The Community Living Nurse is responsible for providing nursing services that meet requirements of DMRDD community RN program to clients in the Medicaid Waiver program. He or she will determine clients' level of need for RN services. Duties: * Conducts monthly face-to-face quality assurance visits to assigned residential sites to assess client's healthcare, behavior and psychological needs. Provides immediate written feedback. * Be available to assigned homes via agency phone consistently each week starting at 8am on Monday and ending at 5pm on Friday. Answer calls and respond to texts within 20 minutes. * All nurses should participate in a weekend and holiday on-call rotation. * Ensures that Community RN requirements are met as outlined by the Department of Mental Health and agency policy. * Complete community RN monthly health summary to include recommendations to client and staff. Submit a copy to Manager and administrative assistant. * Inform Director, Managers and Supervisors of client health status changes or deficiencies in the standard of care provided to the client. Any action steps identified during home visits should be resolved immediately. * Ensure that hours provided for the Community RN program services remain distinct and separate from any other hours the nurse may be employed for with the agency. * Maintains current certifications in CPR, PBS (online training), Abuse and Neglect, Community RN training, and other trainings as required.
