to provide patient care based on age-related needs.
Demonstrates current knowledge and skill in providing appropriate care for patients in the following age groups: adult, adolescent, pediatric and geriatric.
Ability to use computer, MS Word, Outlook, telephone. Knowledge of Cerner preferred.

The 815 Residential and Detox Program The program fully integrates substance use disorder and mental health services; beyond merely providing both services under the same roof or cross-training staff, it adopts a model of providing effective services to the targeted needs of individuals with co-occurring disorders. The program treats both problem types as primary and assists participants to recognize key reciprocal factors such as self-medicating strategies; physiological and psychological changes associated with long-term substance use; and the impact of street drugs on mental health symptoms.
The SUD (Substance Use Disorder) Counselor I is an integral part of an interdisciplinary team that supports our clients through assessment of their needs and strengths in support of their health and recovery. SUD Counselor I will perform counseling in individual and group setting, deliver curricula in accordance with program specific requirements and case management, and assist clients in navigating systems of care while providing a supportive treatment environment.
KEY RESPONSIBILITIES
Facilitate group counseling sessions, community meetings, and teach classes on various treatment-related topics to assist clients in meeting their treatment goals.
Perform crisis intervention and communicate with treatment team as unforeseen situations arise.
Properly document all group counseling sessions and all individual sessions as needed within 24-72 hours per contract requirements.
Must meet residential service delivery goal of 60%.
Proactively link clients to both internal and external resources based on their treatment needs.
Follow up on the progress or status of clients' treatment.
Document client updates and incidents daily.
Maintain safety and security of the clients in the facility by performing periodic house runs.
Ensure client medications are securely stored and properly accounted for as needed.
Monitor and document client administration of medication.
Assign client duties and sign off upon completion.
As needed, accompany clients to offsite appointments related to their treatment.
Collaborate with client and other internal and external resources to develop or maintain problem lists and/or treatment plans, transition plans, progress notes, and appropriate updates in support of their health and recovery.
Complete discharge paperwork/process and other required agency assessments in a timely manner.
Support intake process such as preparing rooms and maintaining supplies.
Perform property and room searches as needed.
Respond to overdose and other medical emergencies.
Perform tests for substance use such as urinalysis and breathalyzer.
Observe clients every 30 minutes and document on observation log, per program requirements.
Collect, clean, or dispose of biohazard waste.
Disinfect and clean rooms and common areas as needed.
Accurately enter data in various electronic systems for all clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
Read and respond to emails, calls, and Teams messages in a timely manner.
Complete assigned training as needed and as required by the due date.
Attend and actively participate in meetings.
Plan and facilitate celebrations and special events.
Work on weekends and holidays as needed.
QUALIFICATIONS
Education, Certification, Licensure
High school diploma or equivalent required. Bachelor's degree in health and human services, social work, psychology, or another related field preferred.
Must be registered as an Alcohol and Other Drug Counselor or as an SUD Counselor with CA approved certifying agency (CCAPP, CADTP, or CAADE).
Current First Aid and CPR certification or ability to obtain within 30 days of hire.
