As a Host Home Provider / Caregiver, you and your family have the opportunity to open your hearts and home to an adult with developmental disabilities or a brain injury. You provide care, support, and mentorship in areas of daily living, accessing community resources, and finding employment. This includes providing nutritious meals, assisting with personal hygiene, providing transportation, ensuring access to community activities, and providing supervision based on their needs. Basically, you care for them as you would any family member. You ensure that they have the freedoms and help needed to achieve their goals and live as independently as possible.
Some aspects of this caregiving position are not easily accomplished, but the relationship you build with this individual is definitely worth the effort. Acting as their advocate, you always keep their best interests in mind. You have come to see them as family and sympathize in their sorrows and rejoice in their happiness. Seeing them smile from the guidance, care, and compassion you show to them is priceless. Safely drive and operate your delivery vehicle at all times.
Use hand held device for routing information, customer delivery information.
Must be comfortable driving and working in varying weather conditions.

MAJOR RESPONSIBILITIES & DUTIES
1. Implement the consumers support strategies and other curriculum in an effective and creative
manner.

SPECIFIC DUTIES
1. Implement the persons support strategies and other curriculum in an effective and creative
manner.
Implement supports as directed in support strategies plan. Participate in the development of the person's support strategies.
Assist the person to participate in activities and community outings.
Conduct ongoing reviews of the supports provided to the persons assigned and report ideas, progress and concerns to person your supervisor and/or coordinator.
2. Responsible for welfare of persons assigned.
Be responsible for the supervision of all assigned persons at all times.
Be aware of and support the person as concerns arise. Follow-up with concerns or recommendations as needed.
Assist with the personal needs of the person
(e.g. personal hygiene, toileting needs, accidents, etc).
Be present in the lunchroom during the lunch hour to assist the person as needed. Teach eating manners/social skills as appropriate during this time.
Assist consumers to load on and off the bus at the designated times.
3. Completion of all required paperwork.
Take effective and specific progress notes as specified in the consumers support strategies. Progress notes should be taken immediately after the class or in the event of group activities at the end of the day.
Complete incident reports as necessary and forward original document to Coordinator within required time frames.
Assist in the completion of assessments as needed.
4. Team Player.
Attend staff meetings/trainings as required for the position.
