Position Summary:
The Community-Based Specialist utilizes the Homebuilders Model to provide families with a
wide array of services to equip and supply family members with the necessary tools to
successfully prevent out of home placement or reunification services. Use the training and supervision guidance to implement an evidence-based model.
Teach by direct methods, generally including the following components: presentation of the skill, modeling, role-play, constructive feedback, coaching, praising and encouraging. Develop an understanding of systems that serve families including the legal, social services, health care, and school systems. Consults and advocates with other service providers when appropriate to help family members meet their goals.
Work with children, youth and families as a partner toward successful accomplishment of goals developed by the family to move them toward their version of success.
Qualifications:
Master's degree in social work, psychology, marriage and family therapy, or related
human service field.
Bachelor's degree in social work, psychology, sociology, or
related human service field with at least 2 years of direct social service experience.
Has a belief system and engages in faith practices that are compatible with those of
the United Methodist Church
Must be able to pass standard background checks, including: drug screen,
employment physical, SLED, DSS Central Registry and Sex Offenders Registry

In this role you will:

What you will need to succeed:
* DVM degree or equivalent.

What is a Community Training Home (CTH)?
Clients who live in a Community Training Home are encouraged to be independent in daily activities and care, while still receiving support as needed from the trained Direct Support Professional. The goal for individuals in the CTH program is to learn how to become as independent as possible and become involved in their community.
Job Description:
As a Direct Support Professional (DSP), you will provide supervision and support to individuals who need direct, personalized care and individualized training. The candidate that best suits this position will be caring and motivated!
Direct care for individuals can include:
Preparing meals from a selected meal plan.
Assisting individuals with self-help skills and daily activities such as toileting, personal hygiene, eating, or administering medication.
Following any facilitated or behavioral support plans.
Monitoring individuals and completing monthly progress reports on all objectives.
Transporting individuals to the day program or planned outings.
Always supervise and advocate for individuals.
Assist individuals during extracurricular activities.
Provide one on one care to an individual if that level of care is required for position, including but not limited to feeding, toileting, etc.
Attends initial Burton Center staff orientation, in-services and workshops as appropriate to enhance job performance.
Provide appropriate and accurate documentation during each shift through online
Therap system
Required Qualifications:
High School Diploma, GED, or ability to pass a competency test
Valid SC Driver's License
Able to attend mandatory two-week training course
Preferred Qualifications:
Clear oral and written communication skills
Lift 25lbs frequently and 50lbs occasionally
Ability to care and advocate for others
Ability to bend, stoop, kneel and squat
Ability to stand or walk for long period of time
Ability to drive and transport individuals who may be wheelchair bound
Direct Support Professional
Direct support professional job in Camden, SC
The Direct Support Personnel II assists individuals withintellectual disabilities or mental health challenges with growth anddevelopment of interpersonal skills and activities of daily living in the home,community or residential setting. The Direct Support Personnel II worksdirectly with the resident and provides supports for activities of daily livingand adaptive skills. The Direct Support Personnel II should follow the guidanceof the resident/ patients Individual Service Plan.
Minimum Requirements:
+ High School diploma or its equivalent
+ Valid driver's license
+ Valid and current auto insurance
+ Complies with all relevant professional standards ofpractice
+ Current CPR if applicable
+ TB questionnaire, PPD or chest x-ray if applicable
+ Current Health Certificate (per contract or stateregulation)
+ Must meet all federal, state and local requirements
+ Must be at least 18 years of age
+ Demonstrates ability to prioritize tasks
