Americorps disability program navigator job description

NCRA offers a variety of support services for formerly incarcerated individuals with a particular focus on reducing barriers for post-secondary access and persistence in order to reduce recidivism in Washtenaw County, Michigan. AmeriCorps' Navigation Coaches will provide direct technical coaching and assistance and work to make the post-secondary campus climate more accessible and less intimidating. Coaches will receive training to help identify mental health and trauma resources for participants. In addition, coaches will serve as an advocate for returning citizens in post-secondary systems and programs to ensure access and program persistence. AmeriCorps Navigation Coaches meet with each student from their caseload for 45 minutes weekly. Evidence-based practices such as including one-to-one interventions, data-centered advising, and culturally relevant resources and interventions. Initial intake forms, quarterly reviews, on-campus and off-campus resource navigation, academic readiness support, and technology resources will be provided by the AmeriCorps Navigation Coaches in order to set up the student for success. Each Navigation Coach will be situated at either an educational institution or local non-profit that services formerly incarcerated individuals. Coaches will receive training in both areas of access and persistence. This includes but is not limited to: 1. Academic Support: Items such as applying to post-secondary opportunities; requesting transcripts; submitting a FAFSA, technology assistance, and communications with faculty. 2. Under the Direct Supervision of the Prevention

Supervisor, The Syringe Services Program (SSP) Patient Navigator will play a

critical role in providing linkage to care for clients who utilize our Syringe

Services Program. The primary role of the SSP patient navigator will be to

successfully link SSP clients to community-based programs that address the

medical and psychosocial needs of people who inject drugs (PWID), especially to

treatment for substance use disorder and care and treatment for infections

related to injection drug use (IDU), including viral hepatitis, HIV, and

bacterial infections including endocarditis. The SSP Patient Navigator will

assist SSP clients who may need a series of linkages in order to successfully

access their desired program. Potential assistance may include help applying

for identification, Medicaid/ health insurance or unemployment. They will link

SSP clients to programs that are non-judgmental, able to service active PWID,

and otherwise culturally competent to meet the needs of this community. They

will ensure newly diagnosed HIV+ and/or Hep C+ clients are referred and linked

to medical services. They will provide HIV 101 and Hep C 101 to clients and

various organizations within the community. They will participate in daily

Syringe Services needs including providing new syringes to clients, disposing

of used syringes, educating clients on overdose prevention and prevention

education. The ideal candidate will have in-depth knowledge of the health care

system in Los Angeles.

Qualifications

Required:

+ Knowledge of HIV/AIDS, including Antibody testing, community resources, Hepatitis C and STD knowledge, harm reduction, syringe exchange

+ Skill in database management and data entry

+ Ability to complete required program documentation

+ Public speaking and leadership ability

+ Skill in providing individualized HIV health education, HIV testing and Counseling, Hepatitis C education, Hepatitis C testing and counseling, overdose prevention, medical and substance abuse referral, and abscess education

+ Ability to follow through on assignments

+ Ability to work independently and as part of a group

+ Ability to discuss sensitive matters in a non-judgmental manner and to respect confidentiality

Job Description:

The Children and Family Program Navigator is responsible for coordinating the care of children and families
that are residing in Illumination Foundation’s family emergency shelters.

Tasks/Responsibilities:

Administrative/ Record Keeping

  • Maintain updated accurate case notes, records, relevant paperwork on KIPU HMIS.
  • Input and enter all data from screenings on relevant google docs, KIPU, and HMIS.
  • Track program services for the purposes of data collection proof of desired outcomes
  • Maintain a daily log and update case files/notes as necessary
  • Ensure shelter is well managed, clean, and safe for families and guest.
  • Maintain proper inventory of shelter supplies
  • Cleaning and preparing rooms for new families entering shelter along with other client spaces such as
  • refrigerator and pantry space
  • Conduct room checks on a regular basis and on a as needed basis
  • Collaborate with other departments at Illumination Foundation as necessary and appropriate, including
  • volunteer and communications department.
  • Complete referral paperwork as deemed appropriate for children and families.
  • Collaborate with outside partners and agencies when necessary

Direct Services

  • Complete intakes for families at family emergency shelters
  • Assist with leading or providing support in group classes/activities for children and parents

  • Consult with parents regarding parenting, child development and other familial concerns.
  • When attending appointments with families advocate for clients relating to housing, health, education, and social and emotional wellbeing.

  • Provide referrals and track linkages to relevant services for children on caseload.
  • Attend case conferences and supervision as needed.
  • Collaborate with Children’s Department to provide children’s services to families staying in IF emergency and bridge housing facilities.
  • Continually update resources for activities and lesson plans
  • Monitor for signs of emotional or developmental troubles in children and communicate to supervisor
  • Monitor for signs of emotional, physical, and sexual abuse and communicate to CPS as deemed necessary by law
  • Prioritize individual and client health and safety.
  • Be able to meet targets and work under pressure.
  • Communicate any concerns and feedback to Supervisor with tact and professionalism.
  • Set and maintain healthy boundaries with clients.
