POSITION SUMMARY:
Targeted Case Management includes assessing, linking, coordinating, and monitoring services from mental health, physical health, social, educational, entitlement, and vocational rehabilitation to help children, families and adults live work, and participate fully in their community. It includes a collaborative coordination and development of a culturally specific individualized services plan in partnership with the individual, which reflects strengths and self - identified goals. As an integral part of case management services, discharge/transition services are arranged or coordinated by the case manager as an individual is preparing for discharge and transition from one level of care to another. These services are designed to support the attainment of individual-defined goals as identified in the individuated Treatment Plan (e.g. stable living arrangements, quality relationships, employment, vocational training or school attendance.) All services will be provided within a Recovery-oriented system of care, which builds on the individual's strengths and incorporates community, natural systems, and clinical supports and services in the management of the individual's needs. Attend Integrated Decision Team Meetings. Transport clients in personal vehicle as part of service provision. The FIT BHCM will be assigned to be on call for coverage after hours and on weekends. Staff are required to complete all corporate and industry related training for child welfare services, including the principles and techniques of Signs of Safety. Responsible to meet productivity requirements.
Qualifications
EDUCATION EXPERIENCE/REQUIREMENTS:
Bachelor's degree in Human Services from an accredited college with a minimum of one year of previous experience working with adults and children with serious persistent mental illness.
Completion of Targeted Case Management Certification required.
Must have excellent writing, communication, computer, organizational and time management skills with the ability to work independently and as part of a team.
Proficiency in completing progress notes, treatment plans, and pertinent information concerning the consumer required.

Position Summary

Position Summary:
This is a professional clinical position providing clinical assessments exclusively to individuals referred to Mental Health Court to determine acceptance into same. Additionally, the Counselor will provide short term counseling as needed to individuals assigned to Mental Health Court until which time the individual has been linked to needed service providers, including community mental health, specialty programs for mental health or substance use disorders, private practice providers or a medical home that is designed to address all health including physical and behavioral health needs. The Counselor for Mental Health Court is part of a teaming model to include an Intensive Case Manager. The Intensive Case Manager and the Counselor will work in partnership with the individual assigned to Mental Health Court to identify and address all of the individual's needs, in particular the individuals unmet needs, to ensure the individuals success with Mental Health Court and reducing recidivism with the criminal justice system. The Counselor is responsible for providing 1-2 brief treatment sessions per week per individual for a period not to exceed 12 weeks, while the ICM is working to connect the individual, as needed, to longer term community mental health or substance use services. The Counselor is responsible for remaining up to date on all action of the court. The Counselor will meet with individuals in the community to include shelters for the homeless, residential programs, jail or in the individual's home. The Counselor will work in partnership with the entirety of the Mental Health Court team to ensure the individuals overall success, and to remove any identified barriers. The Counselor is responsible for keeping the Court up to date on all progress or regress related to treatment, by providing verbal and written progress notes to the ICM that will become a part of the formal court proceedings. The Counselor will serve a minimum of 40 unique individuals per year.

The Counselor - FIT functions as a member of an Integrated Decision Team (IDT) consisting of the Counselor, a Behavioral Health Case Manager, a Peer Mentor, and an IDT facilitator. In additional to clinical interventions, the Counselor - FIT is responsible for ensuring the parent has an individualized treatment plan that is strength based and built collaboratively with each member of the IDT team during the IDT session. The treatment plan, risk level, and clinical interventions are developed by the Integrated Decision Team that consists of the family, natural supports, and formal supports that care about and know the parent best. Each IDT member, including the parent, contributes valuable knowledge, expertise and perspective that informs the treatment plan and clinical response. In addition, the Counselor - FIT is responsible for reporting and discussing at IDT's the parent's progress/lack of progress, barriers, interventions, safety issues, or any other information important in resolving the substance use and safety concerns that bring the parent into treatment. The Counselor - FIT ensures services are delivered using standards of trauma informed care principles, and that plans and clinical interventions address issues regarding how trauma impacts family functioning, and that plans and interventions use trauma resolution skills. The Counselor - FIT ensures that services are Culturally & Linguistically Competent and are tailored to the unique culture of the parent. The Counselor - FIT will ensure that clinical services occur in the community at a place convenient and comfortable for the parent. This is a field position. The Counselor - FIT is responsible for meeting an annual client facing client care number of 1,316 hours annually. Achieving the minimum required weekly, monthly and annual client facing client care hours is an essential function of this position.

Qualifications
EDUCATION EXPERIENCE/REQUIREMENTS:
Master's degree in counseling, Psychology, Social Work or related field required.
Minimum of 2 years previous experience working with at risk population (children and families).
CAP certification preferred.
Must have sufficient clinical knowledge to provide clinical intervention with adults and children who manifest a range of psychopathology, utilizing various treatment approaches including individual, family, play, and group therapy, individual psychopathology and normal childhood development.
Have sufficient clinical knowledge of Cognitive Behavioral Therapy, Trauma Focused- CBT and other evidence-based treatments.
Must acquire knowledge of community resources.
Must take the
Wraparound for Clinicians Training
(3-hours) within 30 days of employment. Preference will be given to individuals who have taken the 3-hour Wraparound for Clinicians training.
