PRINCIPAL DUTIES AND RESPONSIBILITIES: Provide direct patient care to Betances patients. Diagnose and treat acute illness, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes. Maintenance of a clinic panel of people living with HIV and other patients appropriate to ongoing infectious disease oversight. Prescribe or administer medication, therapy and other specialized medical care to treat or prevent illness, disease or injury. Explain procedures and discuss test results or prescribed treatments with patients. Monitor patients' conditions and progress, re-evaluating treatments as necessary. Provide health and wellness advice; order age-appropriate screenings, immunize patients against preventable diseases. Adhere to documentation and quality standards set by New York State and any other standards implemented by the Chief Medical Officer. Refer patients to other medical specialists, when necessary. Maintain an ongoing relationship with back-up hospital and its professionals. Ultimately, have admitting privileges at that institution. Timely completion of accurate and detailed notes, patient care forms, and letters. Must adhere to BHC policies and procedures. Must participate and complete quarterly peer reviews in a timely fashion. Must attend monthly Provider Meetings. What Your Role Will Be: Transport participants to various community and recreational activities that support skill-building and personal growth Ensuring safety and providing appropriate supervision of participant support in the community Lead group sessions where participants can learn practical skills for daily life in the community, such as communication, teamwork, and problem-solving. Actively engage participants in activities, encouraging participation and maintaining interest to create a positive and meaningful experience. Foster independence by assisting participants to develop work, recreational, and social skills Assist individuals in identifying and working towards life goals Serve as a positive role model, building trust and supportive relationships with participants. Supports the participants achieve their personal goals to improve their quality of life Minimum Qualifications: High school diploma or GED required. Valid NYS driver's license with at least 1 year of driving experience. 3 - 5 years of experience in roles such as Day Habilitation Specialist, Paraprofessional, Lead Teacher, Teacher's Aide, or Home Health Aide. Preferred experience with individuals with intellectual and developmental disabilities. Comfortable operating an agency vehicle (caravan). Must possess and maintain a personal cell phone for work-related communication; this is a condition of employment. Work Location: Starts and ends in Westbury, NY (Nassau County) Work Hours/Schedule: Full time, Monday - Friday, 8 AM - 4 PM Pay: $20.00/hr.     Direct Service Provider (DSP) Bilingual Mandarin Salary Range: $16.52-$19/hr Part Time, Flexible Hours Approx 6 hours a week Essential Duties and Responsibilities: The role of a Direct Support Professional (DSP) is to work with children and their families to provide resources and support, as well as strengthen daily living skills. Applicants can choose to work in our OWPDD program with individuals who have developmental disabilities or in our HCBS program with individuals facing mental health challenges. The services are provided in the client's home or community setting giving the families respite while gaining important life skills. Activities as a DSP for the OPWDD program recipients primarily involve teaching them the following: how to accomplish daily living tasks, improve socialization among peers, and acclimate to community activities (i.e. traveling, interacting with store staff/community helpers, and adhering to appropriate social boundaries in various settings). Assist individuals in meeting their valued outcomes as outlined in their service plan. Set schedule according to the needs of the client and the DSP. Communicate effectively with families and supervisors. Problem solve and communicate with supervisors as concerns arise. Complete and submit documentation detailing the activities engaged with the individuals in a timely manner. Bilingual in Mandarin Complete mandatory trainings. Education and/or Work Experience Requirements: Prior experience providing direct services to individuals with a developmental and/or intellectual disability OR pursuing a degree in education, social work, nursing, or psychology. Computer proficiency (MS Office Word, Excel and Outlook). Can communicate in either Mandarin or Cantonese as well as English. Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices. Applicants must be at least 18 years old and legal to work in the US. High school diploma or GED required, Associates or Bachelor's Degree preferred, in some cases a Bachelor's Degree may be required.
