Support, and assist the individuals in enhancing skills that will further their independence, increase their community involvement, and improve their quality of life. Participate with the Program Director in the completion of individual assessments. Maintain records of individual's progress in specific program areas. Assist and or instruct individuals in activities of daily living (meal prep, house cleaning, grocery shopping, laundry and general maintenance) self-care skills (bathing, dressing, personal hygiene); safety and prevocational skills when appropriate. Document each encounter with the individual as it relates to their lSP outcomes. Motivate and assist the individuals in all areas designated in the ISP as outcomes. Display a positive, flexible, and cooperative attitude toward supervisors, coworkers and the general public. Assist individuals with medical appointments. Report or communicate all pertinent medical information on a continuity of care form to all appropriate persons (i.e. Program Supervisor(s), Medical Department, Supports Coordinator and Family Members). Assist individuals in development of skills to independently advocate for themselves. Procure necessary services as needed. Meet with Program Supervisor(s) as needed to clarify job responsibilities, discuss individual needs. Attend trainings and in-services as required. This position requires you to frequently stand, walk, climb stairs, squat, and bend. Must routinely lift 20 lbs or more. Must also assist with lifting individuals, and have the ability to monitor vital signs.
