About:
This is a mixed APP role comprised of both patient care and indirect patient care and the ability to grow your own panel of patients.
Monday - Friday, no weekends; 4 10-hour shifts.
Patient Volume - 12-14 patients per provider per day.
Providers: 7 total Physicians/APPs.
Qualifications:
Ability to obtain medical professional license in the State of WA and a DEA with full prescriptive authority.
Strong communication and interpersonal skills with the ability to work effectively with the physical and emotional developments of all age groups.
At least one year of experience as an APP in Primary Care strongly preferred.

Must complete courses via online learning system as assigned by Client Mgr.

requires the ability to work nights and weekends as scheduled. Individuals assigned to this position are responsible for but not limited to clerical/receptionist/cashier, weight room attendant, and offsite gym monitor duties. This position is responsible for accepting payments from customers and making sure participants comply with the usage rules relative to using the Recreation Center, and off-site facilities. The position also requires inspecting/cleaning of the cardio/weight room equipment, takes attendance both at the Recreation Center and off-site. The position is within the Parks, Recreation and Cultural Arts Department, and works under the direction of the Customer Service Supervisor.
Essential Functions
Includes but is not limited to such essential functions as listed below:
* Provide information to the public on the phone or in person about recreation activities and programs, policies and procedures.
* Process registrations for recreation activities; process refunds and registration changes, when necessary.
* Process reservations for park, pool and room facility rentals.
* Receipt fees from revenue producing programs, operate the point of sale cash register, and balance funds receipted at completion of shift.
* Assist with room set up and take down, moving chairs and tables; insure proper and orderly conduct of customers in their use of the facilities.
* Perform data entry and miscellaneous clerical and computer functions.
* Notify Customer Service Supervisor when facility maintenance is required.
* Observe customers use of the facilities and ensure they comply with usage rules relative to using the weight room/cardio space and equipment.
* Clean equipment per scheduling.
* Takes attendance and ensures class registrations at either the Recreation Center or offsite facilities.
* Report any accidents or incidents to Customer Service Supervisor immediately, and notify 911, if necessary
* Work off site monitoring Department affiliated activities.
* Work nights and weekends as scheduled.
Minimum Qualifications
At least six (6) months' work experience with clerical, cash handling, customer service, and using cardio/weight equipment preferred. Must be at least 18 years old. Must have current First Aid and CPR certifications, or be able to obtain them within one month of hire. Must also pass the Washington State Patrol background and a credit check.

Job duties include (but are not limited to):
• Provides support to the individual to ensure objectives are met in accordance with their individual treatment plans.
• Assist with personal care needs, including bathing, dressing, hygiene, meal prep and restroom assistance.
• Be alert and responsive to client needs at all times to prevent escalation of behaviors.
• May be required to coordinate, organize and/or assist with household activities such as light housekeeping and meal preparation, yard maintenance, packing, and moving the furniture of individuals receiving services.
• Monitors the administration of medication.
• Arrange and schedule medical, dental, and/or psychological/psychiatric appointments for individuals in care.
• Must provide transportation and accompany individuals receiving services to planned and/or necessary activities and appointments - via personal vehicle, bus, etc.
• Maintains daily progress notes and any other appropriate documentation in accordance with CIS policy, program standards, or other regulatory policies.
• Engage in community activities, outings, and appointments with the client to ensure social integration and fulfillment.
•Build a positive and trusting relationship with the client by being patient, understanding, caring and consistent.
Specific Requirements for This Client:
· The client requires assistance with all activities of daily living, including using the restroom, bathing, and dressing.
· The ideal candidate must be willing to drive their car frequently as the client enjoys going for rides and being out in the community.
· The client has many basic and complex needs and behaviors which may increase if not attended to promptly. The candidate must be alert and responsive at all times.
· The client can escalate quickly if needs aren't met as soon as possible, so the candidate must be patient, calm, and quick to respond.
· The candidate should be comfortable working with a client who exhibits high intensity behaviors and not be easily put off by them.
· The candidate should be dedicated to getting to know the client and providing consistent and compassionate care.

Job Description
The live-in model allows for structure in a client's home environment. Our Live-in model is a 7 day on/7 day off schedule. You will be required to live in the individuals' home-on location and will be paid 16 hours a day (minus any unpaid break time). Live-ins work approximately two weeks per month and earn full-time pay plus overtime! Bi-weekly compensation will be for 105 hours (25 of which are paid at a rate of time and a half). You will also be paid for any interrupted sleep time, if applicable Live-ins are also provided with a weekly food budget during your week on. You will receive one 7-hour break during your scheduled shift.
All off time must be spent off location - the individual's home cannot be your primary residence
