Responsibilities

Assists in training and monitoring of individuals in a variety of service areas per each clients Individual Support Plan/Person Centered Plan.
Provides directsupport services/add-on services, documents the effects of services as goals are performed, and provides entries into client records for goal progress within 24 hours of services rendered, as per each clients Individual Support Plan/Person Centered Plan. Ensures that all services are delivered in a manner consistent with RYANS PLACE, LLC. s mission, values, and philosophy and with the service parameters of Medicaid funded services.
Encourages independence of the individual by following habilitation plans (Short Term Goals) and all related documentation and procedures (this includes medical treatments, Medication Administration procedures, behavioral and medical observations/interventions, and any additional clinical documentation to be kept daily according to Client Specific Competencies). Ensures that individuals fully participate possible in all daily living routines.
Is trained and performs all duties including, but not limited to: partial to full physical assistance for lifting, positioning, transferring, personal care, etc. Independent Contractors are required to provide written statements regarding any physical restrictions for work confirmed by a physician/physicians assistant. Operates motor vehicles as necessary to transport clients with valid documentation of a Drivers License, Auto Insurance Declaration Page, and a clean/current driving record.
Attends all assigned meetings, trainings, and clinical supervisions to acquire necessary skills. Participates in 10 hours of Continuing Education Units (CEUs) annually according to Policy 8P.
Qualifications

Completes the personal belongings inventory for individuals on admission and per ComServ, Inc. policy following admission as assigned by the supervisor. (residential only)
Purchases food according to recommended diets and ensures food purchases are completed. (residential only)
Financial:
Adheres to financial policies and procedures of the program and company.
Supervision:
Not applicable.
Regulatory:
Ensures and maintains a clean and safe work and living area. Reports problem areas to the direct supervisor.
Adheres to all regulatory standards, organizational policies and procedures and monitors compliance across shifts. (i.e. fire drills and disaster drills, confidentiality, etc.)
Checks water temperatures weekly.

Follows and implements behavioral interventions as specified in the Person Centered Plan/Individual Program Plan/Crisis Plan/Behavioral Support Plan and behavioral expectations of the program service.
