Responsibilities
* Develop key relationships within assigned Key Accounts and manage external aspects of patient referrals and admissions
* Communicate with customers to determine services and care that best meet their needs, for the right care in the right place at the right time
* Efficiently and effectively collaborate with referral sources and Center teams to assess patient needs for compatibility with Center clinical capabilities
* Thoroughly educate customers and understand customer expectations to provide a positive experience, meet or exceed expectations, and to help reduce unnecessary rehospitalizations
* Help facilitate smooth patient and resident transitions
* Develop and manage Account assignments in key market segments as determined with sales manager (eg physician groups, hospital(s), Assisted Living, Independent Living, Home Health, and community contacts)
* Collaborate with referral sources and team members to facilitate open communication
* Develop and maintain strong relationships with Decision Makers, Decision Influencers, and other Contacts as possible and appropriate
OTHER JOB DEMANDS:
Ability to work with individuals at all levels both within the organization and with outside contacts. Adequate computer skills for required data entry, reports and communication. Have or develop an understanding of financial options for Skilled Nursing coverage in order to explain options to potential patients/ residents and family members. Comply with HIPPA as this position may have access to protected health information in the resident's clinical and financial records to conduct job duties.

Saint Luke's Home Care & Hospice is seeking a Clinical Liaison who will work primarily on consults and referrals from SLHS hospitals facilitating patient transfer to SLHS Homecare and Hospice. The liaison will respond quickly to in hospital Hospice consults by meeting with patients and their families to educate on hospice services and facilitate a smooth and seamless transition to Saint Luke's Hospice House, Inpatient hospice or patient's home environment. The liaison will collaborate with Hospital and Hospice providers to determine Hospice eligibility and appropriate patient level of care upon discharge. The liaison may also consult on complex Saint Luke's Home Health referrals and Home Infusion teaches as needed.
Shift: Full-time, Monday - Friday, 8 a.m. to 5 p.m. with occasional flexibility for after-hour coverage.
The Work:
Visits patients (and their families) to discuss goals of care upon receipt of consult request.
Collaborates with Hospice provides on Hospice eligibility and appropriate level of care for patient.
Coordinates with Hospital providers and staff to provide a seamless transition for patients to SLHS Home Health, Hospice, and Infusion services.
Fosters partnerships and relationships, through day-to-day interaction with SLHS hospital staff, providers, and Care Progression team.
Participates in capacity management discussions and meetings as requested.
Provides support to intake and referral processes by documenting and creating episodes in EPIC.
Provide statistics and referral information to the Supervisor Clinical Referral Services upon request, always looking to improve the liaison role to meet the needs of the agency.

Types of clients: Mobile Crisis Response (MCR) works with clients in active crisis who have outreached the 988 mobile crisis hotline number and are in need of in-person assessment and support. We work with clients of all ages in the BJC BH catchment area (St. Louis County, parts of STL City, Iron County, Washington County, and St. Francois County)
Case load: There is ZERO caseload with this position - you will simply respond to 988 crisis hotline calls as needed
Team Dynamic: Mobile Crisis Response is a 24/7/365 Program and you work with another Team member when you respond to these calls
You will use a Company Vehicle to respond to crisis calls -
you will not use your own vehicle
Location: Team members have touchdown space/office located at 8300 Eager Road in Brentwood - but work with clients anywhere in the community. MCR teams go to where the individual-in-crisis is (home, park, store, bus station, street intersection, etc)

Responsibilities:
1. Serve as territory business owner with a focus upon impact and territory analysis. They interact directly with key management personnel, providers and staff members to assure patient access to care. This individual appropriately educates HCPs and their office staff on clinical documentation, securing treatment approvals, patient tracking, claim submission, reconciliation management, patient education, drug inventory, patient copay assistance and drug acquisition channels. This position helps HCPs understand barriers that may impede or delay appropriate patient access to treatment and/or prevent optimal patient outcomes. Additionally, the Reimbursement Liaison provides education on appropriate and accurate billing and coding practices for Glaukos' products in line with payer requirements to the highest levels of specificity.

Part-Time Community Outreach/Enrollment (O/E) Staff

that is projected to end May 2026. Our Community Outreach/Enrollment (O/E) Staff are integral members of the Rural Health Group team who will provide outreach and enrollment assistance to uninsured individuals living in our six-county service area.
In-reach will be conducted by contacting all uninsured patients seen in the last 24 months, coordinating an eligibility screening with the patient and enrolling those who qualify. When new uninsured patients present to any of the 12 clinics in our system, front desk staff will contact O/E staff to conduct eligibility screening.
Community-wide outreach and enrollment will occur in areas where low income populations work, live and shop. Specifically, working collaboratively with local schools Rural Health Group O/E staff will conduct screenings during PTA meetings, school events and onsite at our school-based health centers. Working with employers in retail and food service, Rural Health Group O/E staff will provide "Lunch and Learn" presentations at area businesses to raise awareness as well as screen/enroll people for the health insurance marketplace. In the local emergency department and during discharge planning, Rural Health Group will have O/E staff onsite to screen/enroll uninsured.
This is done collaboratively with NC Community Health Center Association to coordinate services with other FQHCs, free clinics, and hospitals working on screening and enrollment. Monthly coordination meetings have been established with the four FQHCs located contiguously with Rural Health Group's service area to leverage resources and reduce duplication of efforts. In addition, working closely with hospital case managers and discharge planners, Rural Health Group O/E staff will identify uninsured patients who would benefit from screening/enrollment visit.

MainStreet Family Care is expanding our community footprint, and we're launching a brand-new role designed for someone who genuinely thrives on movement, variety, and fresh experiences. If you're the type of person who panics when you don't have your next trip locked in - and you draw energy from meeting new people and being out in the community - this is the opportunity you've been waiting for!
This is a high-ownership role with a broad creative runway. You'll be based at our Birmingham, AL headquarters alongside our Marketing Team, but you'll spend 30-50% of your time on the road representing MainStreet at schools, pediatric offices, fairs, festivals, and community events. With limited day-to-day oversight, you'll shape your own outreach strategy, build partnerships, and create compelling content that showcases MainStreet's impact across the region.
Schedule: Monday-Friday, 8:00 AM - 5:00 PM (flexible 40-hour schedule once settled)
Travel: 30-50%
Location: Birmingham, AL HQ with frequent regional travel
Essential Functions:
• Conduct high-volume outreach to schools, pediatric offices, civic groups, and community organizations to build strategic partnerships
• Independently plan and execute outreach activities and events, bringing creativity, strong logistics, and an eye for engagement
• Occasionally representing MainStreet or KidsStreet on television and/or radio segments as needed.
• Serve as MainStreet's brand ambassador at back-to-school bashes, health fairs, festivals, and other high-visibility community events
• Drive daily cold-calling efforts and maintain consistent follow-up with prospective partners
• Produce community-facing content (photos, videos, short-form posts) that highlights events, partnerships, and MainStreet's presence in each market
• Maintain structured outreach pipelines, event calendars, and tracking sheets using Excel
• Identify opportunities for expanded brand visibility and propose new outreach concepts
• Build and sustain relationships with key community stakeholders
• Coordinate with Marketing on collateral needs, content direction, and promotional strategies
• Research and join social media groups for moms in our KidsStreet markets.
• Support additional marketing and administrative efforts as needed
Qualifications:
• Highly independent; thrives when given broad direction and significant autonomy
• Strong comfort with high-volume cold calling and phone-based engagement
• Confident communicator with a friendly, polished, and professional presence
• Demonstrated ability to create engaging, community-focused content for social media
• High attention to detail; strong organization and follow-through
• Experience in outreach, events, marketing, or partnership-building preferred
• Proficiency in Excel and comfort maintaining structured tracking documents
• Willingness to travel 30-50%, including occasional evenings/weekends for events
