RESPONSIBILITIES: Perform all assignments in accordance with established organizational policies and procedures as outlined in the organizations policy manual, employee handbook, and as instructed by the supervisor. Provide direct care according to the individual's plan of care (ISP). Documenting care and medication according to company and state policy and procedures. Input clinical data into Home Health Software as instructed. Meet with the family to discuss the needs of patients. Teach developmentally disabled adults how to adopt basic independent living skills. Help physically disabled move from room to room around their homes. Assist individuals in dressing, bathing, and completing basic hygiene tasks. Oversee any required rehabilitation exercises.
