Delivers services that consist of support in the general areas of self-care, communication, fine and gross motor skills, mobility, therapeutic activities, personal adjustment, relationship development, socialization, in both the individual's home, and community setting.
Assists individuals in acquiring, maintaining, and improving self-help, domestic, socialization, and adaptive skills that will lead to their ability to become more independent with life skills.
In-home staff for clients with disabilities may perform a variety of essential tasks, including assisting individuals with daily personal hygiene routines like bathing, toothbrushing, and dressing. They also provide support with household tasks such as vacuuming, washing dishes, cleaning, cooking meals, and doing laundry.
Facilitates the acquisition of skills that enhance community integration and promote advancement in competitive employment opportunities. Collects data monitoring performance of the individuals supported and records progress related to identified outcomes.
Demonstrates effective problem-solving skills to address challenges promptly and proactively.
Records billing promptly upon service delivery, and fulfills other necessary paperwork as assigned.
Completes Electronic Visit Verifications (EVVs) for each delivery of service.
Transports individuals to and from specified community-based locations as needed.
Adheres to all assigned schedules, including punctual arrival and departure, requesting advance time off, and promptly informing the supervisor of any changes to the assigned work schedule.
Effectively utilizes technology to enhance service delivery, troubleshoots technology issues for seamless service, collaborates with IT for system maintenance and upgrades, and adheres to privacy and security protocols for electronic information.
Effectively treats individuals with dignity and respect by recognizing and valuing their inherent worth and unique perspectives.
Effectively communicates with stakeholders to ensure clarity, alignment, and informed decision-making.

PHYSICAL DEMANDS
The physical demands described are representative of those that must be met by an employee to successfully perform the essential functions of this job. The following positions or frequent movements are typical of the job: sitting, walking, standing, reaching, pushing, pulling, lifting, stooping, running, and crawling, often for extended periods of the workday. Assignments may require the ability to physically move clients during possible physical interventions to maintain a safe learning environment.
The position requires that an individual be able to learn new tasks quickly and effectively. Job requirements may change frequently. The incumbent must be able to travel to multiple locations as needed, when needed to conduct organizational business. Subject to inside environmental conditions.
EXPECTED SCHEDULE
This is a full-time 40-hour-per-week position, Monday thru Friday with varying shifts between 8am-5pm, as determined by management.
MINIMUM QUALIFICATIONS
Minimum High School diploma or GED required
Basic understanding of Microsoft Office products and Internet applications required
Strong computer and typing skills required
Community Participation Support Training required within 60 days of hire
Clean driving record (no more than 2 moving violations in a 3-year span), valid PA driver's license, current vehicle inspection, up-to-date vehicle registration, and proof of adequate insurance coverage required
Successful completion of Vista's Crisis Intervention training
Successful completion of ODP medication administration training.
Required passing of all pre-employment regulatory and safety-sensitive employee requirements including, but not limited to, PPD testing, physical exam, and drug tests

ESSENTIAL FUNCTIONS
Support individuals in developing and maintaining skills necessary for independent community participation.
Assist in planning and implementing activities that promote community engagement and social interaction.
Ensure the safety and well-being of individuals during community outings and activities.
Provide emotional support and encouragement to individuals to foster self-confidence and independence.
Assist individuals with activities of daily living as needed including, but not limited to, toileting, cleaning, washing dishes, cooking, and doing laundry.
Maintain accurate documentation of individual progress and incidents as required
Collect data monitoring performance of the individuals supported, and records progress related to identified outcomes
Document billing within 24 hours of service delivery
Record behavior support data as required
Document consumer funds for each client daily
Complete incident reporting form on day incident occurred, when applicable
Transport individuals to and from specified community-based locations as needed. Work all assigned schedules, including arriving and departing on time, requesting time off in advance, and notifying your supervisor of changes to your assigned work schedule.

PHYSICAL DEMANDS
The physical demands described are representative of those that must be met by an employee to successfully perform the essential functions of this job. Responsibilities
Here's what to expect on the job:
Establish and sustain a productive relationship with parents and guardians of Agency with Choice participants, and the designated Managing Employer and Support Service Professional(s) of each Agency with Choice participant.
Establish and sustain productive working relationships with all local and state agencies.
Conduct annual visits with each Managing Employer to support the Managing Employer, review satisfaction and compliance with Agency with Choice (performance reviews, updating of emergency back-up form, updating staff roles, updating HMA, completion of all required paperwork).
Attend all ISP meetings and document information into Matrix.
Update daily activity logs, task sheets, and transportation logs, as necessary, with specific participant outcomes; including the distribution of required forms to Managing Employers and Support Service Professionals.
Provide re-education training and on-going training for Managing Employers and Support Service Professionals.
Manage and audit all time submitted (review for accuracy, audit for over utilization and overtime, communicate with Managing Employer if over utilization occurs, review service documentation corresponding to time sheets). Manage overtime ensuring follow-up with Managing Employer regarding any concerns (i.e. issue notice memos).
Manage and record (i.e. Matrix activity notes) all on-going communications (i.e. e-mails and telephone calls) to and from Managing Employers, Support Service Professionals, and other staff members regarding services, complaints, questions, and/or concerns.
Qualifications
Minimum requirements:
Spanish/English bilingual is a plus, but not required
6 months to 2 years of previous job-related experience expected
High school diploma or General Education Development (GED) certificate required
A valid driver's license and 6 months driving experience
Access to a vehicle with valid insurance, registration, and inspection
