Direct Support Professional

RHD's Community Choices is seeking caring and motivated staff to assist individuals with mental health and/or physical disabilities with learning job skills and/or activities of daily living in our residential support programs. The candidate will work in our Chester County locations. Also, must be motivated to work in a team environment. Competitive pay and benefits package. We are hiring for full-time, part-time, and on-call positions for our 7am-3pm, 3pm-11pm, and 11pm-7am shifts.

We are currently providing sign on bonuses of $1500 to be paid after successful completion of 90 days employment.

Responsibilities/Duties

Engage the participant according to established supervision levels and focus on targeted areas of recovery.

Assist in the process of identifying, developing and implementing the Individual Recovery Service/Behavioral Plans.

Implement the goals from the participant's recovery service plan and/or from therapist/consultant

Assist the participants with day to day activities and implement social skills.

Support participants to successfully integrate into the community, such as, teaching them transportation skills, connecting them to community activities and resources, and helping them identify gainful employment.

Assist/teach self-medication administration for participants as prescribed (after completion of approved training-Quick Mar.

Prepare, assist with preparing, or supervise menu planning and meal preparation as needed.

Required Experience

One year experience assisting individuals with mental health and/or physical disabilities is strongly recommended.Required Education

High School diploma or GEDRequired Qualifications

Valid Driver's License is required

Clear criminal background and other background checks in accordance with state requirements

Ability to pass and complete all required trainings

Ability to convey, understand, and follow spoken and written instructions in English

CROSSROAD CHILD & FAMILY SERVICES:

Crossroad is a nonprofit organization employing more than 100 full-time and part-time individuals in a variety of positions. Tasks include:
  • Supervising youth.
  • Role modeling for youth.
  • Teaching life skills to youth.
  • Assisting with education and recreation programs for youth.

Positions needed (Full-time):
  • 2nd and 3rd shifts available, with potential to move to 1st shift.
  • 2nd shift (2:30 pm - 11:00 pm during weekdays and weekends).

  • Staff work 4 days on/ 2 days off.

The Direct Support Personnel assists with growth and development ofinterpersonal skills and Activities of Daily Living in the home or community toindividuals with MRDD or mental health challenges who live independently (withassistance) in their own residences (supportive living) or a communityresidential property.
Essential Duties and Responsibilities:

+ Provides training/assistance to assigned individuals with Interpersonal skills and Activities of Daily Living in accordance with theoutcomes and goals in the individual's support plan and under the directsupervision of the Team Leader, Program Coordinator, or Program Director

+ Addresses all outcomes and goals as outlined on theindividual's ISP and BSP are performed

+ No changes in care are initiated without direction from the Team Leader

+ Any changes in status are reported to Team Leader, ProgramCoordinator, or Program Director

+ Documents care per company policies and procedures,evidenced by notes are completed, accurate and turned in on a timely basis, andin accordance with documentation policy guidelines

+ When unable to complete an assignment, for any reason,office leadership is informed immediately. Changes in assignment due toconsumer or family request must be verified. Maxim Healthcare Services must beaware of changes in assignment and reason for change. Confirms schedule weeklywith Team Leader/office leadership

+ Participates in Multi-disciplinary Team conferences and In-services as requested, verified by sign-in sheets
