To be a “Master of the Mise en Scène:” The French expression used to describe the art of creating a story through the physical environment, ‘mise en scène' means being responsible for setting the stage for a successful day. From preparing your dining room for breakfast to setting up the library for an event, to preparing the resident's room for their nighttime routine, you are the guide through the resident's experience.

What are we looking for?

DIRECT SUPPORT PROFESSIONAL - COMMUNITY CONNECTIONS
This is an entry level human services position providing desired support to persons with disabilities, in community settings, to help individuals lead more fulfilling lives. Employees will use positive support techniques to bring about meaningful and positive lives and to assist individuals in meeting their goals.
These techniques will be used in structured situations and generalized into "everyday" situations such as mealtimes, life skills, hygiene, or community activities. This employee assists in the implementation of personal plans and establishes effective interpersonal relationships with each person served as well as community members.
Individuals being supported may have some form of Intellectual Disability/Developmental Disability. These individuals may be prone to poor social & communication skills, impulsive behaviors, poor decision making, and verbal/physical aggression. Individual Support Plans target a broad range of skill deficit areas such as communication, self-determination, problem solving, self-care, and socialization.
Close supervision and direction is received from the Community Support Coordinator and the Program Supervisor. This is a non-exempt, hourly position.

Job Summary
This position is responsible for creating, leading, and supporting assigned participants in their daily programming in the areas of academic, social, life and leisure skills as well as recreational activities. Assist in the development and implementation of Personal Centered Individual Service Plans (ISP), including but not limited to social, community inclusion, communication, behavior, and skill development. Ensure compliance with all State and Federal regulatory guidelines, including Medicaid, Licensure, and Commission on Accreditation of Rehabilitation Facilities (CARF), as well as other funding sources and company policies and procedures.
Primary Duties

Provide active engagement and support for all individuals assigned, including but not limited to community integration and inclusion, training, leisure, communication, mobility, behavior, personal care (feeding), bus loading and unloading, medication, safety and risk management.
Provide input and implement Person Centered Plans (PCP) outcomes, support activities, and protocols as written in order to ensure a safe and healthy environment, proper documentation, and progress. Responsible for a safe, clean, and orderly environment. Immediately report any unsafe conditions, medical concerns to the Program Manager or designee. Follow established written protocols and plans. Ensure and maintain accurate records and documentation per the PCP, regulatory and company standards. Inform the Program Manager or designee of missing or inaccurate information immediately.
Communicate with family members, guardians, authorized representatives, residential providers and support coordinators regarding participant's illness, incidents, and progress at the direction of the Program Manager or designee. Keep team members apprised of any concerns.
Additional Responsibilities
Maintain participant records, daily notes, quarterly reports, per Medicaid, Licensure, and CARF standards.
Participate and/or provide input for the PCP process (plan, quarterlies, daily notes) and work in conjunction with the team in the implementation, monitoring and review of the individual's written plan.
Utilize appropriate behavior intervention techniques, proper lifting, feeding, and transferring techniques as written.
