Position Summary:
As a Direct Support Professional, you will provide hands-on support, supervision, and companionship to individuals with intellectual and developmental disabilities in residential, day, or community-based settings. Your role is essential in helping individuals achieve their personal goals, maintain health and safety, and enhance their quality of life.
Key Responsibilities:
Provide direct care and supervision to individuals in accordance with individualized service plans (ISPs).
Support clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and meal preparation.
Assist with medication administration and health-related tasks (training provided).
Foster community integration by accompanying individuals to appointments, recreational activities, and volunteer/work sites.
Document daily activities, progress notes, behavioral observations, and incidents accurately and timely.
Promote independence, dignity, and choice for all individuals supported.
Respond appropriately to crisis situations, using de-escalation techniques and behavioral support plans when necessary.
Maintain a safe, clean, and positive environment in all settings.
Qualifications:
High school diploma or GED required.
Must be at least 18 years old.
Valid driver's license and acceptable driving record (preferred).
Ability to pass a background check and drug screening.
CPR/First Aid certification (or willingness to obtain upon hire).
Experience working with individuals with developmental disabilities or in a caregiving role preferred, but not required-training provided.
Strong communication, problem-solving, and teamwork skills.

1. Is trained and approved by OMHAS as Peer Support Specialist.
2. Training and experience in Trauma Informed Care, recognizing the impact of trauma on self as caregiver, as well as client. Gives input from a trauma-recovery perspective into the Civilian Mobile Crisis program and procedures affecting treatment of clients, with the aim of reducing or eliminating re-traumatizing clients, providing choice, respect, and consideration for clients and their recovery.
3. Documents observations and interactions in electronic health record.
4. Assists with dangerous and potentially dangerous situations and takes appropriate measures to assure client/worker safety.
5. Takes initiative to interact with client and to help clients feel as comfortable as possible while at Netcare.
6. Provides exceptional customer service to clients, community members, coworkers, and other professionals.
7. Documents and bills appropriately for all client interactions.
8. Maintains productivity standards as determined by management.
9. Reviews email and internal mail delivery daily and responds if needed in a timely manner.
10. Performs other related duties as required.
11. Provides services to, or on behalf of clients in person or via telephone including (but not limited to):
a. Assisting in the implementation of treatment plans in accordance with identified time frames.

Bathe, shave, dress and undress, care for hair, finger nails, eyes and ears (cleaning eyeglasses, etc.), perform daily dental care, perineal care, as well as any other customary daily activities of living for residents.
Lift, turn, move, position, transfer and transport residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, vehicles, etc.; assist resident with walking with or without self-help devices.
Respond to resident calls promptly. Ensure call cords are within reach of residents. Ensure residents who are unable to call for help are checked frequently.
Assist resident with bowel and bladder functions; take to bathroom, offer bedpan/urinal, etc. Collect specimens as instructed.
Keep incontinent residents clean and dry. Check and report bowel movements and character of stools as instructed. Change clothing, linens, etc. when it becomes wet or soiled.
Change bed linens, make beds (occupied and unoccupied); keep linens tight to avoid wrinkles from forming under the resident; provide extra covers as requested.
Routinely checks on residents to ensure personal care needs are being met. Turn bedfast residents at least every two (2) hours.
Perform restorative, rehabilitative and special treatments procedures as instructed. Applies slings, ace bandages, and braces as instructed. Provide Range of Motion exercises.
Assist resident in preparing for activity and social programs, meals, appointments, medical tests, etc.
Prepare residents for meals and perform after meal care; take to bathroom, wash hands and face, transport to and from, brush teeth, clean dentures, etc.
Observe and record intake of food and fluids; report changes in eating habits.
Serve food trays, assist with feeding as indicated (cutting foods, feeding, orienting to food items and placement and temperatures). Keep water pitchers clean and filled with fresh water and within easy reach of the resident. Serve snacks.
Weigh and measure residents as instructed. Measure and record temperatures, pulse, and respirations (TPRs), as instructed. Observe and report symptoms, reactions, and changes, such as the presence of pressure areas, bedsores, and skin tears.
Participate in and receive nursing reports at shift changes. Assist with carrying out the service plan document (AL) or care plan document (HC), through the point of care application.

JOB SUMMARY:
Community Psychiatric Supportive Treatment (CPST) Service provides an array of services delivered by community based, mobile individuals or multidisciplinary teams of professionals and trained others. Services address the individualized mental health needs of the client. They are directed towards adults, children, adolescents, and families and will vary with respect to hours, type and intensity of services, depending on the changing needs of each individual. The purpose/intent of CPST is to provide specific, measurable, and individualized services to each person served. CPST services should be focused on the individual's ability to succeed in the community; to identify and access needed services, and to show improvement in school, work and family and integration and contributions within the community.
ESSENTIAL FUNCTIONS AND DUTIES: The CPST service is comprised of the following activities as they relate to the individual's symptoms of mental illness and corresponding deficits in current functioning:
(1) Coordination and implementation of the service recipient's ISP, including ensuring that the ISP reflects the most current interventions necessary to address the individual's mental health needs and symptoms of his/her mental illness, as evidenced by the service provider's:
(a) Participation in the development of the ISP;
(b) Coordinating other services and providers identified in the ISP to ensure that the ISP is being implemented as written;
(c) Monitoring the individual's progress in achieving goals and objectives/anticipated outcomes as documented on the ISP; and
(d) Monitoring the individual's status in relation to his/her ISP goals to identify when a change in mental illness symptoms indicates the need for a clinical review of the individual's mental health assessment and ISP. Such clinical review shall be performed by an appropriately qualified individual in order to determine whether a revision of the goals, objectives and/or interventions is warranted.
(2) Support in crisis situations, including the service provider:
(a) Working with the individual, and family, guardian and/or significant other, as appropriate, to develop a crisis management and contingency plan; and
(b) Coordinating and/or assisting in crisis management and stabilization as indicated.
(3) Assessing the individual's needs, including psychiatric, physical health, entitlement benefits, wellness, support system, and community resources, e.g., the need for housing, vocational assistance, income support, transportation, etc., in order to:
(a) Incorporate those needs and accompanying rehabilitative services and activities in the ISP; and
(b) Coordinate linkages to needed community services, support systems and resources. In addition, when the individual's mental illness impedes his/her ability to access these services him/herself, the service provider shall:
(i) Assist the individual in accessing needed community services, support systems and resources, and
(ii) Assist the individual to develop the skills to access needed services, support systems and resources for him/herself.
(4) Individualized, restorative interventions and training to improve interpersonal, community integration, and independent living skills when the individual's mental illness impacts his/her ability to function in and adapt to home, school, work and community environments. Specific training may address:
(a) Socialization abilities, including communication, interpersonal relationships, problem solving/conflict resolution, and stress management;
(b) Support system development;
(c) Employment readiness activities, excluding skill specific vocational training. Examples of employment readiness abilities which may be impacted by a person's mental illness include work related social and communication skills, personal hygiene and dress, time management, etc.; and
(d) Other interventions and training necessary to ameliorate life stresses resulting from the individual's mental illness.
(5) Assisting the individual to acquire psychiatric symptom self-monitoring and management skills so that the individual learns to identify and minimize the negative effects of the mental illness that interfere with his/her daily functioning.
(6) Advocacy and outreach when the individual's mental illness prevents him/her from doing this for him/herself.
(7) Mental illness, recovery and wellness management education and training. The education and training may also be provided to the individual's parent or guardian, and family and/or significant others, when appropriate, and when:
(a) This education and training is based on the individual's mental illness and symptoms; and
(b) This education and training is performed exclusively on behalf of and for the well-being of the individual, and is documented in the ISP.
(8) Adhere to the agency's personnel policies and procedures, ODMH, Medicaid and Insurance standards, and fulfill documentation and reporting requirements.
(9) Inform both the school site and/or (Your Names) when ill or unable to attend.
(10) Participates in quality assurance and program evaluation studies.
(11) Attend and participate in scheduled or required training, staff meetings, peer review, workshops, and supervision.
(12) Participate in professional development trainings and workshops offered in the community and agency that is pertinent to jobs duties and responsibilities.

Essential Duties
Provide 5-Star care to our amazing residents
Bathe, shave, dress and undress, care for hair, finger nails, eyes and ears (cleaning eyeglasses, etc.), perform daily dental care, perineal care, as well as any other customary daily activities of living for residents.
Lift, turn, move, position, transfer and transport residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, vehicles, etc.; assist resident with walking with or without self-help devices.
Respond to resident calls promptly. Ensure call cords are within reach of residents. Ensure residents who are unable to call for help are checked frequently.
Assist resident with bowel and bladder functions; take to bathroom, offer bedpan/urinal, etc. Collect specimens as instructed.
Keep incontinent residents clean and dry. Check and report bowel movements and character of stools as instructed. Change clothing, linens, etc. when it becomes wet or soiled.
Change bed linens, make beds (occupied and unoccupied); keep linens tight to avoid wrinkles from forming under the resident; provide extra covers as requested.
Routinely checks on residents to ensure personal care needs are being met. Turn bedfast residents at least every two (2) hours.
Perform restorative, rehabilitative and special treatments procedures as instructed. Applies slings, ace bandages, and braces as instructed. Provide Range of Motion exercises.
Assist resident in preparing for activity and social programs, meals, appointments, medical tests, etc.
Prepare residents for meals and perform after meal care; take to bathroom, wash hands and face, transport to and from, brush teeth, clean dentures, etc.
Observe and record intake of food and fluids; report changes in eating habits.
Serve food trays, assist with feeding as indicated (cutting foods, feeding, orienting to food items and placement and temperatures). Keep water pitchers clean and filled with fresh water and within easy reach of the resident. Serve snacks.
Weigh and measure residents as instructed. Measure and record temperatures, pulse, and respirations (TPRs), as instructed. Observe and report symptoms, reactions, and changes, such as the presence of pressure areas, bedsores, and skin tears.
Participate in and receive nursing reports at shift changes. Assist with carrying out the service plan document (AL) or care plan document (HC), through the point of care application.
