Purpose
In collaboration with supervising physicians, provides for the expansion of individualized healthcare services by diagnosing and treating thoracic conditions. Responsibilities include outpatient and peri-operative management of thoracic surgery patients.
Responsibilities
Obtains patient history and performs assessment via observation, interview, and examination.
Orders, performs, and interprets diagnostic studies. Performs health assessments and screening.
Identifies normal/abnormal health characteristics, develops plan of care, and initiates appropriate interventions. Timely response to patient questions/issues that arise as triaged by clinical staff and aids with emergencies, as needed.
Provides direct treatment and management of conditions via prescriptive authority or referral to other healthcare providers or community resources, inclusive of administering medications according to the formulary protocol or upon patient-specific approval from the supervising physician or transmits orally or in writing on a patient's record, a prescription from the supervising physician to a person who may furnish such medication.
Counsels and educates patients and their families/caregivers concerning health conditions, treatment options and community resources.
Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.
Participates in orientation and education of staff and students.
Collaborates quality health care with interprofessional team members to manage/coordinate patient care for both in-patient and out-patient treatment.
Fosters identification and implementation of innovative solutions to improve patient care or department operations.
Integrates and contributes to professional practice standards, policies, procedures, protocols and leads improvements in patient safety-related processes.
Other related functions as assigned.
Required Qualifications
Master's degree, Licensed Advanced Practice Nurse Practitioner or Physician Assistant with prescriptive authority, credentialed by the Texas Board of Nursing or the Texas Medical Board.
Ability to work efficiently and flexibly in a fast-paced environment with minimal guidance.
Ability to address individuals and information with a high degree of sensitivity and confidentiality in various situations.
Capacity to work well with physicians and other care providers as part of a multi-disciplinary healthcare delivery team.
Capacity to work with trainees, including medical students and surgical residents.

The Direct Support Professional staff provides direct care services and supports to persons with intellectual and/or developmental disabilities. Duties consist of quality interaction and support of individuals in all activities of daily living, personal care, routine housekeeping, cooking, shopping, skills training and community integration. Settings in which services are provided include group home and community (public) settings.
Performs moderately complex (journey-level) habilitative and rehabilitative care work with individuals who have developmental disabilities. Works under general supervision, with limited latitude for the use of initiative and independent judgment.
This position is a floater position between group homes, serving homes that require the most need. There are 2 shifts available: evening & overnight. This position may work on a rotating schedule that changes every other week.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Service Delivery
Provides direct care, meaningful activities and skills training to individuals with intellectual and/or developmental disabilities as outlined in the Person Directed Plan, Individual Plan of Care and Implementation Plan. This may include assisting individuals with hygiene and personal care.
As a member of the service planning team, provides input regarding individuals service participation and progress towards goals.
Provides transportation for individuals based on identified service and support needs.
Assists in the implementation of behavior support plan, as determined by Licensed Behavior Analyst.
Health and Safety
Provides direct supervision of self-administration of medications, or administers medication in accordance with Physician's orders and as delegated by R.N.
Observes individual for signs and symptoms of disease, injury, reaction to medications, behavioral changes, or other conditions that warrant intervention. Report significant changes in individual's physical conditions or behavior to provider nurse, behavior support specialist, program supervisor, or in emergency situations, 911.
Utilizes approved techniques from Prevention and Management of Aggressive Behavior (PMAB) training to manage aggressive behavior among individuals, and provide intervention to ensure that individuals are protected from harm or injury.
Documentation
Completes progress notes and service logs within timelines specified by division policy.
Completes behavior data cards for clients with a behavior support plan.
All documentation must meet billing guidelines for the individual's specific program (ex. Home and Community-based Services).
Completes incident reports within the time frames specified by the center.
Responds to inquiries or directives from division staff tasked with data management and quality assurance. All inquiries should be responded to within 1 business day, unless a shorter timeline is required due to state reporting of encounter data or billing.

Duties include overseeing the daily operations of the animal shelter; supervising staff, volunteers, and community service workers assigned to the shelter; ensuring consistent implementation of shelter protocols and a high standard of animal care; issuing citations for code violations and making court appearances as required; coordinating the transfer and placement of animals with approved rescue and placement partners; supporting and guiding the Lead Animal Care Technician and Outreach Coordinator in rescue-related activities; and assisting with the development, training, scheduling, and overall engagement of shelter volunteers. Additional responsibilities include providing assistance with animal adoptions, reclamations, and euthanasia; responding to inquiries and providing information to the public, city departments, and partner agencies; ensuring accurate record-keeping; and compiling related reports. An Associate degree, or equivalent, and three years of experience in animal control or a related field, with one year of supervisory experience required. Must have knowledge of animal services ordinances. Must also have, or obtain within the specified timeframe, Animal Control Officer certification and euthanasia certification.

MATHEMATICAL SKILLS:
Basic math skills required.
REASONING ABILITY:
Ability to apply common sense understanding to carry out simple one or two-step instructions. Ability to deal with standardized situations with only occasional or no variables.
COMPUTER SKILLS:
Use computer, printer, and software programs necessary to the position (i.e., Word, Excel, Outlook, and PowerPoint).
Ability to utilize Internet for resources.
CERTIFICATIONS, LICENSES, REGISTRATIONS:
Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain Texas Driver License within three (3) months of employment.
