The Opportunity
Seeking Hospital Medicine physician assistants and nurse practitioners.
Current national certification, DEA, ACLS, and BLS are required.
At least two (2) years of hospitalist/inpatient experience is required.
PRN day shifts available, typically 7 AM to 5 PM, with occasional weekend coverage; responsibilities include rounding on 12 to 14 patients.
Candidates wanting to work in an academic setting with current residents desired.
Providers with strong clinical skills, excellent bedside manner, compassion, good communication skills, creative problem-solving skills, team player attitude, and a desire to learn and advance in their career.
Current VA state license is a plus.

Scope of Role:
Responsible for providing direct support within a single program or unit within a program, typically supporting 4 - 6 individuals in residential settings, or up to 10 individualsin periodic programs.

Records: Accurately documents progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality.

General Health Care: Monitors individual's health; documents health concerns; communicates with nurse or supervisor as appropriate; calls 911 in the event of emergency; follows individual health care directives.

Milage Reimbursement
Beginning the day at a remote location: The staff member shall be reimbursed for milage for the distance from their home base to an assigned location and back to their home base whenever that distance from their residence to the assigned location is greater than their normal commute to the home base.
Traveling from home base to a remote location after reporting to home base: The staff member shall be reimbursed for milage for the distance from their home base to an assigned location and back to their home base. At the end of the shift, if the distance to their residence is less than their normal return trip to their residence at the end of the shift then no milage reimbursement shall be made.
SOAR365 updates the milage reimbursement rate in accordance with the Internal Revenue Service guidance.

Delivers purposeful age-appropriate activities and supports for participants.
Implements activities, using specified training techniques and interventions, related to, but not limited to, developing participant social interaction skills, personal hygiene training, motor skill coordination, and activities etc.
Collaborates with other staff within Day Support and Respite programs.
Administers medications to participants as required.
Ensures participants are engaged in activities and offers choice to participants.

Learns safety procedures and how to correctly use belts and straps in vehicles.

COMPENSATION AND MILAGE REIMBURSEMENTS:
The DSP Floater position shall be compensated at one hourly pay grade higher than a Direct Support Professional.
Milage Reimbursement
Beginning the day at a remote location: The staff member shall be reimbursed for milage for the distance from their home base to an assigned location and back to their home base whenever that distance from their residence to the assigned location is greater than their normal commute to the home base.
Traveling from home base to a remote location after reporting to home base: The staff member shall be reimbursed for milage for the distance from their home base to an assigned location and back to their home base. At the end of the shift, if the distance to their residence is less than their normal return trip to their residence at the end of the shift then no milage reimbursement shall be made.
SOAR365 updates the milage reimbursement rate in accordance with the Internal Revenue Service guidance.
Requirements and Qualifications:
High School / GED graduation required.
One year of experience working with persons with disabilities preferred.
Medication Administration certified or will obtain within 60 days of starting date and maintains certification throughout employment completing continuing education and recertification requirements as defined by SOAR365 in a timely manner.

The Sponsored Residential DSP provides services to adults with developmental disabilities in the sponsor DSP's home. The Sponsored Residential DSP ensures that they, their home, and their backup staff maintain compliance with the office of licensing and human rights under the Department for Behavioral Health and Developmental Disabilities (DBHDS), DMAS, and others as required.
Additionally, the role is expected to ensure that services are aligned with Heart Havens' mission and values. The Sponsored Residential DSP is expected to demonstrate person-centered skills, values, and attitudes in their communication, documentation, and support provided to the residents. Education and Experience: High school diploma or GED preferred. 6 months previous direct support experience required.

Essential Functions:
-Provides support to residents as outlined in their ISPs, Residential Plan for Supports, and other relevant support
plans (e.g. Behavior support plan)
-Understands and supports residents according to their health protocols and health related support instructions
-Provides residents with appropriate supervision when working in the program and never leaves resident alone in
the home or in the community unless written in their individualized support plan
-Supports residents with their Activities of Daily Living to the level needed for their success
-Encourages residents to engage with others, increase independence, and participate in meaningful activities at
home and in the community
-Encourages and support residents' individual choice in their day-to-day lives and respects their choices
-Administers medications according to organizational procedures and the 6 Rights of Medication Administration
-Uses customer service skills when interacting with residents, families, case managers, medical professionals, etc.
-Completes accurate and timely daily documentation to include but not limited to ISP Notes and T-Logs

submitting documentation that reflects that care
-Notifies program leadership of incidents, behavioral changes, medical updates, and important messages (e.g.
phone messages)
-Completes incident reports (General Event Reports) in a timely manner and seeks guidance as needed
-Follows dietary recommendations, food preparation orders during meal preparation, and
documents any menu changes
-Documents individual choice and preferences in documentation
