Health Maintenance and Safety: Assists in maintain a safe environment for all the people we serve and follows all specialized care plans in accordance with the ISP (Individual Service Plan); provides care and assistance to promote good health and a healthy lifestyle in accordance with the ISP and physician's instructions; assists in recording information regarding health events, conditions, and status; assists in providing First Aid/CPR and seeks emergency medical care whenever required.
Personal Care: In accordance with the ISP, supports our clients as needed with administering personal cares which may include eating, grooming, dressing, bathing, and toileting; assists with mobility including lifting them from seated or lying down positions with the use of adaptive equipment as needed; may assist them with walking and maintaining stability.
Living Skills Development: In accordance with the ISP, provides support by teaching skills that foster independence and community integration. Such skills may include: cooking, money management, household shopping, laundry, personal hygiene, etc.; supports the client in maintaining their home and accessing activities in the community; supports their spiritual enrichment choices; records progress of their skill development and achievements.
Behavioral Development: Serves as a role model and mentor, demonstrating desired behaviors and coaching by using approved techniques and strategies; in accordance with the ISP may use approved physical intervention techniques when necessary to ensure everyone's safety. Must be able to work as part of a team to achieve positive outcomes for the client and programs.
Operational Support: Maintains cleanliness of the program area and/or home by performing housekeeping duties such as cleaning, washing clothes and dishes, maintaining cleanliness of the program vehicle, basic maintenance of the outside house appearance; completes tasks beyond the clients' abilities, follows all policies & procedures for handling money.

POSITION SUMMARY
The APP will provide patient care throughout the Urology continuum, maintaining expertise in assessment, diagnosis and treatment but focusing these skills toward Genitourinary symptom management and supportive care. This position will assist the physician in all aspects of patient care, including examining patients, performing complex history and physicals, compiling medical data, administering or ordering diagnostic tests and interpreting test results.
POSITION TECHNICAL RESPONSIBILITIES
Triage patients to determine their need and urgency for medical attention.
Perform comprehensive and problem-focused history and physical exams, interpret and correlate laboratory and diagnostics procedures. Record such information in a manner acceptable to the attending physician and the Health Information Management department. Formulate treatment plan within the scope of practice and in collaboration with supervising physician.
Order and interpret diagnostic studies.
Prepare patients for key components of surgery including preoperative and postoperative education, implementing protocols and procedures and verifying understanding of preparative information.
Assist as needed with procedures in clinic including BCG, urodynamics, cystoscopy, stent removal, vasectomies, and PTNS.
Assess, evaluate and treat (within scope), patients appearing for treatment on an emergency basis.
Triage phone calls and provide advice for medical questions.
Perform emergency department and inpatient consults as appropriate.
Perform daily rounds on inpatients.
Order & coordinate care with other services/specialties.
Discharge patients and dictate discharge summaries.
Perform health assessments by interviewing patient and family members as appropriate.
Educate and counsel patient and/or family care giver regarding diagnosis, treatment and prevention.
Promote wellness through screening and prevention.
Document examination outcomes, referral physician letters and other records. Ensure that attending physician verifies the final diagnosis and the examination outcome documentation.
Order, prescribe and administer medications and devices as allowed under Wisconsin law.

Education:
Required: Nurse Practitioner: Master or Doctorate degree from accredited Nursing Program or Physician Assistant: Bachelor or Master's degree from accredited PA Program.

Read and understand individualized service plans (ISPs) for each resident and provide support to the residents per their service plan

Provides transportation assistance to students for home visits or appointments and delivers medications.

Qualifications
High School Diploma or equivalent required. Associate or bachelor's degree in a social or behavioral science field preferred.
