Job Summary
The Emergency Room Advanced Practice Provider (APP) is a member of the ER team and his/her primary responsibility is the management of acutely ill and/or injured patients in the emergency room setting. The ER APP is under the direct supervision of the ER supervising physician or designated back-up physicians. The APP performs physical exams, collects and documents data, conducts approved diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications, and provides direct patient care services in the Emergency Department. The APP contributes to excellence in patient care, research, teaching, consulting, and provides leadership to the organization and to the APP profession.
Job Duties and Responsibilities include:
* Provide direct clinical services for acutely ill and injured patients according to established protocols
* Utilizes evidence-based clinical practice standards under the supervision of the ER supervising physician
* Obtain patient health histories and complete physical examinations.
* Document findings in the EHR, noting pertinent normal and abnormal findings.
* Order and interpret appropriate diagnostic and laboratory tests through the EHR system.
* Explain the necessity, preparation, nature and anticipated effects of procedure(s) to patients, patient's family, staff, and healthcare learners.
* Perform approved therapeutic or diagnostic procedures based upon patient's clinical status to include wound suturing and orthopedic splinting.
* Document patient preparation and response to procedure(s).
* Utilize prescriptive privileges as allowed by NC licensure. Document all prescriptions in patient's medical record.
* Assess patient for change in status and institute appropriate interventions.
* Initiate necessary emergency interventions to stabilize patient.
* Provide family-centered care by communicating with the family regarding the patient's condition.
* Assess family adaptation, coping skills and the need for crisis or other intervention.
* Utilize current research and evidence- based decision-making in all clinical practice.
* Incorporate clinical research findings in the development and implementation of standards of care.
* Support IRB approved clinical research/trials though a variety of activities including but not limited to: obtaining consent; ordering diagnostic tests; recording laboratory & diagnostic results; administering therapeutic interventions; and documenting and reporting patient outcomes including toxicities or adverse events.
* Perform and participate in quality/performance improvement activities and clinical research.
* Serve as a clinical expert and leadership consultant to members of the healthcare team and to the larger community.
* Participate in and support accreditation, compliance, and regulatory activities of the organization. Identify educational needs of the family and participate in teaching opportunities.
* Assess educational needs, plan, provide, and evaluate educational programs for nursing staff, interdisciplinary healthcare team, and community. Precept NP/PA students and other new healthcare learners.
* Participate in the development, review, and evaluation of emergency care medical management practice protocols within legal guidelines incompliance with DUHS policies.
* Role model professionalism through conduct, appearance, communication, mutual respect, leadership, ethical decision-making, critical thinking, and problem-solving skills.
* Demonstrate responsibility for professional practice through active participation in professional organizations and continuing education. Enhance the body of knowledge to the APP profession or other areas of specialization through exchange of ideas and knowledge in professional organizations, oral presentations, posters, research activities, and written publications.

Education
PA: Graduate of accredited Master's Degree or Doctoral Degree PhysicianAssistant program

* Update therapy plans per provider request and send via in basket for signature
* Assist providers with order entry and review. Reviews in advance all orders for South Durham, Brier Creek, and Cary Clinics to confirm dosing and scheduling accuracy along with accuracy of referrals. Referral review and diagnosis accuracy to ensure no more than one diagnosis is associated with the therapy plan.
* Update same day pre-medications, especially in anticipation of patients who have had a prior hypersensitivity reaction.
* Present during all hypersensitivity reactions, assist with patient recovery, order entry, and how to disposition the patient following (i.e., may be discharged to home setting or need to be admitted to the ED).
* Resource for same day questions, i.e., medication orders, therapy plans and necessity for update and review
* Place same day notes of patients who necessitate "ok to treat" order and detail the clinical situation in form of short version of SOAP note.
* Update medications, add medications that have "fallen off therapy plans"
* Assist in transfers of care to ED. Communicate directly with referring provider in instances that warrant immediate attention
* Provides education/in-services on therapy plan entry to service lines. Work with Epic as a super user for therapy plan design
* ACLS certification required (candidate may obtain this certification during onboarding).

It is anticipated that the APP will see patients at Duke Gastroenterology North Durham and main campus 2J clinics and may also see patients at other Durham locations depending on Division needs and clinic availability.
Job Duties & Responsibilities
* Record the history of present illness, past medical, family and social history, perform physical examinations in inpatient and outpatient setting
* Establish and development treatment plan; care will be provided in conjunction with MD's as needed
* Prescribe appropriate medications to a specialty pharmacy that will handle prior authorization and other necessary paperwork; order refills as needed
* Patient and family teaching
* Order appropriate labs, tests and procedures and follow up with patient as needed
* Assist in covering the GI Day Hospital and urgent slots and perform procedures such as fibroscans and paracentesis

Job Summary
We are seeking an Advanced Practice Provider in our Duke Gastroenterology North Durham Clinic located in Durham, NC to provide medical care to adult patients with gastrointestinal conditions in a general GI inpatient and outpatient clinic. Primary hours will be Monday through Friday, day shift, with hours flexible to meet the needs of the patients.
Duties may include but are not limited to those listed below and may include other outpatient procedures or services as deemed appropriate by the supervising physician. It is anticipated that the APP will see patients at Duke Gastroenterology North Durham and may also see patients at other Durham locations depending on Division needs and clinic availability. Coverage of inpatient GI consult service will occur one week every 4-6 weeks or more often as needed.
Job Duties & Responsibilities
* Record the history of present illness, past medical, family and social history, perform physical examinations in inpatient and outpatient setting
* Establish and development treatment plan; care will be provided in conjunction with MD's as needed
* Prescribe appropriate medications to a specialty pharmacy that will handle prior authorization and other necessary paperwork; order refills as needed
* Patient and family teaching
* Order appropriate labs, tests and procedures and follow up with patient as needed
* Perform hemorrhoid banding
* Coordinate care and refer to other medical professionals as needed
* Document clinic, telephone, and other patient encounters in the Epic (Maestro) system
* Review lab results in Maestro, communicate and interpret results
* Collaborate with HUB Triage Nurses regarding patient triage and return patients calls within 24-48 hours; provide feedback regarding scheduling of urgent patient appointments
* Review outside records including clinic notes, hospital records, outside radiographs, lab, and other test results
* Collaborate with nursing staff to complete patient paperwork including medication pre-certifications for insurance companies, durable medical equipment requests, disability paperwork, and handicapped permit applications
* Attendance at GI Grand Rounds is expected. Participation in other divisional educational events is also encouraged

We are seeking an Advanced Practice Provider (APP) for our Department of Neurosurgery - Spine Team of Duke Raleigh. The APP will provide direct patient care services in collaboration with the supervising physicians to triage and evaluate new spine patients. This position will involve outpatient services and minimal inpatient services. There is opportunity for continuity of care as well.
Primary hours will be Monday through Friday 7:30am- 4:30pm PM with hours flexible to meet the needs of the patients. Weekends and holidays are not required unless clinician elects to meet the needs of their patients.
Job Duties & Responsibilities
* Take patient histories, conduct physical examinations, review imaging, generate a plan, and document findings in the patient's record; write progress notes; prepare patient workups and summaries noting pertinent positive and negative findings; obtain psychosocial history relevant to patient care, and assume responsibility for developing therapeutic relationships with patients. Make recommendations on which Neurosurgeon to refer patient to for additional care and possible surgery. Support from supervising physician throughout day.
* Respond to patient inquiries and concerns via telephone or MyChart. Accessible during work hours and consult with supervising physician as needed.
* Order and schedule laboratory studies and diagnostic procedures; explain necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient. Arrange inpatient admission for patients if needed.
* Carry out therapeutic procedures such as removing sutures and staples and performing dressing changes; monitor progress of patients, report progress to supervising physician, and maintain record of each patient's progress, consulting with supervising physician when patient's progress does not meet anticipated and/or predetermined criteria.
* Provide coverage for fellow Advanced Practice Providers (APPs) during their absences, including but not limited to the Family and Medical Leave Act (FMLA) leave, ensuring continuity of care and maintaining high standards of patient service.
* If interested, participate in basic science and clinical research projects conducted by supervising physician; prepare statistical reports and analyses setting forth progress, adverse trends and appropriate recommendations or conclusions. Review the work of lower rated employees; train new personnel. Participate in teaching students and residents in techniques and procedures which are unique to specific clinical setting.
