Responsibilities
Assist in pre-admission and admission process with customers, as needed, as back-up to Admissions Department personnel. Duties to include providing tours, signing of paperwork, etc. Interview clients and families to determine financial status and need for Medicaid assistance. Assist families in applying for clients' Medicaid (ICP) approval. Attend annual review for Medicaid eligibility if families unable to attend. Assist facility bookkeeper in tracking and reporting status of Medicaid Applications/Approvals. Assist Social Services Director, as needed, with interviewing residents, families to obtain relevant past and present social histories, including past behaviors, adjustments, and lifestyles. Documents progress and updates plans of care at interdisciplinary conference, as needed. Assist residents and families with adjustment to admission to facility by providing one-on-one counseling and/or group discussions. Help Social Services Directory with planning of post discharge services and follow-up care. Maintain effective communication with residents, families, health care professionals and facility staff. Assist residents and families in any situation, as needed. This person may have access to protected health information in the patient's financial and medical records to conduct job duties.
Qualifications
Minimum of High School diploma or equivalent. Bachelor's Degree in Social Work or Human Services is preferred. One year of supervised social work experience in a health care setting, working directly with individuals. Experience or knowledge of Institutional Medicaid Program (ICP) qualifications and application process preferred. Ability to work with a variety of people in situations varying from community and health care settings. Able to make decisions concerning procedures based on evaluations and interpretations of clients' case histories

Responsibilities
Completes comprehensive activity assessment and develops indicated activity plan with approved time frame. Child, Adolescent, Adult and Geriatric.
Provides a variety of age specific therapeutic activities and recreational interventions. Child, Adolescent, Adult and Geriatric.
Conducts special events as assigned. Child, Adolescent, Adult and Geriatric.
Escorts patients to and from the unit safely.
Attends and participates in Treatment Team meeting, maintaining current documentation of patient's progress toward identified goals.
Motivates patients to participate in scheduled activities within their identified physical and mental capabilities. Child, Adolescent, Adult and Geriatric.
Clear and concise documentation relating to patient observations and interactions Child, Adolescent, Adult and Geriatric.
Adheres to policies and procedures and demonstrates practices that ensure a safe environment for patients, visitors, co-workers and self.
Accurately follows directions of Educational Services Coordinator and licensed personnel.
CPR certification.
MOAB/CPI certification.

Summary:
The Activities Therapist plans, organizes, develops, and directs quality activities for patients, ensuring that the recreational, physical, intellectual, spiritual, and social needs of each patient are met in accordance with all applicable laws, regulations, and Life Care standards. Activities Therapists work as members of a multidisciplinary treatment team. May organize and conduct music, art, recreation, or other therapy programs, depending on specialty.

Must perform proficiently in all competency areas including but not limited to: general activities duties,, supervisory responsibilities, care plan and documentation responsibilities, patient rights, and safety and sanitation principles and techniques of therapy as applies in an acute care psychiatric facility
2. Must demonstrate understanding of basic medical and psychiatric terminology as well as basic regulations governing the treatment of people in psychiatric health facilities.
3. Communicate effectively, both orally and in writing while appropriately and descriptively charting patient progress and behavior. Maintain detailed records of activity programs and participation of individual residents, identifying progress toward established care plan goals.
4. Adapt treatment programs by graduating tasks to meet patient's level of function
5. Develop, organize, and implement a program of adjunctive therapy activities to meet the social, emotional, physical and other therapeutic needs of patients as identified on the patients' plan of care and within the specified budget.
6. Must be able to make regular in-room visits to patients uninterested or unable to participate in group activities
7. Must exhibit excellent customer service and a positive attitude towards patients. Coordinate and verify that assistance is provided to residents for activities and events as necessary. Create an ongoing calendar of activities, which creatively meets multiple needs and is accessible and appealing to both men and women of all ages and abilities.
