ESSENTIAL JOB FUNCTIONS: Review and Implementation of the Individualized Plan (ISP) and Behavior Support Plan (BSP). Commitment to providing quality services to individuals and their families. Monitoring of individual physical and emotional health. Follow operational policies and procedures developed by the Agency for all areas of individual care. Submit all required documentation by the required deadlines. Maintain open, positive, person centered communication with Supervisor(s) and stakeholders. Attend all required trainings as scheduled Adhere to staffing rotation expectations as established by CSS policy and program processes. MINIMUM REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: Confidence in interaction with individuals of all ages and with a variety of disabilities. Good written and verbal communication skills Be able to work independently and within a team atmosphere Ability to be flexible in all areas Resourceful in accomplishing tasks and directives Provide support without fostering dependence Make defendable, common sense decisions Pass CPI Association requirements for physical intervention, CPR, First Aid Training and Medication Administration in accordance with CSS policy. QUALIFICATIONS: Position must have a High School Diploma or GED Equivalency. Meet driver-approved standards as described in the Vehicle Usage Policy. A valid driver's license and good driving record, as well as proof of current auto insurance is required. It is required to have dependable transportation with functioning heat and air conditioning systems and functioning seat belts for safety for transporting individuals served. Prefer one (1) year experience working with individuals with disabilities. PHYSICAL REQUIREMENTS: Heavy lifting (over 50 lbs.) on a repeated basis required for care of individuals for performing job functions such as transferring. Also required to operate equipment requiring full hand dexterity, traverse unsteady terrain, ascend/descend stairs, bend, twist, stoop and kneel to perform the essential functions of the Behavior Support Professional position. Must be able to move objects, and use abdominal and lower back muscles to provide support over time without fatigue. Constant movement and use of limbs; this position requires good manual dexterity, coordination and stamina. As a routine part of this position it may be necessary to work in other facilities, work additional hours and may also be required to adjust schedules to meet the documented staffing needs of the individual(s) served. May be required to perform other duties as assigned by Supervisor to accomplish Agency goals and meet the needs of the individual(s) served. This position requires driving for business and transportation of individuals served. It is required to have dependable transportation with functioning heat and air conditioning systems and functioning seat belts for safety. The CSS provides services with or on behalf of a particular client in accordance with his/her individual rehabilitation plan to maximize the client's adjustment and functioning within the community while achieving sobriety and sustaining recovery, maximizing the involvement of natural support systems, and promoting client independence and responsibility. The CSS is part of an interdisciplinary team that assists consumers suffering from chemical dependency. S/he identifies life problems with the consumer through a structured assessment process and arranges services for consumers. S/he advocates for the consumer to obtain needed services and assistance to which the person is entitled and monitors the services provided. The CSS helps the consumer when s/he has a crisis. Work is performed under the supervision of a counselor supervisor or program manager and reviewed through direct observation, reports, and staff conferences. EXAMPLES OF WORK Work may include, but is not limited to, the following examples: Remains alert during working hours; attends work as scheduled, and complies with attendance policies. Attends multi-disciplinary treatment team meetings to identify strengths and needs related to development of the individual's rehabilitation plan. Provides clear and concise information regarding consumer problems, progress, and plans in a timely manner. Updates the rehabilitation plan accordingly. Attends periodic meetings with designated team members and the client, whenever feasible, to review and update the rehabilitation plan. Contacts consumers who have unexcused absence from the program in order to re-engage the person and promote recovery efforts. Arranges and refers consumers for services and resources and, when necessary, advocates on behalf of consumer to obtain the services. Monitors service delivery by providers external to the program and ensures communication and coordination of services. Locates and coordinates services and resources to resolve a crisis Provides experiential training in life skills and resource acquisition. Provides information and education to an individual in accordance with the person's rehabilitation plan. Develops plans with the consumer, for his/her discharge. Records and reports consumer participation, progress, and adjustment against treatment goals in a timely manner. Maintains timely, legible, clear, concise, and well organized tracking and other records of services provided. Develops and maintains effective communication and professional, cooperative working relationships with public assistance, social service organizations, and members of staff.
