* This position has a sign-on bonus of $3,000 along with a $500 referral fee for other LPN/RNs you refer for the company* Hourly rate: $26-$32/hr The Outpatient LPN provides intensive, community-based services to individuals with serious mental illness. The LPN plays a key role in supporting the physical and behavioral health needs of our consumers through direct care, medication management, and health education. This position is ideal for a compassionate nurse who thrives in a collaborative, field-based environment and is committed to recovery-oriented care.
Key Responsibilities:
* Medical Oversight and Coordination
* Conduct health assessments and monitor vital signs during home and community visits
* Coordinate medical appointments and follow-up care with primary and specialty providers
* Assist with lab coordination and specimen collection when appropriate
* Maintain medication inventory and ensure proper storage and handling
* Ensure compliance with HIPAA, ADMH, and agency policies
* Document all services accurately in the electronic health record (EHR)
* Participate in quality improvement initiatives and required trainings
* Medication Management
* Administer prescribed medications and monitor for side effects or adverse reactions
* Educate consumers on medication adherence and chronic condition management
* Support medication reconciliation and adherence tracking
* Collaborate with prescribers to report concerns or changes in consumer health status
* Team and Consumer Support
* Participate in daily ACT Team meetings and contribute to individualized treatment planning
* Support crisis intervention and stabilization efforts in the field
* Provide outreach and engagement to consumers who may be reluctant to participate in services
* Collaborate with psychiatrists, nurses, therapists, case managers, and peer specialists
* Promote wellness and recovery through education and supportive care
* Build trusting relationships with consumers to encourage participation in services
* Documentation and Compliance
* Accurately document all clinical interactions, interventions, and observations in the electronic health record (EHR).
* Ensure compliance with HIPAA, OSHA, and other regulatory standards.
* Participate in quality improvement initiatives and audits as needed.
* Maintain up-to-date licensure, certifications, and required training.
Qualifications:
* Current LPN license in the State of Alabama
* Nursing degree from an accredited institution.
* Minimum of 1-2 years of nursing experience (behavioral health or community-based care preferred)
* Valid driver's license and reliable transportation required
* Strong communication, organizational, and interpersonal skills
* Proficiency in Microsoft Office and EHR systems
* Ability to work independently and as part of a multidisciplinary team

Key Responsibilities:
* Medical Oversight & Consumer Health:

* Prepare Medication Administration Records (MARs) for new or transitioning residents.

* Consumer Care & Support:
* Support the physical and hygiene needs of residents as needed.
* Promote consumer rights, dignity, and independence.
* Assist in planning and implementation of health-related groups (e.g., nutrition, hygiene, wellness).

* Documentation & Compliance:

Qualifications:

This role involves providing direct medical care, supervising Medication Administration Certified (MAC) workers, and ensuring compliance with all medical protocols and best practices.
Key Responsibilities:
* Medical Oversight & Consumer Health:
* Monitor the medical condition of residents regularly and coordinate appropriate care under RN or physician guidance.
* Track and document vitals (blood pressure, pulse), weight, and other health metrics per program requirements.
* Maintain and audit medical records to ensure timely medical, dental, and psychiatric appointments.
* Respond to medical emergencies and provide necessary care or referral.
* Ensure infection control procedures are followed in all settings.
* Consult with RNs, physicians, and families regarding health issues, treatments, or changes in condition.
* Medication Management:
* Administer medications as prescribed and supervise MAC workers in proper medication administration procedures.
* Ensure compliance with medication protocols, including documentation, secure storage, and regular audits.
* Review medication logs daily for discrepancies or refill needs; update RN accordingly.
* Prepare Medication Administration Records (MARs) for new or transitioning residents.
* Provide training and oversight to direct care staff regarding medication policies and procedures.
* Supervision of MAC Workers:
* Provide ongoing supervision, support, and training to MAC-certified personnel to ensure adherence to medication administration standards.
* Conduct periodic checks and competency evaluations of MAC workers.
* Serve as a resource for medication-related questions or concerns.
* Consumer Care & Support:
* Support the physical and hygiene needs of residents as needed.
* Promote consumer rights, dignity, and independence.
* Assist in planning and implementation of health-related groups (e.g., nutrition, hygiene, wellness).
* Participate in team-based care planning and implementation of behavior or support plans.
* Documentation & Compliance:
* Complete all required documentation, including progress notes, incident reports, and daily logs, in a timely and accurate manner.
* Maintain compliance with all licensing, regulatory, and agency policies.
Qualifications:
* LicensedPracticalNurse (LPN) in good standing with the state of employment.
* Nursing degree from an accredited program.
* Experience with individuals with ID/DD, behavioral health needs, and medically fragile conditions preferred.
* PCM Practitioner or Level 2 (as required by site) certification; MAC supervision experience a plus.
* Strong clinical judgment, documentation, interpersonal, and organizational skills. Ability to work independently across multiple sites.

* Immediately report any vehicle issues, accidents, or violations to appropriate personnel.

Work Environment & Schedule:
* This is a non-exempt, full-time position.
* Standard hours are Monday through Friday, 8:00 AM - 4:30 PM CST.

A Emergency Department Nurse is a highly specialized health care professional who cares for a range of medical emergencies and clinic type patient care in the hospital environment for the purpose of recognizing, stabilizing and planning patient care activities and based on acuity of patients.
Some of the many skills performed
* Follow Advanced Cardiac Life Support (ACLS) guidelines
* Follow Pediatric Advanced Life Support (PALS) guidelines
* Follow Basic Life Support (BLS) guidelines
* Orthopedic splinting
* Provide advanced cardiac care, including:
o Manual defibrillation;
o Synchronized cardioversion;
o Transcutaneous pacing;
o Perform Electrocardiogram monitoring, including 12-lead ECG;
* Establish vascular access for medication administration and fluid resuscitation via several routes:
o Peripheral intravenous cannulation;
o EJ intravenous cannulation (with competency)

* Place a nasogastric tube and perform gastric suctioning;

* Administer a variety of medications.
* Utilizes the Emergency Severity Index (ESI) for patient triage
