1. Clinical Competence
Initiates, develops, implements, and makes necessary revisions to the plan of care in collaboration with the physician and other health care professionals involved in care.
Directly and/or indirectly supervises care provided by the home health aides and licensed practical vocational nurses, provides instruction as appropriate, and assigns tasks according to State and federal regulations. Also provides required supervisory visits.
2. Documentation and Care Delivery
Provides high quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care, and in coordination with other members of the health care team.
Accurately and timely completes the comprehensive assessments (OASIS) including medication reconciliation.
Accurately and thoroughly documents patient visits per policy, and payer requirements and syncs timely per LHC policy.
3. Quality
Makes the initial and/or comprehensive nursing evaluation visit, ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source, accurately determines primary focus of care, develops the plan of care within State specific guidelines with the physician, and submits accurate documentation.
Communicates relevant information timely and effectively with appropriate agency staff including but not limited to: any patient care issues or needs, visit assignments, dates of scheduled visits, and schedule changes to scheduler, orders and OASIS data sets, coding requests, schedule home visits, to coordinate care with other clinicians, Communicates timely and effectively with physicians, patients, and family members to ensure quality care and service excellence.
4. Professionalism (on time, completes training, compliant)
Comes to work each day with a positive attitude, willing to help the patient and agency in any way they can. Is open to learning new skills.
Is punctual for all meetings and actively participates in all meetings including case conferences. This includes having a camera on for video meetings.
5. Teamwork (on call, productivity)
Takes direction from Clinical Director and Executive Director professionally and completes tasks assigned timely, including required learning. This includes all other duties as assigned.
Assists in the orientation of new agency personnel and serves as a preceptor to other staff and students. Actively participates in the survey/survey readiness activities, performance improvement plan including but not limited to: constantly works to reduce unnecessary patient hospitalizations, improve patient safety, and implements processes and best practices to ensure positive patient outcomes.
Schedule flexibility is needed in this role to ensure we provide care to the patient as needed. Requires the ability to participate in a weekend rotation and potential availability on observed holidays to ensure we are meeting the demands of our patients.
Adheres to and participates in the agency's utilization management model.

The Team

Why You? - Basic Qualifications
6+ years of experience in ad tech, metadata architecture, digital marketing technology, CDP, or campaign ops.
Proven expertise with metadata frameworks, taxonomies, data classification, or audience structures.
Understanding of AI/agentic automation and its application in ad tech or marketing operations.
Strong collaborator able to drive alignment across technical and non-technical stakeholders.
Preferred Qualifications
Experience with Tealium, SFMC, Orbit, and enterprise identity/metadata systems.

Clinic Summary
The Duke Eye Center Labs procedural clinic is located in the Duke Eye Center Hudson Building 2351 Erwin Road, Durham, North Carolina 27710 and supports Duke Providers as they perform intravitreal injections and/or laser photocoagulation for patients with retinal disease. Outfitted with state-of-the-art technology and equipment, and the latest medications and intraocular implants. World-class Duke Ophthalmologists treat patients for diseases including diabetic retinopathy, age-related macular degeneration, inherited retinal degenerations, and vascular occlusions.
Job Summary
This position is part of a team of ophthalmic personnel skilled in assisting Duke Eye Center providers and other technical support colleagues primarily by preparing patients for intraocular injections. High level disinfection (HLD) of instruments, retrieving of special prepared medications from Duke pharmacy, and retrieving/stocking of supplies are some of the secondary job tasks of this position.
Schedule:
Mon - Fri - 2/8 hours, day shifts
No weekends, holidays, or call.
Job Duties
Perform preliminary screening examinations for physician review including, but not limited to, ocular motility tests; tonometry tests to determine intraocular pressure and pupil testing for size, equality and reaction prior to dilation.
Assisting the medical and nursing staff during examination and procedures.
Disposing of contaminated supplies, instrument processing, high level disinfection and sterilization of medical instruments
Preparing patients for intraocular eye injections by instilling eye drops and preparing medication for administration
Assisting the physician with documentation and assuring the patient and family members are instructed on post procedure care and making appointments
Direct the flow of patients examined by an Eye Center physician; obtain medical and personal data for patient charts; screen patients for conditions, alleviate patient and family concerns by explaining procedures and method of treatment.
Sterilize and set up instruments for ophthalmic procedures, intraocular injections, and laser treatments.
Assist the physician in the preparation of routine microbiological specimens and in various research studies. Perform other related duties.
Minimum Qualifications
Education
The position requires completion of Ophthalmic Technician training and certification as a certified Ophthalmic Technician by the Joint Commission on Allied Health Personnel in Ophthalmology (JCAHPO).
One of the below certifications preferred:
Certified ophthalmic technician (COT)
Certified ophthalmic assistant (COA)
Certified ophthalmic medical technician (COMT)
And must have Basic Life Support (BLS) CPR Certification
