THE ORGANIZATION PROVIDES SUPPORTED COMMUNITY LIVING SERVICES TO ADULTS WHO HAVE AN INTELLECTUAL DISABILITY. SUPPORT SERVICES ARE BUILT AROUND THE CONSUMERS' WANTS AND NEEDS AND VARY FROM 24 HOUR SUPPORT TO A FEW HOURS A MONTH. THIS YEAR THE ORGANIZATION PROVIDED SERVICES TO 152 INDIVIDUALS; 24 HOUR CARE WAS PROVIDED TO 140 PEOPLE AND HOURLY SUPPORT SERVICES WERE PROVIDED TO AN ADDITIONAL 12 PEOPLE. RESIDENTS WERE PRIMARILY FROM LINN COUNTY, IOWA. THE AGE RANGE OF THE PEOPLE SERVED IS FROM 20 TO 83 AND THE PRIMARY SOURCE OF SERVICE FUNDING IS VIA THE MEDICAID HOME AND COMMUNITY BASED SERVICES PROGRAM. SERVICES OFTEN INCLUDE ASSISTANCE AND TRAINING WITH COMMUNITY INTEGRATION, HYGIENE, MONEY MANAGEMENT, SOCIAL INTERACTIONS, DOMESTIC DUTIES, HEALTH AND SAFETY, AND VOCATIONAL SKILL ACQUISITION. SERVICE SUPPORTS ARE INDIVIDUALIZED AND INTENDED TO ALLOW THE CONSUMER SERVED TO BE AN ACTIVE MEMBER OF THEIR COMMUNITY.
