The Care Coordination Specialist plays a key role in the Collaborative Care Management (CoCM) model within our behavioral health practice. This role serves as the first point of contact for patients referred into behavioral health services. The Specialist is responsible for obtaining patient consent for participation in Collaborative Care, ensuring seamless handoff of referrals to Behavioral Health Care Managers, and supporting the care team by coordinating services that address patients behavioral and psychosocial needs.
Key Responsibilities
Serve as the initial liaison for patients referred to behavioral health services.
Conduct informed consent discussions with patients, ensuring understanding of the Collaborative Care Model.
Collect and document patient consents in compliance with organizational and regulatory standards.
Facilitate warm handoffs and timely referrals to Behavioral Health Care Managers (BHCMs).
Maintain accurate and thorough documentation in the electronic health record (EHR).
Coordinate with medical providers, BHCMs, and psychiatric consultants to ensure continuity of care.
Monitor referral completion and follow up with patients as needed.
Provide patients with information, resources, and support to improve access to care.
Participate in care team meetings and communicate relevant updates to the behavioral health team.
Maintain compliance with HIPAA and ethical standards of professional practice.
Qualifications
Education: Masters Degree in Social Work (Non-Clinical; Administrative)
Licensure (Required): LMSW
Strong interpersonal and communication skills with the ability to engage patients sensitively and effectively.
Knowledge of behavioral health and medical systems of care.
Detail-oriented with strong organizational and documentation skills.
Ability to work collaboratively in a multidisciplinary team environment.
Preferred Experience
Prior experience in care coordination, patient navigation, or case management.
Familiarity with the Collaborative Care Model (CoCM) and/or integrated behavioral health.
Experience in oncology, chronic illness, or primary care settings.
Bilingual (Spanish/English) strongly preferred.

Provide supervision and actively engage participants, while providing positive reinforcement
Assist with daily living responsibilities, such as, cooking meals, keeping a clean-living environment, assistance with medications, grocery shopping, healthcare appointments, etc.
Participate in community activities with participant, such as, going to movies, bowling, Kroc Center classes/workouts, hiking, fishing, camping, Specialized Needs Recreations (SNR), Special Olympics, etc. (Must be willing to use personal car for community outings)
Complete personal care needs for certain individuals (toileting, showering, perineal care, etc.)
Foster a positive living environment
Encourage independently living skills
Complete and maintain participant documentation
Prevention and management of crisis (to provide a safe environment)
What You'll Need:
Must be at least 18 years old
Have a valid Driver's License
Have a reliable transportation and valid insurance
Have the ability to pass a Federal Background Check
Be free from communicable diseases and skin lesions
CPR & First Aid Certified (will pay for training after being hired)
Assistance with Medications Certified (will pay for training after being hired)
Ability to work independently, with little supervision

The Direct Support Professional assists adults with developmental disabilities in their daily lives. Assists with complete care of dietary, hygiene, grooming, and dressing needs. Redirects behaviors with various activities including walking, reading, games, math, and computer projects.
Essential Functions
Outline and prepare daily routine(s) for clients' (visual and active) as written in Individual Instruction and Support Plan (IISP)
Assist clients with bathroom needs.
Prepare daily meals and assist with clean-up.
Pick-up/drop-off clients in company vans or personal Vehicle from/to their home/facility (if necessary).

Here's What You'll Do:
Direct Support Professionals assists adults with developmental disabilities in their daily lives. Assists with care of dietary, hygiene, grooming, and dressing needs and Redirects behaviors with various activities as needed.
Performance Expectations and Responsibilities:
Provides support to program participants that meets their needs, provides opportunities for community
Integration a nurturing, safe, and healthful living environment.
Must be familiar with the Residential Service Guidelines issued by the Developmental Disabilities Administration.
Provides each participant with an array of instruction and support services, as described in the participants' Individual Instruction and Support Plan (IISP).
Ensures that the daily needs of the participants are met, including personal hygiene, food preparation, house cleaning, shopping, and monitoring of health needs, including supervision of medication.
Assists participants with regularly accessing the community to take part of a variety of outings of the participants' choosing. This may require transporting participants or arranging for transportation.
Ensures that participants are always treated with dignity and consideration. Promotes and safeguards each participant's ability to make decisions and to freely exercise his or her rights.
Interacts with participants, provides informal counseling, and problem-solves with participants.
Attends staff meetings as required by the House Manager or the Site Supervisor and/or Administrator.
Attend training required by the agency, DDA, and other parties.
Takes part in the development and implementation of the IISP for each participant with whom they work.
Will become Nurse Assistant Registered (NAR) at employee's own expense initially and annually.
Must enroll and complete Nurse Delegation study course and take the test within 30 days of employment.
Provides necessary support and supervision of participants.
Maintains professional working relationships with other services providers, the participants' parents/relatives/guardians, and other agency staff. Responsible for timely completion of all documentation, including, but not limited to, the medication log, shift notes, progress notes, data sheets, incident reports, and client financial records.
Performs other duties as assigned by the House Manager or the Site Supervisor and/or the Administrator.
