Dell Medical School in conjunction with Dell Children's Medical Center of Central Texas (DCMCCT), is seeking Physician Assistants with cardiac critical care credentials to be part of the Texas Center for Pediatric and Congenital Heart Disease, working collaboratively with a multidisciplinary team to serve the growing need of the regional pediatric population, delivering world class pediatric cardiac critical care.
Responsibilities
Counsels and educates cardiac critical care patients and their families/ caregivers concerning health conditions, treatment options and community resources.
Documents delivery of cardiac critical health care and nursing processes in accordance with care standards and network nursing policy.
Participates in orientation and education of staff and students.
Obtains cardiac critical care patient history and performs assessment via observation, interview and examination.
Orders, performs, and interprets diagnostic studies for cardiac critical care patients. Performs health assessments, screening, and patient care.
Provides direct treatment and management of cardiac critical care conditions via prescriptive authority or referral to other healthcare providers or community resources. Engages in discharge planning.
Other related duties as assigned.
Required Qualifications
Physician's Assistant or Licensed Registered Nurse credentialed from the Texas Board of Nursing. Licensed Advanced Practice Nurse Practitioner with Prescriptive Authority specializing in Cardiac Critical Care. Credentialed from the Texas Board of Nursing in Acute Care nursing. Experience in caring for children with complex congenital and acquired heart disease in a Pediatric and Congenital Critical Care Unit. Highly proficient in Microsoft Office applications. Ability to work efficiently and flexibly in a fast-paced environment with minimal guidance. Ability to address individuals and information with a high degree of sensitivity and confidentiality in various situations. Capacity to work well with physicians and other care providers as part of a multi-disciplinary healthcare delivery team.

Key Responsibilities
Develop Core Communication Services: Build and maintain backend services for voice/video calling (signaling servers, call routing logic, media gateways) using SIP and WebRTC. Implement features like call setup, conferencing, transfers, and recording with a focus on efficiency and reliability.
Enhance Audio Quality (DSP): Implement and tune digital signal processing algorithms for superior call audio. This includes noise suppression, echo cancellation, jitter buffer optimization, and voice activity detection to ensure crystal-clear, uninterrupted communication even on poor networks.
Optimize Media & Codecs: Work with real-time media streaming (RTP) and various codecs (Opus, G.711, H.264, etc.). Optimize codec configurations and adapt bitrates on the fly based on network conditions to balance quality and bandwidth.
Integrate Voice AI Features: Embed speech-to-text (ASR) and text-to-speech (TTS) capabilities into the platform. Enable AI voice agents to participate in calls by streaming audio to AI services and injecting synthesized speech responses. Manage conversation flow between humans and AI (handling interruptions, timing responses) to make interactions feel natural.
Ensure Scalability & Resilience: Design services with a cloud-native approach (microservices, containers) for deployment on Kubernetes. Implement high-availability strategies (clustering, failover) across global data centers so that the platform achieves five-9s uptime with no downtime for maintenance.
Performance & Reliability Tuning: Continuously profile and improve system performance end-to-end. Minimize call setup times and audio latency through efficient coding (C/C++ for media processing) and system optimizations.
DevOps & Support: Use CI/CD pipelines to deploy updates safely with zero downtime. Write comprehensive automated tests (unit, integration, load) for your features. Participate in on-call rotation to troubleshoot and resolve production issues in real time, and implement lasting solutions to prevent recurrence.
Collaboration: Work closely with Product Managers, front-end teams, AI/ML team and with network engineers.
Qualifications
Real-Time Communications: 5+ years of experience developing VoIP or real-time communication systems. Strong knowledge of SIP protocol, WebRTC, and related networking (RTP, NAT traversal). Proven ability to implement call logic and troubleshoot signaling and media issues.
Audio/DSP Expertise: Hands-on experience with audio processing in real time. Familiarity with noise reduction, echo cancellation, jitter buffers, and other voice QoS techniques. Comfort optimizing or using audio codecs (Opus, G.711, etc.) and improving call quality under varying network conditions.
Strong Coding Skills: Proficiency in C/C++ for high-performance, multi-threaded systems programming. Experience writing efficient, low-latency code (lock-free structures, memory management). Additionally, skilled with a higher-level language like Go or Java for building microservices and control logic.
Cloud & Scalability: Experience building and deploying services in a cloud-native environment (Docker, Kubernetes). Knowledge of designing scalable microservices and using cloud infrastructure (AWS, GCP, or Azure) for load balancing, monitoring, and fault tolerance.
Voice AI Familiarity: Exposure to integrating speech recognition and text-to-speech in applications. You've perhaps worked with voice assistants, IVR systems, or call center AI - you understand basic latency/accuracy trade-offs and how to interface with speech APIs/SDKs.
Security & Compliance: Basic understanding of securing voice communications (TLS, SRTP) and safeguarding customer data (GDPR, HIPAA considerations for call recordings, etc.). Designs solutions with privacy and security best practices in mind.

Position Summary:
The Lead Direct Service Provider provides peer to peer direct service support to members enrolled in the Behavioral Health system. It is a key role for fulfilling The Arizona 12 Guiding Principles in the Children's System of Care.
Essential Duties/Responsibilities:
· Relay information between the member and staff, ensuring clear and accurate communication.
· Responsible for working with ages 0-25 which includes the Transitional Age Youth population (TAY).
· Report member feedback and any relevant issues directly to the onsite manager.
· Assist in coordinating meetings, appointments, or respite services.
· Complete administrative duties as directed by the site manager - Act as the point of contact for the team, addressing queries, resolving issues, and ensuring adherence to company policies and procedures when Manager is off site.
· Assist parents/caregivers in navigating systems and meeting the treatment goals for their enrolled members.
· Deliver interventions and activities that meet the needs of the child and build on their strengths in order to achieve positive outcomes.
· Collaborate with other individuals involved in providing support and care to families enrolled in services.
· Empower members by providing information, support, education, training, and connection with community resources.
· Empower parents/caregivers to advocate the treatment services for the enrolled member/their family.
· Provide clinical intervention to assist parent/caregiver in reaching the goals per the Individual Service Plan of members.
· Attends/facilitates Child Family/Adult Recovery Team Meetings for members assigned for Coordination of Care and maintain continued education with caregivers, case managers and other entities as part of the treatment team.
· Complete and maintain member documentation in accordance with regulations and MIKID best practices. Facilitate peer-to-peer support groups and/or provide skills support.
Supervisory Responsibilities:
Yes, The LDSP assists with additional administrative duties in support of their direct supervisor and peers.
Requirements
Education and Experience:
· High School Diploma or G.E.D required.
· 2 Years experience in the Behavioral Health field.
· Effective written and verbal communication skills.

Required Skills/Abilities:
· Must be able to demonstrate an accepting stance while responding with empathy and clarify to the family.
· Must be able to encourage collaboration and have knowledge of the system of care in the community in which he/she works.

· Provide services to parents/caregivers in the home and/or community.
Physical Requirements and Work Environment:
· Prolonged periods of sitting at a desk and working on a computer.
· Must be able to lift up to 50 pounds.
· Constantly stooping, bending, standing and walking during a shift.
· Must be able to be work in both indoor/outdoor working conditions including extreme weather (heat/cold).
· Must be able to remain in a stationary position 15% of the time.
· Occasionally moves about inside the office to access filing cabinets, office machinery, etc.
· Occasionally operates a computer and other office machinery, such as copy machine and printer.
· Occasionally ascends/descends stairs to visit other offices or attend meetings.

POSITION SUMMARY
Under the general direction of the Direct Service Manager, a Direct Care Provider is responsible to provide support, guidance, and direction, re-direction to the member attending Community or Center Based. Attendant Care: DSP's will support individuals in their home with personal care needs and may also provide companionship in community activities. (Example: assisting an individual with bathing; changing and laundering bedding; escorting an individual out with errands or desired activities).
Respite: DSP's will support and care for individuals while providing short term support/relief to the individuals primary caregivers. DSP's may also provide companionship in community activities or events. (Example: supporting an individual with their needs while the primary caregiver goes out; taking an individual to the playground, movies, stores, party, etc.).
Requirements / Qualifications:
Must possess or be able to obtain a Level 1 Arizona Fingerprint Clearance
Must have reliable transportation
Must have a valid drivers license
Must have a Flexible Schedule - Primarily available after school, evening and weekend hours
Must possess or be able to attend training for:
CPR and First Aid certification
Article 9 certification
Introduction to Developmental Disabilities certificate
Additional certification classes
Must be motivated to make a positive impact in the lives of others
Must have a desire to support others in reaching their full potential
Must be able to promote self-esteem with our members and be an advocate for them
Working experience with individuals with special needs (preferred)

POSITION SUMMARY
Under the general direction of the Direct Service Manager, a Direct Care Provider is responsible to provide support, guidance, and direction, re-direction to the member attending Community or Center Based. Direct Care Providers provide MIKID licensable services to the primary caregiver while supporting the emotional, physical and mental well-being of the member and his/her family. Direct care services may be provided in the child's home, a licensed facility or in the community.
Requirements
EDUCATION AND EXPERIENCE
High School degree or equivalent.
Must be age 21 or over. (Medical licensing regulation)
One year experience in behavioral health or childcare.
Effective written and verbal communication skills.
Excellent organizational skills.
Experience in the behavioral health field is preferred.
Ability to write and speak effectively.
Must be able to be work in both indoor/outdoor working conditions.
Bi-lingual a plus.
Preference will be given to person who has a family member with behavioral health issues or a person who has participated in a significant way in the life of a person with behavioral health issues.
CPR/1st Aid Certified
Current Arizona Fingerprint Clearance or the ability to obtain a clearance card.
Must have valid Arizona Driver's License, business use vehicle insurance and vehicle registration.
Must complete all necessary paperwork and mandatory training prior providing services.
