General Description
The DirectSupportProfessional will serve as a Client Mentor in our newly approved Youth Transitional Housing Program. DSP/Client Mentor will provide essential 24-hour awake coverage and supervision of the shelter and its clients. This position is responsible for the welfare, safety, and comfort, as well as daily support to the youth clients ages 18 -24 years old who are experiencing homelessness or are at risk of being homeless. Additionally, clients may be economically and/or emotionally detached from their support network. Through this work, the client mentor will help youth improve basic life and interpersonal skills, access educational opportunities, assist with job preparation and attainment, access physical and mental health care, and obtain stable housing. The client mentor helps to create an environment in which youth are empowered to achieve the highest level of self-sufficiency possible. While participating in this program, clients will live at a site-based transitional housing program, located in Washington, DC.
The DSP/Client Mentor will provide crisis intervention and behavior modification supports to include active listening, redirection and mediation. The incumbent is required to stay awake, alert and always engaged during shift. All employees are expected to demonstrate the organizational values and sensitivity to the diversity of the organizations client base.

Participate in regular supervision with Shelter Coordinator and by providing regular updates, status reports and program notes.

Required Qualifications
High School Diploma/GED (required).
Minimum 1 year of experience of youth-related work, to include working with youth who are experiencing homelessness, mental health issues, and substance abuse.
Successful completion of background checks is required prior to hire, which includes a TB test, CJIS (FBI) clearance, DC Police Clearance, drug and alcohol screen and a health and wellness general screening by a nurse practitioner or physician and active CPR/First Aid Certification. These documents must be presented prior to hire and must be maintained and updated annually.
Experience working with transition-aged youth is preferred.
Sensitivity to the needs of youth (particularly youth of color, LGBTQ, low-income, and those who have experienced trauma) who are homeless or at risk for homelessness.
Familiarity with the Strengths-Based and Housing First approaches to case management.
Well-versed in trauma-informed service delivery, with experience executing this approach in housing programs.

Proactive Procedures:

2. Maintain a warm, normalized, homelike environment.
3. Perform personal care routines such as feeding, grooming, toileting, dressing and utilization of adaptive device (e.g., wheelchairs) in accordance with direction given.
4. Observe and report immediately the signs and symptoms of illness, accidents/injury, medication effects, or any unusual incidents affecting the individual's health and welfare to the appropriate designed authority.
5. Administer medication as a licensed TME (Trained Medication Employee)
6. Supervise individual's activities on an assigned shift.
7. Under direct supervision and instruction, may implement routine habilitation programs and activities with individuals and assist in ADL assessments.
8. Lift and move non-ambulatory individuals.
9. Assist in transporting individuals in the capacity of van driver and/or van assistant upon assignment.
10. Monitor individual's activities and assure that safety and health care practices and observed.
11. Function as a personal support person.
12. Interact frequently and positively with individuals including modeling of appropriate personal conduct, manners, and personal appearance.
13. Be alert to and safeguard individual's welfare at all times; serve as an advocate for the individuals, uphold all individuals' rights and actively assure that such rights are not being denied. Staff must use discretion in the event that customer has exhibited behaviors when making requests to be alone.
14. Report all allegations of abuse and specified in he policy and procedures.
15. Assess individual's abilities to perform daily living activities.
16. Attend Interdisciplinary Team meetings as assigned.
17. Offer constructive comments as a team member with contracted professional staff in developing and implementing individual programs.
18. Feed, bathe provide self-care/grooming for the individuals or assist in the training of each individual as indicated in the Active Treatment Plan and IPP.
19. Implement each individual's Active treatment Plan on a daily basis and accurately document IPP's.
20. Actively participate in special recreational activities (i.e. fields trips, parties, holidays, celebrations, church, etc.)
21. Accompany individuals into the community to engage in individual activities.
22. Participate in implementing leisure time and recreational activities.
23. Assist in transporting individuals to activities and appointments away from home. Take all individuals to therapy programs, clinics, and actively participate in there training and ensure cleanliness of the van after use daily.
24. Exercise judgment in the application of established policies or procedures in order to assure individual well-being and safety.
25. Inform the supervisor and nurse on duty of any illness or injury to the individual verbally and in writing in the Medical concerns book. The incident report is to be completed within one hour of the incident with all of the details of the incident.
26. Communicate clearly verbally and in writing and document necessary forms for all incidents pertaining to the operations in a timely manner in the following areas:
• Progress notes, One to One, data sheets, Active Treatment Plans and IPP.
• Unusual incidents involving individuals, staff or other persons affecting the home.
• Documentation of tasks completed (shift change over, and Household Duty Sheets.
• Seizure, accidents, or other conduct, either positive or negative, in the daily documentation book.
• Home communication logs, (lotion logs and Bowel Movement charts).
27. Complete laundering, ironing, and sewing of individual's personal clothing on a daily basis.
28. Perform housekeeping tasks necessary to maintain the living area in an orderly and sanitary condition daily every shift.
29. Carry out directions and instructions of supervisors in a cheerful, cooperative, and timely manner.
30. Maintain a positive, cooperative work attitude, and harmonious relationships with all staff members.
31. Participate in job related training and staff meetings.
32. Perform all other duties as assigned.
33. Follow all Innovative Life Solutions, Inc Policies and Procedures at all times.
34. Follow all directives requested by Clinical Services, QMRP, RN, LPN, House Manager, and Team Leader as it relates to individuals, Active Treatment and Innovative Life Solutions, Inc. organizational needs.

Position Summary:
The Life Skills Trainer is responsible for teaching a range of daily living and coping skills to women and children to ensure their well-being, receive services, and maintain effective program operations.
Essential Duties and Responsibilities:
Engage the women and/or children to develop rapport, support case management goals and assist in meeting their needs.
Provide crisis intervention and intervenes during an incident. Provide positive role modeling. Conduct client in-takes.
Develop skills and facilitate and support groups. Teach a range of daily living, coping and social skills to women and/or children. (i.e. emotional self-management, parent/child interactions, problem solving, safety planning, communication, cooking, budgeting, time management, organizational skills.)
Help maintain a safe, secure and non-violent environment for all occupants. Regularly monitor the entire building and the activities and behavior of the occupants. Effectively manage all emergencies by following procedures.
Complete apartment and/or room inspections and bed checks.
Report maintenance problems.
Organizes and conduct on-site and community-based social-recreational activities.
Directly assist women and /or children in completing tasks.
Provide child care as needed.
Ensure that the women and/or children adhere to the agreed upon program expectations and address instances of non-adherence.
Monitor medications and complete related documentation.
Conduct urine toxicology screens and complete related documentation.
Conducts evacuation drills and document.
Interact with internal staff and contacts from other organizations to meet the coordinated needs of the women and/or children.
Screen telephone calls and walk-ins, provide crisis intervention and make appropriate referrals. Communicate all crucial issues to appropriate staff.
Read and complete all relevant documentation and forms.
Transport individuals to appointments, activities and shopping trips.
Distribute and monitor supplies and materials. Maintains organized, clean work environment.
Maintain log and distributes mail and funds.
Attend all trainings and meetings.
Completes other duties as needed.
Education and/or Experience:
Minimum of a High School Diploma or equivalent. Associate's Degree preferred.
Minimum of two-years of experience in a similar role preferred.
Must be able to work a flexible schedule in a 24/7 operation and be on call as needed, including some weekends.

work , as walks, visits and stays vary based on client needs.
Enter the homes of clients and visit with their pets for varying lengths of time (usually 30- 60 minutes each)
Walk pets as required

Make sure no damage caused by pets in the absence of their owners
Confirm that if any potty accidents have occurred in the homes of our clients to clean up.
