This role is responsible for implementation and evaluation of the education plans and programs developed from the annual educational needs assessment and guidance from the market/group and system nurse executive teams. Focuses on establishing collaborative relationships with operational leaders and staff on educational and professional nursing practice initiatives to meet educational needs and achieve expected outcomes related to key performance indicators, and system/nursing strategic plans. Supports the Nursing Excellence journey to achieve and maintain ANCC Magnet and Pathways to Excellence designation within assigned entities.
**Essential Job Functions**
+ This role will serve under the direction of the market practice and education leader to implement system nursing education and professional development programs, as well as support evidence based practice and nursing research efforts to ensure consistent and effective execution across assigned departments and facilities. This includes but not limited to: orientation and onboarding, RN Residency, preceptor program, etc.
+ Uses critical thinking, and applies the nursing process, principles of adult learning, teaching/learning theories and age appropriate teaching strategies to the implementation of educational programs.
+ Responsible for staff education and preceptor support and development including coaching/mentoring for assigned departments.
+ Understands principles for learning design and integrates learning technology to ensure effective learning experiences, competency assessment and validation and reflect practice changes, regulatory agency or hospital competency validation requirements.

Job Summary:
In collaboration with the cardiopulmonary team, the Exercise Physiologist II is responsible for developing, implementing, and evaluating the exercise component of the program and for reviewing patient history, presenting lectures, and performing exercise prescriptions. Monitors the patient's response to therapy and makes recommendations to change or modify based on the assessment. Serves as preceptor to new team members.
Essential Functions:
Review medical history, nursing intake and exercise history/ETT prior to goal setting session. Becomes familiar with patient profiles, with special attention to musculoskeletal function and cardiovascular histories. Calculate target heart range and risk stratification class, when appropriate, prior to six-minute walk test
Educate patient in self-monitoring target heart rate (THR) and rate of perceived exertion (RPE)
Write exercise prescription for clinic based on results. Document patient response to exercise, heart rate, ECG rhythm and any signs and symptoms a patient may be having.
Perform six-minute walk test or sub max exercise tolerance test during initial evaluation and discharge and documents findings
Provide patients with guidelines for exercise to be followed at home
Document on patient Individualized Treatment Plan (ITP) every 30 days; reassessment of exercise goals, response to exercise, discharge planning, and ongoing education of the patient.
Will be directly involved with and act as site supervisor for Clinic Student University Internship program.

Mercy Health is looking for a full time Polysomnographic Trainee for third shift position in Sleep Center. Trainees work under the direct supervision of a Registered Polysomnographic Technologist to develop competency in and perform comprehensive evaluation and treatment of sleep disorders, including polysomnographic diagnostic and therapeutic services, patient care, and education.
Job Duties:
Performs diagnostic testing for sleep disorders including Home Sleep Studies (HST), Baseline Diagnostic Polysomnogram (PSG), Positive Airway Pressure titrations (CPAP, BiPAP, iVAPs, ASV), Multiple Sleep Latency (MSLT), titration of oral devices for OSA, and supplemental oxygen titration.
Demonstrates organizational, computer, interpersonal, and technical skills to complete quality clinical procedures and excellent patient care.
Assists with physician clinics and facilitates treatment compliance with patient education, mask fittings, and desensitization sessions, as needed.
Performs studies utilizing the standard 10-20 system of electrode placement.
Assures cleanliness and safety of patient care, technical, supply, and storage rooms in the Sleep Center.
Maintains proper cleanliness, care for, and functioning of sleep study equipment and supplies.
Ensures accurate registration, payment collection, and charging for patient encounter in patient's electronic medical record (EMR).
Reviews patient chart including physician order, face to face, history and physical, medications, comorbidities, and prior patient sleep encounters to confirm accuracy of order, appropriateness of testing, and identify potential issues & effects of patient health on sleep study.
Reviews orders and performs appropriate sleep disorders testing according to facility policy & procedures as well as American Academy of Sleep Medicine, state and federal government, and accrediting body standards & regulations.
Accurately completes all workflows, charting, scoring, documentation, and necessary scanning in the patient EMR and sleep study system for each patient encounter.
Assists with scheduling of patient visits & procedures, reminder calls, filling vacancies from cancellation list, and proper tracking of patient reschedules, cancellations, and no shows in the EMR.
Promptly reports any facility cleanliness, maintenance problem, safety hazard, equipment malfunction, supply issue, patient complaints, patient or staff injury, security concern, infection control issue, etc. to management and the appropriate supporting department.
Completes all required continuing education in compliance with credentialing body, Sleep Center and hospital accrediting bodies, and BSMH policy.
Performs other tasks deemed necessary by manager to facilitate customer service or business operations including shift, work schedule, department, or location change.

**Job Summary:**

**Job Summary:**
The Licensed Practical Nurse (LPN) is responsible for the delivery of patient care under the direction of the Physician. Job Summary:
Coordinates and schedules patient testing or procedures, including interacting with physician offices, other health care providers and staff in departments that provide services to ensure patients are scheduled as soon as possible. Obtains scheduling orders, instructs patients, and enters data into the hospital computer system. Gathers, updates, and maintains daily records of scheduled patients. Demonstrates knowledge of provider and non-provider payers and communicates to patient, physician, precertification, and other appropriate departments. Maintains statistical data for department supervisor as requested to determine scheduling volume, patient types, and number procedures.
Essential Functions:
Gathers and interprets relatively complex patient data from physician offices and/or patient daily to enable scheduling of testing, surgical procedures, and pre-certification when required. May be responsible for pre-registration of self-scheduled patients.
Provides physician office staff and/or patient with hospital approved instructions regarding test preparation. Answers questions pertaining to testing, scheduling, and testing preparation. Refers patient to specialty area staff instructions, which are outside of the ordinary.
Enters patient scheduling information into computer, updates patient demographic and insurance information and provides pre-registration forms which include testing information for registering the patient prior to testing.
Used as a resource in resolving testing/ procedures discrepancies and is used as a resource in finding solutions to problems with patient testing and scheduling. Obtains approval from department designed contract and/or supervisor for non-routine appointment scheduling.
Checks for medical necessity for patients at time of scheduling.
Works with departments to schedule stat patients. Coordinates scheduling with multiple facilities or departments to ensure outpatients are scheduled as soon as possible.
Compiles and prepares statistical data as requested.
Responsible for performing all other duties, such as filing, photocopying, and relaying data as assigned.
Promotes strong customer focus and handles all calls with professionalism.

Overview
Responsible for contributing to in-depth investigations for suspected fraud or abuse with respect to provider, pharmacy, employer, member, and broker interactions involving the full range of products at Sentara Health Plans. Responsible for contributing to the review of the quality of pharmacy, physician, ancillary and hospital based coding in routine desk audits as well as occasional on-site audits. Contribute to the review of reimbursement systems relating to health insurance claims processing and ensures adherence to Optima Health policies and procedures for its various product offerings. Specific progression of responsibility is a follows dependent upon education, certifications, and experience:
Conducts investigation-related training.
Negotiates settlement agreements to resolve disputes.
Maintain current knowledge of relevant laws, regulations and standards.
Updates department policies and procedures and assists in training staff on changes.
Prepares routine department reporting as needed.
Education
Bachelor's Degree REQUIRED; Degree in a related field of study preferred.
Certification/Licensure
Certified Professional Coder REQUIRED (or achieved within 12 months of hire date)

Key Responsibilities
· Manage/support the development, implementation, and maintenance of records management policies and procedures.
· Develop and provide training and guidance to staff on records management practices, policies, and tools.
· Assist business departments in creating systems that allow for accurate categorization, filing, and indexing of records for easy retrieval.
· Perform regular audits of records to ensure compliance with legal, regulatory, and organizational standards for maintenance, retention, and destruction.
· Provide support for the preparation, transfer, and storage of records, ensuring proper documentation is maintained throughout the process.
· Support the identification and retention of records in accordance with applicable organizational retention schedules.
· Support implementation of electronic records management systems and other technology solutions.
· Respond to internal and external inquiries related to records and information retrieval, in conjunction with other internal departments, as appropriate.
· Ensure that records that have reached a destruction point are disposed of in a secure manner.
