The Community Relations Manager will oversee the community relations team and the organization's corporate volunteer program. It will ensure that the community relations team members are meeting outreach and fundraising goals and will manage the organization's corporate, community, and event volunteer program. This position will also collaborate with colleagues and leadership to identify and strengthen corporate relationships in support of Easterseals Midwest's mission. Essential Functions: * Collaborates with organizational staff to identify opportunities for grassroots community outreach, fundraising, and advocacy efforts. The Crisis Services Manager is responsible for managing staff and program services for individuals who meet the criteria for the Crisis Services Program. This position manages supports for individuals in critical need situations while ensuring quality standards. The Crisis Services Manager supports staff and the needs of the individuals' receiving services; ensures individual, family/guardian and funder satisfaction; and monitors budgetary requirements. Ensures program services comply with agency guidelines, funder requirements and accreditation standards. Essential Functions: * Provides oversight and direction to staff regarding Crisis Services Program services, medical care, and behavior support. Ensures team members are informed on the support needs of each individual prior to them entering supports. Provides training and support on generalized behavioral supports and plan implementation to ensure positive outcomes. * Ensures that services meet quality, productivity and organizational/funder requirements, and ensures stakeholder satisfaction for services provided. * Communicates changes in individual functioning, health status or behavior to Crisis Services team, doctors, service coordinators and other providers as required. * Attends meetings with referrals to provide information on services and determine if program is a good fit for the individual. The Health Home Care Coordinator is responsible for gathering and processing information for the Health Home program from individuals, providing information and referrals for services, maintaining and monitoring program data, and reporting program-related data. Ensures program outcomes and objectives are being met, and ensures services provided meet organizational guidelines, funder requirements and accreditation standards. This position fulfills the Health Home Facilitator role per DMH Health Home guidelines. Essential Functions: * Gathers, researches and processes Health Home program information from individuals for program enrollments, transfers and discharges. Responsible for ensuring all required information is obtained, verified and entered within established timeframes. Ensures all state requirements and internal standards are met before submitting information to DMH. * Accesses multiple hospital data portals and reports to monitor program individuals' admissions and discharges. Obtains information, creates reports and communicates hospitalization information to nursing staff daily. * Extracts metabolic screening values from electronic records and enters that information into DMH and internal systems. Monitors upcoming due dates for screens and communicates that information to nursing staff. * Answers questions regarding the Health Home Program services. Essential Functions: * Assesses individuals' abilities, skills and interests to determine job readiness and promote successful employment. * Assists individuals throughout the employment process using a variety of resources and curriculum instruction. * Identifies potential accommodations that would support individuals on the job. * Provides on-the-job supports through observation, modeling, and implementation of individualized tools as well as provides follow-up services to ensure employment retention. * Communicates with the individual and worksite supervisor to ensure success on the job. * Manages caseload and schedules services ensuring that assigned services are being delivered while maintaining productivity expectations. * Performs all required documentation and ensures compliance with organizational, funder and accreditation requirements. * Attends meetings with individuals and support team members to discuss Individual Support Plans (ISPs) and review supports and services provided. * Establishes and builds positive relationships with businesses and community partners. Researches potential employers/community partners and develops relationships that lead to volunteer and/or job opportunities. * Participates in staff meetings, departmental trainings, and opportunities for ongoing growth and development. Qualifications: * High school diploma or equivalent required. College coursework in human services or business-related field preferred. * Knowledge of individuals with disabilities required. * At least one year of experience working with individuals within the human services field preferred. * Experience working in employment services preferred. * At least one year of experience working in customer service or sales preferred. * Basic computer skills and knowledge of Microsoft Office suite required. Additional Requirements: * This position may work day, evening, night and/or weekend hours depending on caseload. Schedules can vary from week to week according to the needs of the organization. * This position requires driving personal vehicle and may occasionally use company vehicle. Transports individuals to employers, volunteer/internship sites, individuals' homes and office location. * Must have reliable vehicle and maintain current driver's license and proof of insurance. * Must complete and maintain all required trainings/certifications. * Must maintain confidential information in accordance with HIPAA regulations.
