Position Summary: This clinicalpharmacist position in the Clinical and Patient Care Services Division provides clinical pharmacy services, including order verification, clinical monitoring, and support of clinical pharmacy and departmental programs. Responsibilities include:

Position Summary: The Duke University Hospital (DUH) Outpatient Parenteral Antimicrobial Therapy (OPAT) program improves patient safety by facilitating earlier discharge of patients on intravenous (IV) anti-infectives, providing close laboratory monitoring and documentation, and improving communication at transitions of care. The main role of this position is to support operations of the OPAT clinic, joining one clinicalpharmacist practicing in this role. The position will also support transitions of care workflow from inpatient to the outpatient setting for OPAT patients with a focus on reducing readmissions. The position will have dedicated time to support the Antimicrobial Stewardship and Evaluation Team (ASET) inpatient stewardship program on a weekly basis. ASET is dedicated to enhancing the quality of antimicrobial use throughout Duke University Hospital (DUH). This position will provide support alongside two ambulatory infectious diseases/HIV pharmacists, three antimicrobial stewardship pharmacists, and working collaboratively with the Department of Pharmacy, medical staff, and other healthcare workers under the direction of the Infectious Diseases Pharmacy Services Coordinator to support both inpatient to outpatient infectious diseases pharmacy programs.
Duties and Responsibilities:
Clinical Practice and Medication Management: Infectious Diseases Outpatient Clinic
* Provides clinical pharmacy support for the Outpatient Parenteral Antimicrobial Therapy (OPAT) Program.
* Participates in clinic and provides clinical pharmacy services and oversight for safe use of medication for patients receiving home antimicrobial infusion therapies, such as:
* Completes medication reconciliation activities
* Provides medication counseling/education
* Interprets pharmacokinetic/therapeutic drug monitoring data and makes dose change recommendations as deemed appropriate
* Screens for drug interactions, therapeutic duplications, omissions, etc.
* Monitors patients for therapeutic and/or toxic response to medication

The Pharmacist prepares and dispenses medications while monitoring for cost-effective drug therapy, potential adverse reactions, and incompatibilities. Consults with Medical Staff on drug therapy and educates patients on medication use when requested.
Responsibilities:
- Accurately fills IV and unit dose physician orders and out-patient prescriptions compliant with legal and departmental policies and procedures, working in an efficient and productive manner and reducing non-essential interruptions.
- Demonstrates the ability to assess a situation, consider alternatives, and choose a course of action in filling orders, providing drug information, consultation or general problem solving. Uses this ability to improve outcomes through physician communication, provision of drug information, etc., with appropriate documentation to demonstrate a positive impact on patient care.
- Contributes to the advancement of Pharmacy's image by actively participating in clinical activities such as providing drug information (newsletters, memos, dosing charts, guidelines, in-service, drug class review, etc.) for pharmacy, nursing, or medical staff or participation in developing programs and/or procedures (such as order sets, protocols, guidelines, procedures, etc.) that improve patient care or provide education (hospital or community) as evident through documentation. Distinguished contributors participate in 4 or more activities.
- Initiates or manages projects, in addition to daily staffing activities, that are short term or ongoing which contribute to departmental operations and performs other duties as assigned by management. Distinguished contributors participate in 2 or more activities.
- Communicates concisely and clearly with appropriate personnel on issues, problems, changes, or observations and what actions have been taken either verbally or in writing to prevent adverse events in patient care (i.e. email/discussion with management, submitting reports through the Patient Safety Reporting System). Provides supervision to technicians; communicates clearly and in a positive manner to ensure immediate and/or timely accomplishment of assignments. Distinguished contributors provide solutions to identified issues and actively provide direction to technicians when individuals are not meeting expectations.

Qualifications
Required
- Bachelor's degree in Pharmacy.
- Licensed as a Pharmacist in the state of North Carolina
Preferred
- Experience as a Pharmacist in a hospital setting
