OVERVIEW OF POSITION: Attends to the personal care of adults with developmental disabilities living in a licensed residential home. Assists and participates in the planning, coordination and delivery of daily living activities; including outings, to facilitate development of living skills for the residents and help them achieve a higher level of independence.
ESSENTIAL FUNCTION:
Provides personal care for residents which may include assistance with eating, personal hygiene,showering, restroom assistance, and preparing for bedtime. Assists and/or prepares meals incompliance with dietary restrictions; assists individuals to eat, including those who may requireassistance with G-tube feeding.
Assists in the administration and monitoring of medications.
Assists in care and cleaning of residential home, which includes housekeeping, & laundering ofresidents' clothing.
Conducts daily activities to assist in the development of the residents' living skills, consistent withtheir Individual Program Plan (IPP) and Individual Support Plan (ISP) goals. Provides continuous activetreatment and engagement.
Completes all required reports and documentation on a daily basis. Attends all necessary staffmeetings and completes all required mandatory trainings.
May use company provided vehicle to transport individuals to health care appointments and othervarious locations. Participates in recreational activities in a variety of settings.
Performs other duties as assigned
EDUCATION:
Typically requires High School Diploma, GED, or national equivalent.|DSP-I and DSP-II Certification required or must be attained within first year of hire or first year inposition.|Must possess and maintain valid CPR, CPI and First Aid Certifications.
EXPERIENCE:
1 to 2 years' experience, or equivalent combination of training, education and experience to performthe job successfully.
1-year minimum experience in a licensed residential setting working withmedical fragile adults with developmental disabilities and/or behavioral issues.
KNOWLEDGE, SKILLS, ABILITIES:
Familiarity with charting, medication administration, and positive behavior support.
Working knowledge of behavioral issues, including aggressive, non-verbal and self-injury behaviors.
Working knowledge and understanding of Individual Program Plans and Individual Service Plans.
Working knowledge of Regional Center, Department of Developmental Services, Title 17 and 22.
Ability to communicate well orally and in writing; Ability to interpret and carry out written and/ororal instructions.
Able to work in indoor and outdoor settings, arrange and participate in outdoor activities withresidents of the residential home.
Ability to consistently demonstrate good judgment and decision-making skills; Ability to exercisediscretion and confidentiality pertaining to the work environment.
Ability to pass post-offer medical examination and test for tuberculosis.
Able to obtain and maintain a criminal record/fingerprint clearance from the Department of Justiceand Federal Bureau of Investigation per Easter Seals Southern California and/or programrequirements.
If applicable, able to possess andmaintain driving record in compliance with Transportation Safety Standards, and valid Californiadriver's license; maintain proper auto insurance and vehicle registration.

Location: Glendora- Community-based program - Behavior Program
Hours: Full-time - Monday-Friday 8:00 am to 3:00 pm
Hourly Starting Pay Rate: $22.00 per hour - *Must have one year of behavior management experience*
*We provide CPR/First Aid certification for Adult Day Services*

Responsibilities
Assists individuals with their assessments, planning, and the evaluation process. May support individuals in preparing for and attending meetings with their support team, as well as advocating and facilitating changes to the service plan as necessary.
Assists in the development and implementation of an Individual Service Plan (ISP) consistent with the individual's personal goals. Maintains required documentation and data, including ISP's, quarterly and annual evaluations, and case notes. When indicated, works collaboratively with service consultants (OT's, LVN's, Behavioral Consultants, etc.) on enhanced service delivery, and completes related documentation and data input. When indicated, understands and supports an individual's behavior plan.
Assists individual with the selection of person-centered activities and life skills learning experiences, in accordance with individual desires, ISP and IPP (Individual Program Plan) objectives as needed.
Works with other service members in the planning and delivery of activities consistent with individual and service objectives.
Supervises individuals while they are at the service, and provides personal care support as required; which may include nutrition, personal hygiene, and assisting the individual to look their best.
Supports individuals in finding their preferred way to communicate their needs and choices, and educates them in the use of the appropriate tools to help them find their voice.
Provides guidance to Life Skills Coach 1 and direct care associates, as necessary.
Safely and effectively uses all technology, alternative communication and adaptive equipment related to the individuals and the service. Trains individuals in the use of assistive technology as directed.
Follows procedures, standards, guidelines, and best practices established in designated services; and adheres and complies with applicable regulations external to the organization, to ensure health and safety of individuals and service line associates, compliance and quality of services. Attends in-service trainings and adheres and meetings as required.
Performs other duties as assigned.
Qualifications
EXPERIENCE:
Four to six (4-6) years of related experience is preferred. One year experience working with individuals with challenging behavior is required for positions that will support behavioral services.Or equivalent combination of experience, education, and training to perform the job successfully.
KNOWLEDGE, SKILLS, ABILITIES:
Knowledge of person-centered programming approach providing services to create purposeful and meaningful activities.
Ability to demonstrate actively listening, patience, and empathy in communicating with individuals that use the service.
Able to be creative and flexible, problem solve, organize and complete tasks in a timely manner, follow directions of supervisor, take initiative, work effectively with other team members and volunteers.
Able to interact appropriately and maintain positive working relationships with associates, stakeholders, individuals and their family members, caregivers, and the general public.
Ability to consistently demonstrate good judgment; exercise discretion and handle sensitive and confidential matters appropriately.
Ability to properly interpret and follow policies, procedures, and regulations.
Intermediate proficiency with Microsoft Office applications (e.g. Outlook, Excel, Word, and PowerPoint).
Ability to provide assistance with personal care to participants, including ability to transfer an individual who has no weight-bearing skills.
Ability to resolve conflicts and remain calm and collective in stressful environment.
Ability to lift up to 50 pounds repetitively. Ability to walk, stoop, stand, and kneel for extended periods of time. Ability to push wheelchairs on a variety of terrain. Must be able to tolerate an outdoor work environment inclusive of a variety of weather conditions. Move and re-arrange furniture (i.e.: tables and chairs for activities).
Ability to travel locally with individuals who use the service to participate in activities related to the service.
Ability to obtain and maintain a criminal record/fingerprint clearance from the Department of Justice and Federal Bureau of Investigation per Easterseals Southern California and/or program requirements.
Ability to provide proof of required vaccinations or positive titer showing immunity.
Ability to pass post-offer medical examination and test for tuberculosis. Ability to provide proof of required vaccinations or positive titer showing immunity.
Must pass all drug testing required by ESSC.
EDUCATION:
High School diploma.
