Responsibilities

Participates in orientation and education of staff and students.
Obtains cardiac critical care patient history and performs assessment via observation, interview and examination.

Provides direct treatment and management of cardiac critical care conditions via prescriptive authority or referral to other healthcare providers or community resources. Engages in discharge planning.

Required Qualifications
Physician's Assistant or Licensed Registered Nurse credentialed from the Texas Board of Nursing. Licensed Advanced Practice Nurse Practitioner with Prescriptive Authority specializing in Cardiac Critical Care. Credentialed from the Texas Board of Nursing in Acute Care nursing. Experience in caring for children with complex congenital and acquired heart disease in a Pediatric and Congenital Critical Care Unit. Highly proficient in Microsoft Office applications. Ability to work efficiently and flexibly in a fast-paced environment with minimal guidance. Ability to address individuals and information with a high degree of sensitivity and confidentiality in various situations. Capacity to work well with physicians and other care providers as part of a multi-disciplinary healthcare delivery team.

Responsibilities
Develop and maintain provider engagement, oversight, and education using quality oversight processes.
Proactively identify enrollee needs across the HSCSN provider network through the lens of quality monitoring, oversight, provider education, connection to care, delivery of services, and transition out of care in partnership with multiple teams and stakeholders.
Ensure high quality provider engagement & collaboration through comprehensive training, quality site visits, and audits.

Ensure Home health agency oversight, training, and monitoring of corrective action plans and receive individual and small group refresher training sessions and resource support.
Convene quarterly Home Health Agency sub-committee in collaboration with HSCSN internal stakeholders (Medical Affairs, Provider Relations, Compliance, Quality).
Ensure HSCSN provider focused quality engagement activities and home health oversight process and outcomes are included in the Quality Assessment and Performance Improvement (QAPI) Program, workplan, and evaluation.
Participate in the Mortality Review Workgroup to review mortality events for prevention and improvement opportunities as well as measuring process measures related to contractual compliance with notification of mortality events.
Schedule and coordinate internal monitoring/audits; conduct site visits of every home health agency in the HSCSN Provider Network at minimum annually, but more frequently when required by regulator, leadership, or required by corrective action plan.
Conduct unannounced site visits on a routine basis to evaluate quality of clinical care and ensure compliance with contractural standards.
Ensure that only home health agencies with a pediatric certificate of need (CON), administered by DOH, provide home health services to children and adolescents.
Submit to DHCF annually, for review and approval, all on-site monitoring and evaluation tools utilized for home health agencies by the Quality.
Submit any proposed revisions to monitoring and evaluation tools to DHCF for prior approval before use.
Participate as home care lead in external audits (I.e. OSR) or accrediting efforts (I.e NCQA) to make sure HSCSN's policies and procedures are standards and striving for excellence.
May perform other duties in addition to those outlined in this job description.

Basic Education Standards for positions in the 1102 series:
Bachelor's degree from an accredited education institution authorized to grant baccalaureate degrees.

Current DoD employees or Armed Forces members, who occupied GS-1102 positions or contracting positions with authority to award or administer contracts above the simplified acquisition threshold in an Executive Department on or before September 30, 2000, are exempt from meeting this requirement. Performs endoscopic or open harvesting of the radial artery during coronary artery bypass procedures and prepares radial artery for bypass.
Qualifications
PAs: Bachelor's degree, graduation from an ARC-PA accredited Physician Assistant program, and certification by the National Commission of Certification of Physician's Assistants are required. Candidates must hold an active Maryland license through the Maryland Board of Physicians.
NPs: Bachelor's degree, RN experience, and completion of Acute Nurse Practitioner degree. Candidates must hold an active Maryland license through the Maryland Board of Nursing.
Preferred 5 years of experience as a Physician Assistant or Nurse Practitioner providing direct care to patients preferably in a surgical program setting.
Accepting NP graduate with 5 years as RN in Cardiac Surgery ICU
Credentialing must be obtained from UM Capital Region Health's Medical Staff Office and maintenance of BLS /ACLS certification is required.
Salary is commensurate with clinical experience.

What we need from you.
A high school diploma, associate degree preferred.
Two years of positively referenced work, student, or volunteer experience
Must obtain the Maryland Certified Peer Recovery Specialist (CPRS) within one year of hire.
Self-identifies as an individual with lived behavioral health experience.
A driver's license with 3-points or less and access to an insured vehicle.
Must have a strong commitment to the right and ability of each person living with severe mental illness to live in the community, work in competitive jobs, and have access to helpful, adequate, and continuous support and services.
