    General The Direct Support Professional (DSP) - Community Living Supports is responsible for providing individualized services that promote independence, informed choice, personal growth, and community participation for adults with intellectual and developmental disabilities (I/DD). Supports occur in the person's home, neighborhood, or broader community and are delivered in accordance with the Individual Support Plan (ISP), Oregon Administrative Rule (OAR) 411-450, and Shangri-La policies and procedures. These services are intended to be flexible and responsive to the person's unique strengths, preferences, and needs. Position Details: Status: Part Time up to 29 Hours Location: Salem, OR Shift: This position has varying hours including weekends and evenings and must have on-call availability Starting wage: $20.14 Hourly Wage Scale: $20.14 to $25.48Report to: CES - Manager Knowledge: Knowledge: A High School Diploma or equivalent is required. One year of experience working with individuals with disabilities is preferred. Candidates must have a valid Oregon Driver's License, an acceptable driving record, and must be able to pass a criminal background check. A strong understanding of person-centered planning, alternative communication strategies, the principles of dignity of risk, and the ability to implement trauma-informed support practices is preferred. Interprets and applies written instructions from parents. * Completes timely written documentation of behavior concerns, minor injuries or other child and program documentation in accordance with program policies and guidelines. * Provides professional and timely communication to parents and the Site Coordinator about any child concerns in accordance with program policies and guidelines. * Assures the safety and well-being of enrolled children. Takes appropriate steps to intervene when students are not in control, not in the proper location, or may be in dangerous or unsafe situations. * Provides limited first aid when necessary. Assesses injury to determine whether nursing care is needed. The APP is authorized to prescribe medications according to the state and federal guidelines and laws. This faculty position specializes in caring for adult patients with neurological disorders and is primarily responsible for inpatient clinical care as well as other clinical operations within the Department of Neurological Surgery. The role involves independent patient care, procedural work, care coordination and administrative tasks, and educational contributions. The APP collaborates closely with faculty physicians, APP colleagues, and others in the interdisciplinary team to ensure the highest standards of patient care and service. The APP will support OHSU's missions including teaching, research, patient care, outreach, and public service. Clinical Care Primarily in an inpatient environment that provides 7-day/week clinical coverage, provide comprehensive care for neurosurgery patients from admission to discharge. Perform daily rounding with faculty and develop/implement patient care plans and protocols. Perform neurosurgical procedures according to protocol and competency training. Perform administrative patient care for patients who recently discharged and collaborate with outpatient teams members to prepare patients for next steps in their care. Respond promptly to patient/family inquiries (phone, MyChart), collaborating with team members such as nurses, APPs and physicians to resolve clinical concerns. Identify urgent clinical situations and respond autonomously and effectively, including providing situational resuscitation and emergency treatment. Provide scheduled weekend hospital coverage including rounds, patient discharges, and continuity of care duties according to department protocol and rotating assignments. Care Coordination & Communication Coordinate with interdisciplinary teams, including outpatient staff and other APPs, to ensure streamlined, patient-centered care. Collaborate with surgeons, physicians, and external providers to optimize patient outcomes. Triage patient calls, messages, and requests and determine the appropriate plan of care for recently discharged patients. Educate house staff and clinic medical teams on care plans and best practices to improve patient outcomes. Patient and Family Education Contribute to patient and family education, including development and presentation of educational materials. Provide condition-specific education and counseling to patients and families. Use available educational materials and resources to promote patient understanding and self-management. Program Development, Teaching & Quality Improvement Engage in continuous process improvement, adhering to OHSU policies, service standards, and regulatory guidelines. Participate in quality improvement initiatives, program development, and staff development activities. Teach neurosurgical principles, procedures, and patient care to medical students, house staff, nurses, and support staff. DESIRED QUALIFICATIONS: * Associates or Bachelor's Degree in an appropriate and related field of study. * Two years' experience that demonstrates the ability to work successfully with children in a learning environment. * Familiarity or willingness and ability to be trained in areas of Autism Spectrum Disorder, multiple disabilities, mental health disorders, ADHD, etc. MAJOR DUTIES AND RESPONSIBILITIES (depending on specific assignment): * Works collaboratively with the Inclusion Support Specialist and site staff by assisting with safe and positive child interactions and/or intervention, ensuring safe behaviors, and enhancing social and life skills of children. * Implements, adapts, and supervises activities designed to meet the physical, emotional, and intellectual needs of students who require additional supports. * May intervene with children to implement positive behavioral supports. * Provide complex toileting, feeding, mobility and personal hygiene duties for children requiring such support. * May escort children in one-on-one or small groups to and from recess and playground, and to and from bus locations. * When communicating with children, parents, community members, and other staff members, exercises discretion and assures protection of child confidentiality consistent with the direction and guidance of staff, administrators, and school/district policies and procedures. * Oversees child safety by such activities as recess and playground supervision, bus duty, hallway supervision, etc. Takes steps to intervene when children are not in control or may be in dangerous or unsafe situations. * Provides inclusion support to children in large groups, small groups, and one-on-one. * Takes action, or assists in taking action, to use protective physical intervention with children who are at risk of imminent bodily harm by following school-/district-approved procedures. * Implements behavior support, safety, and accommodation plans as directed by appropriate district personnel and the Inclusion Support Specialist. Documents behavior of children to assist the Inclusion Support Specialist in assessing progress with behavioral and/or safety plans. Maintains or assists in maintaining the child's daily/weekly progress reports. * Completes timely written documentation of behavior concerns, minor injuries or other child and program documentation in accordance with program policies and guidelines. * Facilitates large group and small group recreational and enrichment activities. * Attends program staff meetings and required trainings.
