We have full and part-time positions open in our Community Support and Residential programs. A NYS Driver's license is required.

Position Overview

Essential Job Functions
As a Direct Support Professional (DSP) you will professionally support adult or pediatric individuals with an intellectual and/or developmental disability where they live, work, or in other community settings, to promote their emotional, physical, and personal well-being.
Implement strategies to assist the individual in obtaining their outcome as stated in the (ISP) Individual Support Plan.
Complete documentation to substantiate services(s) being delivered. (EVV, daily service note).
Assist the Individual and the Individual's interdisciplinary team in the development and maintenance of the Individual's ISP (Individual Support Plan).
Immediately reports any changes or problems in the Individuals condition to manager.
Required to complete all documentation in accordance with state and program regulations, Aveanna HealthCare policies and procedures and any applicable accreditation standards.
Always adhere to confidentiality standards and professional boundaries.
Assist individuals reach their goals to be more independent by offering guidance, instruction, and role modeling.
Actively search for opportunities for those you serve to experience life in their homes and communities based on their hobbies and interests.
Support the individuals' preferences and ensure their interests are honored.
Assist individuals in the development of appropriate life skills necessary to reach their highest level of potential including interaction with others, cooking, housekeeping, budgeting, personal hygiene, etc.

The AmeriCorps program collaborates with many community organizations to provide support and services. MHA's AmeriCorps members (members) must commit to one year of service directly in the community. Current slots available are 675 - 900 hours over the course of the year, with a bi-weekly living stipend ranging from $9,000 to $12,000 annually based on hours served. Upon successful completion of the program, members are also eligible for an Education Award, which can be applied toward college tuition or existing student loans.
Recreation Attendant- Facilitates an environment where youth can build friendships and engage in healthy social interactions through daily program activities. The position offers 12-19.5 hours per week over a 50 week school year.
The Hudson LEAPS Afterschool program provides educational support, recreational opportunity, skill building, character education and a nutritional snack for each participant during three hours of structured daily activities. Collaborations with service providers offer specialized services for participating youth. With a focus on providing youth with a message promoting positive attitude regarding school, delaying childbearing and violence prevention, the program offers interesting and engaging alternatives in Hudson for up to 240 youth (K-8th grade). The programs are available every school day and students are expected to participate in each 3-hour session.

The Department of Developmental Services (DDS) seeks a caring and compassionate applicant for the role of Direct Care Worker II (DCW II). The DCW II position is a shift supervisor position that provides oversight and direct services to individuals with developmental disabilities. Duties and Responsibilities (these duties are a general summary and not all inclusive):
· Assist individuals in meeting his/her personal care and nutritional needs;
· Assist and support individuals in making choices in his/her life;
· Maintain individuals' rights by complying with applicable laws and regulations;
· Provide leisure activities; may drive individuals to work, appointments, and to recreational activities;
· Provide daily supervision and training to Direct Care Worker I staff to ensure that they are acting as advocates for the individuals and demonstrate; respect and positive regard for individuals with developmental disabilities;
· Oversee, as the shift supervisor, the health and safety of the individuals by administering medications;
· Communicate with appropriate medical and health care providers regarding health concerns and accompany individuals to various appointments as necessary.
Preferred Qualifications:
· Knowledge of practices and procedures necessary to assure the health and safety of individuals, including evacuation procedures.
· Ability to plan, assign, and coordinate the activities of others.
· Ability to give written or oral instructions in a clear and understandable manner.

Person-Centered Support: Work collaboratively with individuals to identify their strengths, goals, and needs, and develop individualized plans to support their recovery journey.
Skill Development: Assist individuals in developing social, interpersonal, and daily living skills to enhance their ability to live independently and participate in the community.
Advocacy and Empowerment: Advocate for individuals' rights and needs, promoting self-advocacy and empowerment to help them navigate the system and achieve their goals.
Resource Navigation: Help individuals access and utilize community resources, including housing, employment, education, and healthcare services.

Education Requirements differ for each service that can be provided, please read all services above and the educational requirements.
