• BSW or BA in related field required. LSW in social work and five years experience or LMSW and two years experience preferred.
• Experience in Long Term Care or Home Health settings preferred.
• Experience in social service delivery to senior client populations and program management preferred.
• Preferred 2-3 years experience with senior adults in the long-term care setting providing assessment, casework, and care planning for residents and families.
• Ascribes to Christian values and principles.
• Works with little supervision, is self-motivated, flexible, organized, and exercises ethical and professional judgment in service delivery and maintenance of confidentiality.
• Ability to work cooperatively within the interdisciplinary team.
• Ability to develop and maintain positive rapport with residents and families.
• Proficient in the use of computer, printer, keyboard and software for Word, Excel, e-mail, and database applications.
• Be organized in planning work load of their own duties as well as those of the Volunteers/Interns.
• Ability to use up to 50 pounds of force.
• Ability to walk frequently and occasionally stand and/or sit for prolonged periods of time.
• Ability to communicate effectively orally and in writing.
• Ability to problem-solve, prioritize multiple tasks and organize work for short term and long term planning.
• Ability to work under stress and remain flexible in changing situations.
• Ability to travel occasionally and get to appointments in a timely fashion.

Persimmon Ridge Health and Rehabilitation, a nursing facility located in Portland, IN is looking for a compassionate and passionate Social Services Director. The Social Service Director is responsible for identifying the medically related psychosocial needs of residents and providing services to meet those needs. The Social Service Director will address residents' intellectual, developmental, emotional, spiritual and social functioning so that each resident can attain or maintain the highest level of psycho-social functionb'.tg and well-being possible. Families will also be offered services in that their positive and continued involvement may be critical to the resident's quality of life.

Persimmon Ridge Health and Rehabilitation is seeking a Full-Time Social Service Director to:

+ Assist as directed in pre-admission process which may include involvement in pre-screening, tours, and coordination of information and referrals from community members and agencies.

+ Provide social services in accordance with accepted facility, professional, and governmental regulatory standards.

+ Act as a resident advocate and liaison with facility staff and with community resources as necessary.

+ Maintain social service records including initial psycho-social assessments, ongoing pertinent progress notes, quarterly notes, and discharge plans, documenting in accordance with accepted facility, professional, and governmental regulatory standards.
Qualifications and Skills

+ A Social Service Designee certificate or a Bachelor's or advanced degree, or both, in social work, or a degree in human services field.

+ Indiana board certification in social work under IC 25-23.6-5-1 with at least one (1) year's experience in a health care setting working directly with individuals.
