Responsibilities include providing daily living support to people with disabilities in their home, assisting them to achieve their personal goals. Duties include, but are not limited to, assisting with personal care, bathing, mealtime, positioning, lifting, shopping, and involvement in the community. MISSION STATEMENT Together, we create a warm and welcoming community enriching the lives of those we support. ESSENTIAL RESPONSIBILITIES 1. Supervision a. Staff training and supervision b. Assist in the evaluation process for employees c. Carry out assigned duties when the Manager is absent 2. Scheduling a. Assist the Manager with scheduling and staffing 3. Support plans a. Provide input and information into the development of individual support plans. b. Maintain knowledge and ensure implementation of each person's individualized support plan (including behavior, dietary, medical, etc.). 4. Community involvement a. Assist people in accessing and participating in community services and events of their choice. b. Assist people in developing and maintaining relationships within the community. 5. Driving a. Safely operating and maintaining agency vehicles. b. Assure safety by properly using tie downs, seat-belts, lifts, and all other required safety equipment. 6. Training a. Complete required training (OSHA, CPR, OMA, Relias Learning, and other training as required). 7. Confidentiality a. Maintain confidentiality in all environments. 8. Safety a. Perform safe transfers and re-positioning specific for each person's needs. b. Recognize, report, and react to any safety hazards in all environments and in a timely manner. c. Comply with outside agency regulations (Dept. of Health, ARSD, OSHA, etc.) 9. Personal care a. Respect individual choice while providing personal care. b. Provide quality assistance with personal care (using the bathroom, bathing, eating, oral hygiene, dressing, medical care, etc.). 10. Health a. Safe and proper administration of medications and treatments. b. Recognize & timely report health concerns. 11. Dietary a. Assist with following dietary requirements and individualized diet plans, including preparing meals according to dietary guidelines. 12. Documentation a. Complete and accurate documentation in all required areas (ie. Therap, programs, services, computer, and fabrications) using professional language. 13. Documentation Oversight a. Ensure Direct Support Professionals are completing accurate documentation in all required areas (ie. Therap, programs, services, fabrications) using professional language. 14. Cleaning a. Complete cleaning tasks as assigned. b. Follow the cleaning product usage guide. 15. Teamwork a. Consistently work in a positive and cooperative manner with fellow team members. b. Effective communication between coworkers, shifts, and departments. c. Consistently taking the initiative to complete job requirements. 16. Relationships a. Develop and maintain professional and respectful relationships with all DakotAbilities stakeholders. 17. Attendance a. Work shifts as assigned with regular and reliable attendance. b. Follow the guidelines for contacting a manager with scheduling issues c. Attend staff meetings. 18. Contract Work a. Ensure compliance with Labor Laws to ensure proper payment for work done by people supported. b. Ensure that work completed is done at 100% accuracy for quality control. c. Documentation is completed for work done by people supported and turned in on time. PHYSICAL REQUIREMENTS Ability to stay awake and alert during all shifts. Ability to work in awkward positions. Ability to work in sustained flexion and crouched positions. The following actions are performed continuously: - Lift > 10 lbs. and see.
